Resumo: This is a qualitative study based on the testimony of directors of organizations addressing HIV/AIDS and people living with HIV/AIDS in the Dominican Republic. Although the manifestations of stigma and discrimination related to HIV/AIDS have decreased in the Dominican Republic in comparison to past decades, ignorance and lack of supervision from government authorities foster that people with HIV do not receive adequate health services and the protection of their right to employment. Some groups are more affected than others as a product of the reinforcement of existing prejudices in the Dominican society, such as antihaitianism. Facing the changes in the spread of the epidemic I propose some reflections on gender and HIV, and suggest how to reinforce existing positive initiatives and the adoption of new ones.

Este es un estudio de carácter cualitativo, basado en testimonios de dirigentes de instituciones dedicadas a la prevención del VIH y personas viviendo con VIH en la República Dominicana. Aunque la manifestación del estigma y discriminación ha mejorado en la República Dominicana respecto a décadas anteriores, la ignorancia junto con la falta de supervisión de las autoridades hace que a muchas personas portadoras se les prive todavía de la adecuada atención en salud y el derecho al empleo. Unos grupos son más afectados que otros como producto del reforzamiento de los prejuicios existentes en la sociedad dominicana, tales como el antihaitianismo. Ante los cambios de las vías de expansión de la epidemia planteo algunas reflexiones sobre género y VIH, y sugiero cómo reforzar las iniciativas positivas existentes y adoptar nuevas soluciones.

Resumo: Objetivo: Conocer si existe relación entre el crecimiento estatural y el uso de corticoides en pacientes pediátricos con diagnóstico de Síndrome Nefrótico, atendidos en el Servicio de Nefrología del Instituto Nacional de Salud del Niño por un periodo de tres años, entre el 2003 y 2009. Materiales y Métodos: Se evaluó 70 historias de pacientes de Consultorio externo del Servicio de Nefrología del Instituto Nacional de Salud del Niño, con diagnóstico de Síndrome nefrótico, que tuvieran una evolución de tratamiento con corticoides de 3 años. Se realizó un estudio de tipo retrospectivo descriptivo, y se evaluaron las desviaciones estándar de la talla para la edad. Resultados: De un total de 70 pacientes en estudio se observo que el 62.9 por ciento eran de género masculino y 37.1 por ciento de género femenino. El promedio de edad de inicio de tratamiento fue de 6.6 años. El 56 por ciento de pacientes pertenecio al grupo II (4-6 cursos de corticoides), el 40 por ciento pertenecio al grupo I (1-3 cursos de corticoides), 3 por ciento fueron pacientes considerados en el grupo III(7-9 cursos de corticoides) y un 1 por ciento en el grupo IV(igual o mayor a 10 cursos). En el grupo I se observó que al inicio del tratamiento, presentó una desviación estándar (DS) promedio de - 1,8; en el primer control obtuvo una -1.2 DS, al segundo control y tercer control se encontraron valores de -1,1 en ambos casos; todos estos valores dentro de los rangos normales de crecimiento para la edad (-2DS a + 2DS). Los pacientes del Grupo 11, al inicio del tratamiento presentaron una OS de -1,57, en el primer control se encontró que el promedio fue de -1,63 OS, al segundo año se encontró que el promedio de OS fue de -1.59 y al tercer año de -1.54. Valores dentro del rango normal de crecimiento para la edad (-2DS a + 2DS)...

Objective: To determinate whether a relationship exists between the growing stature and the use of corticosteroids in pediatric patients diagnosed with nefrotic syndrome treated at the Nefrology Service of the National Institute of Child Health for a period of three years, between 2003 and 2009. Materials and Methods: We evaluated 70 patients stories outpatient c1inic of the Nephrology Department at the National Institute of Child Health, diagnosed with nephrotic syndrome that had an evolution of the treatment with corticosteroids for three years. We did a descriptive retrospective study and evaluated the standard deviations of height for age. Results: Of a total of 70 patients under study was observed that 62.9 per cent were male and 37.1 per cent were female. The average age at the beginning of the treatment was 6.6 years. A 56 per cent of the patients who belonged to group 11 (4-6 courses of corticosteroids), 40 per cent belonged to group I (1-3 corticoids courses), 3 per cent of the patients were considered in group 111 (7-9 courses of corticosteroids) and 1 per cent in group IV (more or equal to 10 courses). In group 1, was observed that at the beginning of the treatment, it presented a standard deviation (SD) averaged -1.8; in the first control, it obtained a -1.2 SD, in the second and third control, in both cases it obtained - 1,1; all of these values within the normal range for age growth (-205 a + 205). The patients in Group 11, at the beginning of the treatment showed DS of 1.57. The first control showed that the average was -1.63 DS, in the second year it was found that the average was -1.59 DS and in the third year was found an average of -1,54; all of these values within the normal range for age growth (-2 SD to + 2 SD)...

Resumo: Three hundred individuals at a Mexican city responded to Zimbardo's Time Perspective Inventory (ZTPI), and self-reported how frequently they engaged in water conservation practices. The ZTPI assesses individual differences in terms of attitudes believed to identify orientations towards a positive or negative past, hedonistic or fatalistic present, and future orientations. Results were processed within two structural equation models, which showed that present orientation negatively affected water conservation. Water conservation did not correlate with past orientation. Yet, that pro-environmental behavior significantly and positively was influenced by Future Orientation. Women reported a higher involvement in water conservation practices, whereas adult individuals (> 18 years old) and those with higher schooling levels presented a higher Future Orientation. Proposals considering these results are discussed aimed at developing sustainable attitudes and behaviors.

Trecentos mexicanos contestaron a los reactivos del Inventario de Perspectiva Temporal de Zimbardo (IPTZ) y auto-reportaron sus conductas de ahorro de agua. El IZPT mide diferencias individuales en t‚rminos de orientaciones hacia un pasado positivo o negativo, un presente fatalista o hedonista y tendencia hacia el futuro. Los resultados se procesaron en dos modelos de ecuaciones estructurales, revelando que la orientaci¢n al presente afecta negativamente al ahorro de agua. Este ahorro no se correlacion¢ con la orientaci¢n al pasado, pero s¡ lo hizo positivamente con la orientaci¢n al futuro. Las mujeres reportaron ahorrar m s agua, mientras que los adultos (> 18 a¤os) y las personas de mayor escolaridad se plantearon con una mayor orientaci¢n hacia el futuro.

Título: Bariatric surgery patients: reasons to visit emergency department after surgery

Resumo: STUDY OBJECTIVE: Morbid obesity prevalence is reaching epidemic proportions in Western society. Long-term weight loss can be achieved by bariatric surgery. This surgery also has a positive impact in the reduction of obesity related co-morbid conditions. The purpose of this study is to determine the reasons that bariatric surgery patients had to visit the emergency department within a three month period after surgery. METHODS: A retrospective chart review study was performed at the UPR Hospital in Carolina. Patients with the diagnosis of morbid obesity who had bariatric surgery were identified. Of the 283 patients who met the criteria, the following information was obtained: gender, age, height, weight, pre-operative BMI, obesity-related comorbid conditions, post operative length of stay (LOS), and reasons and length of stay of Emergency Department (ED) visits within a 3 month period after surgery. Statistical analysis was done with Statistical Package for Social Sciences (SPSS) Program. RESULTS: The same profile of gender and BMI was obtained between the population that had the surgery and the sample that visited the ED, the group of patients between 20-29 years old was more likely to visit the ED. No correlation was found between a longer post operative length of stay and an increased probability of visiting the ED. Of the population, 5% had to visit ED within a three month period. The most common post operative complications were: Abdominal Pain (46.2%), Emesis (38.5%), and Dehydration (30.8%). Other less frequent complications were nausea, DVT, pneumonia, dizziness, gastritis, infected wound and upper GI bleeding. CONCLUSION: The most common reasons that bariatric surgery patients had to visit the emergency department within a three month period after surgery were: abdominal pain, emesis, dehydration and nausea. Título: Descriptive study of cerebrovascular accidents among Hispanic population

Resumo: BACKGROUND AND PURPOSE: Data characterizing stroke patients among Hispanic population are scant. The aim of this study was to describe this population and to assess baseline knowledge of stroke type, risk factors, and family history, among others. METHODS: A retrospective chart review of 253 stroke patients admitted to the University of Puerto Rico Hospital during the fiscal year July 2002 to June 2003 was done. A standardized data collection form was used to obtain the following information from patient records: patient age, gender, stroke type, time of symptom onset, patient risk factors, family risk factors, and patient outcome. RESULTS: We enrolled 253 patients, 120 (47%) males and 133 (53%) females. Patient age distribution was as follows: 18 (7%) were < 44 years, 79 (31%) were bwtween 45-64 years, 156 (62%) were > 65 years. Stroke type distribution was: ischemic 174 (69%), hemorrhagic 49 (19%), and transformation from ischemic to hemorrhagic 13 (5%). Ischemic stroke subtypes distribution was: large-artery occlusion 113 (65%), small-artery occlusion 60 (34%), cardio-embolism 1 (0.6%), 17 (7%) information was not recorded. Time of symptom onset: 153 (60%) > 3 hours, 14 (6%) < 3 hours, and 86 (34%) was unknown. Family history of risk factors was present in 103 (41%), not present 29 (11%) and 121 (49%) was unknown. Most common patient risk factors were: hypertension 212 (84%), > 65 years 156 (62%), male gender 120 (47%), diabetes mellitus 105 (42%), previous history of stroke 91 (36%), hyper-cholesterolemia 44 (17%), history of smoking 51 (20%), alcoholism 43 (17%). CONCLUSION: The data obtained from this Hispanic population is similar to that reported in the literature for the general population. The prevalence of ischemic strokes out numbered by far the occurrence of hemorrhagic strokes in the study group. Resumo: Cardiovascular disease is the leading cause of death for adults 40 years of age or greater in the United States and in Puerto Rico. There are approximately 165,000 out-of-hospital cardiac arrest (OHCA) events every year in the US. Since more than 30 years cardiopulmonary resuscitation (CPR) skills have saved thousands of lives. The majority of cardiac arrest events occur out of the hospital and without early access to CPR and a defibrillator these patients don't have a chance of survival and the majority will die before reaching the hospital. The development of CPR in the 1960's and the invention of automated external defibrillators (AED) in the 1970's revolutionized the means to approach OHCA victims. For the last 10 years the deployment of AEDs in public locations has been a successful model addressing this major public health concern. This article reviews the role AEDs play in the early management of OHCA victims and why public access defibrillation (PAD) programs are necessary.

Resumo: Actually recommended by the International Liaison Committee on Resuscitation (ILCOR), therapeutic hypothermia has been the only modality shown to improve neurological outcomes after cardiac arrest. This article provides the background for such recommendations as well as: pathophysiology, review of brain injury during cardiac arrest, discussion of beneficial effects of therapeutic hypothermia, different mechanisms used to induce it and the latest clinical trials results.

Título: Can etomidate have antiarrhythmic properties?

Resumo: Etomidate is a drug commonly used for Rapid Sequence Intubation (RSI) and Procedural Sedation Anesthesia (PSA) in the Emergency Department because of its rapid onset of action and low cardiovascular risk profile. The antiarrhythmic effects of etomidate are presented in a patient with unstable wide complex tachycardia, which converted to sinus rhythm immediately after its administration. This is the first case in the Emergency Medicine literature and the second case reported of possible antiarrhythmic effects of etomidate since its development.
