[Ab] Resumo:Duodenal gastric heterotopia (DGH) is a common incidental finding at endoscopy, generally assumed to be congenital in origin. A recent study has suggested a possible association with gastric fundal gland polyps (FGPs). In this review of 37 patients with histologically confirmed DGH, the background to our present understanding of the DGH lesion is reviewed, the clinical and pathological associations of the condition in our cohort are identified, and the histological and immunohistochemical features of the DGH lesion are described. An association with the presence of FGPs is again demonstrated, whereas an inverse relationship with Helicobacter pylori is also shown. Immunohistochemical staining demonstrates that the fundic-type glands of DGH express the proton pump antigen (H(+)K(+)ATPase), whereas the overlying surface mucosa expresses the mucin profile of normal gastric-type mucosa (mucin-5AC positive, mucin-2 negative). DGH may represent a further component of the iatrogenic hypergastrinemia-related hypothesis for FGP development, although further study is required to confirm this.

[Ab] Resumo:BACKGROUND: Gastric cancer is one of the most common and lethal malignant cancers worldwide, and numerous epidemiological studies have demonstrated that Helicobacter pylori (H. pylori) infection plays a key role in the development of gastric carcinomas. Our previous studies showed that aquaporin 3 (AQP3) is overexpressed in gastric carcinoma and promotes the migration and proliferation of human gastric carcinoma cells, suggesting that AQP3 may be a potentially important determinant of gastric carcinoma. However, the role of AQP3 in H. pylori carcinogenesis is unknown. METHODS: The AQP3 protein and H. pylori were detected in human gastric tissues by immunohistochemistry and modified Giemsa staining respectively. AQP3 knockdown was obtained by small interfering (si) RNA. Western blot assays and RT-PCR were used to evaluate the change of AQP3 in the human gastric cancer AGS and SGC7901 cell lines after co-culture with H. pylori. Sprague Dawley rats were orally inoculated with H. pylori to establish a rat model colonized by H. pylori. RESULTS: The present study found that AQP3 expression correlated with H. pylori infection status in gastric cancer tissues and corresponding normal mucosa, and H. pylori co-culture upregulated AQP3 expression in human gastric adenocarcinoma cells in vitro via the extracellular signal-regulated kinase signaling pathway. H. pylori infection also increased AQP3 expression in gastric mucosa colonized by H. pylori in a Sprague Dawley rat model. CONCLUSIONS: These findings provide further information to understand the mechanism of H. pylori carcinogenesis and a potential strategy for the treatment of H. pylori-associated gastric carcinoma.

[Ab] Resumo:BACKGROUND: Blood serum and peritoneal fluid acute-phase proteins, oxidative stress indicators, and some enzymes could be used for evaluation of abomasal tissue damage because of displacement in displaced abomasum (DA) cases. OBJECTIVES: The aim of this study was to investigate the concentrations of acute-phase proteins, oxidative stress indicators, and activities of enzymes in blood serum and peritoneal fluid in cattle with right displaced abomasum (RDA) and left displaced abomasum (LDA) and in healthy cows. ANIMALS: A total of 60 Holstein Friesian cows in early lactation were used, 31 with left and 9 with right displaced abomasum without volvulus diagnosis and no other postpartum disease, and 20 healthy cows as a control. MATERIALS AND METHODS: DA diagnosis in dairy cows consisted of physical examination, laboratory, and specific DA tests. Acute-phase proteins, oxidative stress indicators, and enzyme activities were measured in blood serum and peritoneal fluid. RESULTS: In the RDA group, serum haptoglobin (HPG), serum amyloid A (SAA), malondialdehyde (MDA), adenosine deaminase (ADA), myeleperoxidase (MPO), aspartate aminotransferase (AST), creatine kinase (CK, creatine kinase-MB (CK-MB), and gamma-glutamyl transferase (GGT) activity increased significantly, and serum HPG, MDA, ADA, and AST concentrations increased significantly in the LDA group (P < .05). Peritoneal fluid HPG, MDA, ADA, MPO, ALP, GGT, and LDH concentrations increased significantly, whereas NO concentrations reduced significantly in the RDA group, and HPG, MDA, ADA, and TP concentrations increased significantly, whereas concentrations of NO reduced significantly in the LDA group (P < .05). CONCLUSIONS AND CLINICAL IMPORTANCE: There are acute-phase responses, oxidative stress, and abomasal tissue damage because of displacement in DA cases. Especially, HPG, MDA, ADA, and MPO concentrations can provide specific information to help in understanding these changes.

[Ab] Resumo:Gastrojejunal intussusception is a rare complication of gastric surgery. It can occur in cases with gastrojejunostomy done after partial gastric resection or in cases of simple gastrojejunostomy. There are only few cases reported worldwide, Review of literature showed about 200 cases reported till 2006 and a few more cases reported after that. A rare case of retrograde gastrojejunal intussusception is reported. The patient presented in the emergency with severe pain upper abdomen and haematemesis. Contrast enhanced CT scan of abdomen showed hugely distended stomach with retrograde gastrojejunal intussusception on laparotomy, the intussusception was reduced. A jejunojejunostomy was done. The patient is doing well after 1 1/2, months.

[Ab] Resumo:CASE HISTORY: A 10-year-old Friesian dairy cow presented with a history of weight loss, decreased milk production and fluid splashing on auscultation of the heart. Previous antibiotic therapy included oxytetracycline, marbofloxacin and penicillin. CLINICAL AND PATHOLOGICAL FINDINGS: Following a failure to respond to treatment, prescribed by the referring veterinarian for respiratory infection and septic arthritis, the cow was referred to Massey University Veterinary Teaching Hospital with suspected pericarditis. Clinical examination identified a predominantly left sided holodiastolic heart murmur with tachycardia and cording of the jugular veins. Cardiac ultrasound showed an enlarged aortic valve with turbulent blood flow and regurgitation, consistent with bacterial endocarditis. Blood culture demonstrated Paenibacillus spp. Due to the severity of the clinical signs and poor prognosis, euthanasia was performed and a post-mortem examination carried out. DIAGNOSIS: Traumatic reticuloperitonitis with abscessation of the reticulum and rumen wall, which progressed to a bacteraemia and presumptively caused endocarditis, endometritis, septic arthritis, and renal and myocardial infarcts due to emboli from the endocarditis lesions. CLINICAL RELEVANCE: New Zealand dairy cattle are at risk of traumatic reticuloperitonitis and due to varying presentations diagnosis can be complicated. Subsequent bacteraemia is common and in this case the novel bacterial species Paenibacillus was implicated as a potential pathogen.

[Ab] Resumo:In this study, the colonization and distribution of Helicobacter pylori (Hp) in patients with chronic gastric diseases were investigated and the relationship between the periodontal initial treatment and presence of Hp in oral cavity was examined to better understand the connection between Hp infection and chronic diseases. Primers for PCR amplification were designed according to ureC gene and cagA genes of Hp. Specimens were harvested from different sites of 96 patients with chronic gastric diseases and the specimens of dental plaques, gargles and dorsal mucosa were tested for Hp. The 96 patients were treated by bismuth triple therapy and among them, 52 subjects were additionally given periodontal initial therapy. The eradication rate of gastric Hp and oral Hp detection rate were determined 4 weeks and 1 year after the treatment. The results showed that the detection rates of oral specimens were in the order of dental plaques (82.3%), gargles (51.1%) and scrapings of dorsal mucosa of tongue (37.5%). One year after bismuth triple therapy or the triple therapy in combination with periodontal initial treatment, the eradication rate of gastric Hp was significantly higher in the combination treatment group than in group treated by the triple therapy alone (62.8% vs. 32.4%, P<0. 05). Moreover, the Hp detection rate was significantly lower in the combination group than in the group treated only with the triple therapy. We are led to conclude that Hp is present at various parts of oral cavity, oral Hp might be an important source of gastric Hp and the triple therapy plus periodontal initial treatment can enhance the long-term eradication rate of gastric Hp in patient with both chronic gastric diseases and chronic periodontitis.
