Título: Effect of soft tissue thickness on the morphology of lip in orthodontic treatment / Efecto del grosor del tejido blando en la morfología del labio en el tratamiento de ortodoncia

Resumo: The aim of this study was to evaluate the effect of soft tissue thickness of upper lip on lip retraction in orthodonticaltreated females and identify the ratio of maxillary incisor retraction to upper lip retraction. Pre- and post-treatment lateral cephalograms of 100 adults were examined to measure the lip thickness in upper lip and establish the classification standard. All subjects were treated with 4 first premolar extractions followed by upper central incisors retraction. Pre- and post-treatment lateral cephalograms of 19 patients were reviewed to determine the changes of the upper lip and incisor positions through landmarks displacement. An independent-samples t test and one-way analysis of variance were performed. The correlations between maxillary incisor retraction and upper lip retraction were explored by the Pearson correlation method. P-values<0.05 were considered statistically significant. The lip thickness of adult male patients was greater than that of adult female patients. The average ratio of maxillary incisor retraction to upper lip repositioning was 1.6:1,1.9:1 and 2.2:1 in the thin lips group, normal lips group and thick lips group, respectively. Gender differences exist in the thickness of upper lip. Horizontal changes of the maxillary incisor showed a significant correlation to horizontal changes of the upper lip (P<0.001).There were negative correlations between the thickness of upper lip and the ratio between change in maxillary incisor protrusion and change in upper lip protrusion.

El objetivo de este estudio fue evaluar el efecto del grosor de los tejidos blandos del labio superior, sobre la retracción del labio en mujeres tratadas con ortodoncia e identificar la proporción de retracción del diente incisivo maxilar con respecto a la retracción del labio superior. Se examinaron cefalogramas laterales, previos y posteriores al tratamiento de 100 adultos, para medir el grosor del labio superior y establecer un estándar de clasificación. Todos los sujetos fueron tratados con 4 extracciones de los primeros premolares seguidas de retracción de los incisivos centrales superiores. Se revisaron los cefalogramas laterales, previos y posteriores al tratamiento de 19 pacientes, para determinar los cambios del labio superior y las posiciones de los incisivos a través del desplazamiento de los puntos de referencia. Se realizó una prueba t de muestras independientes y un análisis de varianza de una vía. Las correlaciones entre la retracción del incisivo maxilar y la retracción del labio superior se exploraron mediante el método de correlación de Pearson. Los valores de p<0,05 fueron considerados estadísticamente significativos. El grosor de los labios de los pacientes adultos masculinos fue mayor que el de las pacientes adultas. La relación promedio de la retracción del incisivo maxilar al reposicionamiento del labio superior fue de 1,6:1,1; 9:1 y 2,2:1 en el grupo de labios delgados, grupo de labios normales y grupo de labios gruesos, respectivamente. Existen diferencias de sexo en el grosor del labio superior. Los cambios horizontales del incisivo maxilar mostraron una correlación significativa con los cambios horizontales del labio superior (P<0,001). Hubo correlaciones negativas entre el grosor del labio superior y la relación entre el cambio en la protuberancia del incisivo maxilar y el cambio en la protrusión del labio superior.

Resumo: Abstract: Ascher's syndrome consists of double lip, blepharochalasis and sometimes non-toxic thyroid enlargement. It is a rare, benign, rarely reported, and sometimes misdiagnosed condition that most often affects patients under 20 years old. The etiology remains unknown, although factors such as trauma, hormonal dysfunction and heredity have been suggested. Treatment based on surgical intervention is indicated to improve aesthetics or function. The presented case shows a patient diagnosed with Ascher's syndrome based on clinical findings such as double lip and blepharochalasis.

Resumo: Introdução: As fissuras labiopalatinas são malformações congênitas e, no Brasil, estima-se a ocorrência de 1:650 nascimentos. A classificação adotada é a de Spina. A queiloplastia e a palatoplastia são as principais cirurgias executadas. Métodos: Estudo retrospectivo descritivo com obtenção de dados a partir do sistema Smile Train Express referente a pacientes com fissura labiopalatina atendidos por equipe cirúrgica de referência entre 1 de março de 2014 e 1 de dezembro de 2016. Resultados: Foram identificados 477 pacientes, predominando o sexo masculino e os dois primeiros anos de vida na admissão. A fissura mais prevalente foi transforame e unilateral esquerda. O tratamento cirúrgico mais frequente foi a queiloplastia. Conclusões: O padrão epidemiológico está em consonância com a literatura nacional.

Introduction: Orofacial clefts are congenital malformations with an estimated occurrence of 1:650 births in Brazil. The most widely adopted classification system in that country is the method developed by Spina, and cheiloplasty and palatoplasty are the main surgeries performed. Methods: This was a retrospective descriptive study using data collected from the Smile Train Express organization regarding patients with orofacial clefts treated by a reference surgical team between March 1, 2014 and December 1, 2016. Results: A total of 477 patients were identified, predominantly male and in the first two years of life at admission. The most prevalent type of malformation was left unilateral transforamen cleft. The most frequent surgical treatment was cheiloplasty. Conclusions: The epidemiological pattern is consistent with the findings described in the national literature.

Título: Oral mucosa lesions as atypical manifestation of adult-onset Still´s disease

Resumo: Abstract: Adult-onset Still's disease is a systemic inflammatory disorder of unknown etiology, characterized by skin rash, spiking fever, arthralgias or arthritis, and leukocytosis. The typical skin rash is evanescent, salmon-pink, nonpruritic and maculopapular, predominantly on the extremities. It is considered one of the major Yamaguchi's criteria in adult-onset Still's disease. However, atypical skin lesions are also described. Here, a 61-year-old woman with sore throat, spiking fever, polyarthritis and evanescent salmon-pink nonpruritic maculopapular skin rash on the extremities was diagnosed with adult-onset Still's disease. In addition, atypical brown macules on oral mucosa, localized on the inner lips and tongue were also observed. Biopsy revealed a neutrophilic infiltrate. Despite treatment and improvement of the adult-onset Still's disease, the atypical oral mucosal lesions persisted.

Resumo: Abstract Plasmoacanthoma is an extremely rare verrucous tumor located on periorificial regions characterized by dense dermal plasmacytic infiltrates. Some authors classify it as a form of reactive plasma cell proliferation which represents a heterogeneous spectrum of mucocutaneous disorders. These plasma cell proliferations have been considered to be a benign immunologic inflammatory reaction to known or unknown stimuli. However, the etiology of plasmoacanthoma remains highly speculative. We report the case of a 40-year-old woman who presented with a lobulated warty lesion affecting the lower lip. Biopsy from the lesion was compatible with plasmoacanthoma, which remains an underreported disease in the dermatology literature.

Título: Oral phaeohyphomycosis in a patient with squamocellular carcinoma of the lip: second case report

Resumo: Abstract This communication reports the second known case of oral phaeohyphomycosis in a patient with squamocellular carcinoma of the lip. The patient, an 82-year-old black woman, a former smoker (for more than 30 years), suffering from an ulcerous vegetative lesion in the middle third of the lower lip for approximately 12 months. The result of the histopathological analysis indicated carcinoma, with well-differentiated keratinized squamous cells and the presence of septate mycelial filaments. In the direct mycological examination, thick and dematiaceous septate mycelial filaments were observed. After the resection surgery, the patient did not need to use an antifungal drug to treat the phaeohyphomycosis, and no follow-up radiotherapy was needed to treat the squamocellular carcinoma. We stress that the presence of the squamocellular lesion of the lip was a possible contributing factor to the infection.

Resumo: Introducción: El pénfigo vulgar es una enfermedad autoinmune caracterizada por ampollas suprabasales y autoanticuerpos IgG dirigidos contra la desmogleina 3 del desmosoma. El objetivo del trabajo es comunicar la casuística del Servicio de Dermatología del Hospital Nacional en un periodo de 20 años y determinar las características demográficas, clínicas y evolutivas. Material y Método: Estudio retrospectivo, observacional, descriptivo. Se incluyen pacientes de ambos sexos, de todas las edades, que son atendidos en el ambulatorio de Dermatología o en las salas de internación del Hospital y que tienen confirmación histológica. Resultados: En el periodo 1991-2011 se diagnosticaron 22 casos de pénfigo vulgar representando el 12,7% de los casos de pénfigo observados en el servicio. Predominó en el sexo femenino (13 casos), en el grupo etario de 30 a 49 años (15 casos), siendo la media de 42,1 años. La afectación de la mucosa oral se presentó en 18 casos y en 12 fue la inicial. El compromiso cutáneo observado en todos los casos, fue diseminado o generalizado (20 casos). Dos fueron de la variedad vegetante. El tratamiento más utilizado fue la prednisona sola (8 casos) o asociada con azatioprina (8 casos). La evolución fue favorable en 19 casos con curación o remisión. Dos fueron al óbito y uno no recibió tratamiento. Conclusión: El pénfigo vulgar en nuestro servicio es una patología poco frecuente, pero con importante morbimortalidad. Es el segundo tipo en frecuencia, siendo el foliáceo el más observado.

Introduction: Pemphigus vulgaris is an autoimmune disease characterized by suprabasal blisters and IgG auto antibodies directed against desmoglein 3 of the desmosome. The aim of this work is to communicate the casuistics of the Dermatology Service of the National Hospital over a period of 20 years and determine the demographic, clinical and developmental characteristics. Material and Methods: Retrospective, observational, descriptive study. It includes patients of both sexes, all ages, in treatment at the dermatology outpatient or inpatient wards of the Hospital that have histological confirmation. Results: In the period 1991-2011, 22 cases of pemphigus vulgaris were diagnosed, representing 12.7% of cases of pemphigus observed in our service. Predominance in female sex (13 cases) and in the age group 30 to 49 years (15 cases) was observed, with a mean age of 42.1 years. Oral mucosal involvement was present in 18 cases, in 12 of which it was the initial symptom. Cutaneous involvement observed in all cases was disseminated or generalized (20 cases). Two cases were of the vegetative variety. The most commonly used treatment was prednisone alone (8 cases) or associated with azathioprine (8 cases). The outcome was favorable in 19 cases, with cure or remission. Two cases ended in death and one received no treatment. Conclusions: Pemphigus vulgaris is a rare condition in our service, but with significant morbidity and mortality. It is second in frequency after the foliaceous type.

Resumo: Abstract: Lips are structures that play an essential role in aesthetics and in different functions such as nutrition and speech. The complex anatomy of the lips – with three different layers composed of skin, mucosa, and muscles – makes surgical management of this area a therapeutic challenge. The use of flaps for the reconstruction of large defects with low risk of necrosis is possible given the abundant blood supply of the lips. We report a case of surgical reconstruction of the lower lip after the excision of a severe squamous cell carcinoma using a skin-mucosa Abbe-Estlander flap with a satisfactory final outcome.

Resumo: SUMMARY: By analysing cephalometrics before and after the treatment of Angle's class II division 1 malocclusion, soft tissue profile changes were observed, and to explore whether there was a correlation between the change of posterior occlusal plane (OP-P) and soft tissue profile. The study aimed to provide a theoretical reference in the clinical diagnosis and orthodontic treatment of early permanent dentition of Angle class II division 1 malocclusion patients. We randomly selected 20 cases of orthodontics in Shanxi Medical University orthodontics patients (aged 12-18 years) of the lateral cephalograms, Winceph 8.0 software was used to measure the posterior occlusal plane (OP-P), TUL-EP, TLL-EP, nasolabial angle, upper lip base thickness, upper lip tension, lower lip H line distance, chin soft tissue thickness and Z angle. And SPSS 22.0 software was used to analyze the correlation between OP-P and other measurement indexes. The change of posterior occlusal plane was positively related to the distance from upper lip to the esthetic plane, the distance from lower lip to the esthetic plane, the tension of the upper lip and distance from lower lip to H line, and the change of posterior occlusal plane was negatively related to nasolabial angle and Z angle. The change of posterior occlusal plane had no significant correlation with upper lip base thickness and soft tissue chin thickness. The change of posterior occlusal plane has an important guiding significance for orthodontic treatment,by focusing on the change of the posterior occlusal plane to achieve the purpose of changing the soft tissue profile.

RESUMEN: Se realizó un estudio cefalométrico antes y después del tratamiento de ortodoncia en pacientes portadores de maloclusión clase II división 1 de Angle. Se observaron cambios en el perfil de los tejidos blandos y se exploró si había una correlación entre el cambio del plano oclusal posterior (OP-P) y el perfil de los tejidos blandos. El objetivo de este estudio fue otorgar una referencia teórica en el diagnóstico clínico y el tratamiento de ortodoncia de los pacientes con dentición permanente temprana con maloclusión clase II división 1 de Angle. Seleccionamos al azar 20 casos cefalometrías de pacientes de ortodoncia de la Universidad Médica de Shanxi (entre 12 y18 años). Para ello, se utilizó el software Winceph 8.0 para medir el plano oclusal posterior (OP-P), TUL-EP, TLL-EP, ángulo nasolabial, grosor de la base del labio superior, tensión del labio superior, distancia entre líneas del labio inferior H, grosor del tejido blando del mentón y ángulo Z. Se utilizó el software SPSS 22.0 para analizar la correlación entre OP-P y otros índices. El cambio del plano oclusal posterior tuvo una correlación positiva con la distancia desde el labio superior al plano estético, la distancia desde el labio inferior hasta el plano estético, la tensión del labio superior y distancia del labio inferior a la línea H. El cambio del plano oclusal posterior tuvo una correlación negativa con el ángulo nasolabial y el ángulo Z. El cambio del plano oclusal posterior no tuvo una correlación significativa con el grosor de la base del labio superior y la barbilla de tejido blando. El cambio del plano oclusal posterior tiene un importante significado de orientación para el tratamiento de ortodoncia, de tal manera que al cambiar el plano oclusal posterior se podrá lograr el propósito de cambiar el perfil del tejido blando.

Resumo: The authors report a case of diffuse large B-cell lymphoma (DLBL) of the oral cavity. The patient was a 73-year-old white man who first presented at the Division of Stomatology with a large nodular mass in the hard palate and a nodular lesion in the upper lip, which were diagnosed as DLBL. The patient was treated with eight cycles of CHOP chemotherapy (cyclophosphamide, doxorubicin, vincristine and prednisone), but the disease recurred 22 months after the end of the therapy. Both primary sites hard palate and upper lip were involved again and the patient was resubmitted to chemotherapy(AU)

Se trata de un paciente masculino, blanco, de 73 años de edad, que se presentó a la División de Estomatología, con una masa nodular difusa en el paladar duro y una lesión nodular en el labio superior, diagnosticadas como DLBL. El paciente fue tratado con ocho ciclos de quimioterapia CHOP (ciclofosfamida, doxorrubicina, vincristina, prednisona), pero se constató recidiva de la enfermedad en ambos sitios primarios, en el paladar duro y en el labio superior a los 22 meses después del término de la terapia. El paciente fue sometido de nuevo a la quimioterapia(AU)
