Resumo: Se presenta el caso de un paciente de 47 años con enorme giba lumbar derecha que apareció a los 18 años de edad. Fue operado hace 14 años de una formación quística con aumento de volumen de las extremidades y aparición, poco después, de un tumor de 2,5 cm. Diez días antes de su ingreso presentó infección urinaria, luego constipación, dificultad para expulsar gases, dolor y distensión abdominal, y se visualizaba la referida tumoración. En la radiografía se observaron múltiples fecalomas y oblito metálico. Fue operado y se constató la presencia de fecalomas, distensión de asas y el oblito cuya punta era la que se visualizaba y palpaba(AU)

This is a case presentation of a patient aged 47 with a huge right humpback appearing at 18 years old. He was operated on 14 years ago from a cystic formation increasing the volume of limbs and the appearance afterwards of a 2,5 cm tumor. Ten days before its admission showed a urinary infection, constipation, difficulty to release gases, pain and abdominal distention, visualizing such tumor. At radiography on noted many fecaloma and metallic foreign body. He was operated on verifying the presence of fecalomas, loop distention and foreign body whose tip was visualized and palpated(AU)

Título: Double blinded randomized clinical trial to assess the effectiveness of several preparations for colonoscopy / Ensaio clínico randomizado duplo cego para avaliar a eficácia de várias preparações para colonoscopia

Resumo: ABSTRACT Introduction: Colonoscopy is the screening gold standard to investigate several conditions in the colon. The excellence of preparation is a determining factor for a quality colonoscopy. Objective: Compare the quality of colon preparations for colonoscopy with different kinds of laxative medications in a public hospital of Belo Horizonte, Brazil. Method: A prospective double blind randomized clinical trial was conducted from June 2016 to March 2017. A total of 117 Patients were randomised in four groups to receive a type of preparation (Sodium picosulfate, Mannitol, Lactitol, Lactulose). The patients answered a questionnaire and peripheral blood samples were collected before and after the preparation.The quality of the cleansing was accessed according to the Boston Bowel Preparation Scale. Results: 99.1% of patients have taken the recommended dose and 79.5% reported a good tolerability. Endoscopists performed complete colonoscopy in 89.7%, with an polipectomy rate of 47%. The total effectiveness rate of the solutions were 88%. There were no statistically significant differences between groups (p = 0.271). Regarding the laboratory parameters, differences were seen in the pre- and post-test values of sodium, chlorine and creatinine but without exceeding reference values. Conclusion: The four preparations were effective for colon cleansing, with good acceptance, differing only as for costs.

RESUMO Introdução: a colonoscopia é o padrão ouro de triagem para pesquisa de várias doenças colônicas. A excelência de preparação é um fator determinante para uma colonoscopia de qualidade. Objetivo: Comparar a qualidade das preparações do cólon para colonoscopia com diferentes tipos de medicamentos laxantes em um hospital público de Belo Horizonte, Brasil. Método: Foi realizado um ensaio clínico randomizado duplo cego prospectivo de junho de 2016 a março de 2017. Um total de 117 pacientes foi randomizado em quatro grupos para receber um tipo de preparação (picossulfato sódico, manitol, lacticol, lactulose). Os pacientes responderam a um questionário e amostras de sangue periférico foram coletadas antes e depois da preparação. A qualidade da limpeza foi acessada de acordo com a Boston Bowel Preparation Scale. Resultados: 99,1% dos pacientes tomaram a dose recomendada e 79,5% relataram boa tolerabilidade. Os endoscopistas realizaram colonoscopia completa em 89,7%, com taxa de polipectomia de 47%. A taxa de eficácia total das soluções foi de 88%. Não houve diferenças estatisticamente significantes entre os grupos (p = 0,271). Em relação aos parâmetros laboratoriais, foram observadas diferenças nos valores pré e pós-teste de sódio, cloro e creatinina, mas sem exceder os valores de referência. Conclusão: As quatro preparações foram eficazes para limpeza do cólon, com boa aceitação, diferindo apenas quanto aos custos.

Resumo: Introducción: La reparación de la pared abdominal, presenta escenarios complejos, como los abdómenes abiertos con ostomías o fístulas intestinales, que para su tratamiento el cirujano debe buscar la táctica quirúrgica y el manejo ideal para cada caso. Objetivo: Describir los resultados en la reparación de defectos complejos de la pared abdominal y restitución de tránsito intestinal en un solo tiempo. Pacientes y método: Estudio observacional, descriptivo, prospectivo y longitudinal, de una serie de casos de 30 pacientes, ambos sexos, mayores a 18 años, portadores de defectos complejos de la pared abdominal (abdomen abierto cicatrizado por segunda intención y hernias paraestomales) y ostomías que ingresaron para cirugía electiva en la II Cátedra de Clínica Quirúrgica del Hospital de Clínicas de marzo del 2011 a diciembre del 2015. Introduction: Abdominal wall repair, presents complex scenarios, as the bellies open with ostomy or intestinal fistulas, that treatment the surgeon should seek surgical tactics and ideal management for each case. Objective: To describe the results in repair of complex defects of the abdominal wall and return of intestinal transit in a single time. Resumo: ABSTRACT PURPOSE: To evaluate the effects of an intraperitoneal solution of methylene blue (MB), lidocaine and pentoxyphylline (PTX) on intestinal ischemic and reperfusion injury METHODS: Superior mesenteric artery was isolated and clamped in 36 adult male Sprague Dawley rats. After 60 minutes, clamp was removed and a group received intraperitoneally UNITO solution (PTX 25mg/kg + lidocaine 5mg/kg + MB 2mg/kg), while the other group was treated with warm 0.9% NaCl solution. Rats were euthanized 45 min after drug administration. Lung and bowel were collected for histological evaluation (using Park's score) and determination of myeloperoxidase (MPO) and malondialdehyde (MDA) levels. RESULTS: Control samples showed lymphoplasmocytic infiltrate and crypt necrosis of villi. MPO and MDA measurements shown no differences between treated and control groups. CONCLUSION: The combination of lidocaine, methylene blue and pentoxyphylline administered intraperitoneally at the studied dose, did not decreased histological lesion scores and biochemical markers levels in intestinal ischemia/reperfusion injury.

Título: Improving reproducibility and external validity. The role of standardization and data reporting of laboratory rat husbandry and housing

Resumo: Abstract Purpose: To identify the most relevant flaws in standardization in husbandry practices and lack of transparency to report them. This review proposes some measures in order to improve transparency, reproducibility and eventually external validity in experimental surgery experiments with rat model. Methods: We performed a search of scientific articles in PUBMED data base. The survey was conducted from august 2016 to January 2017. The keywords used were "reproducibility", "external validity", "rat model", "rat husbandry", "rat housing", and the time frame was up to January 2017. Articles discarded were the ones which the abstract or the key words did not imply that the authors would discuss any relationship of husbandry and housing with the reproducibility and transparency of reporting animal experiment. Reviews and papers that discussed specifically reproducibility and data reporting transparency were laboriously explored, including references for other articles that could fulfil the inclusion criteria. A total of 246 articles were initially found but only 44 were selected. Results: Lack of transparency is the rule and not the exception when reporting results with rat model. This results in poor reproducibility and low external validity with the consequence of considerable loss of time and financial resources. There are still much to be done to improve compliance and adherence of researchers, editors and reviewers to adopt guidelines to mitigate some of the challenges that can impair reproducibility and external validity. Conclusions: Authors and reviewers should avoid pitfalls of absent, insufficient or inaccurate description of relevant information the rat model used. This information should be correctly published or reported on another source easily available for readers. Environmental conditions are well known by laboratory animal personnel and are well controlled in housing facilities, but usually neglected in experimental laboratories when the rat model is a novelty for the researcher.

Resumo: Abstract Purpose: To investigate the expression of nitric oxide synthase (NOS) and apoptosis associated with ischemic preconditioning (IPC) and pentoxifylline (PTX) in intestinal ischemia (I) and reperfusion (R) injury. Methods: Thirty male rats were assigned to 5 groups: (CG), no clamping of the superior mesenteric artery (90 minutes); (IR-SS) saline + ischemia (30 minutes) + reperfusion (60 minutes); (IR-PTX) PTX + ischemia (30 minutes) + reperfusion (60 minutes); (IPC-IR-SS) 5 minutes of ischemia + 5 minutes of reperfusion (IPC) + saline + I(30 minutes)+R(60 minutes); and (IPC-IR-PTX) IPC + PTX + I(30 minutes)+ R(60 minutes). Results: The application of IPC and PTX showed a significantly lower immunohistochemistry reaction for active caspase-3 (P<0.05) compared to IR+SS. The number of cells immunoreactive to BCL-2 was higher in the IR-PTX group (P>0.05). The NOS-2 expression (qRTPCR) in the IR-PTX group (P<0.05) was higher than the values for the IPC+IR-SS and IPC-IR-PTX groups. The NOS-3 expression was significantly upper in the IPC-IR-PTX group than in the CG (P<0.05), the IR-SS (P<0.05) and the IR-PTX (P<0.05) groups. Conclusions: The BCL-2 and active caspase-3 showed beneficial effects on PTX and IPC. The expression of NOS-2 and NOS-3 in the IPC and IPC-PTX groups showed no synergistic effect.

Título: Effect of atenolol pre-treatment in heart damage in a model of intestinal ischemia-reperfusion

Resumo: Abstract Purpose: To investigate the effects of atenolol in inflammatory mediator and oxidative stress in a myocardial injury by intestinal ischemia/reperfusion in rat model. Methods: Adult Wistar male rats were randomly (n=8), anesthetized and divided in: Sham: submitted to operation only; group SS+IR: intravenous saline infusion following superior mesenteric artery occlusion during 60 minutes (ischemia) and open for 120 minutes (reperfusion); group AT+IR: intravenous atenolol infusion (2 mg/kg) following superior mesenteric artery occlusion during 60 minutes (ischemia) and open for 120 minutes (reperfusion); and group AT+I+AT+R: intravenous atenolol infusion following superior mesenteric artery occlusion during 60 minutes (ischemia) and in the time 45 minutes other atenolol doses were administrated and the artery was open for 120 minutes (reperfusion), all animals were submitted to muscular relaxation for mechanical ventilation. In the end of experiment the animals were euthanized and the hearts tissue were morphology analyzed by histology and malondialdehyde by ELISA, and the plasma were analyzed for tumor necrosis factor-alpha by ELISA. Results: The group SS+IR demonstrated the higher malondialdehyde levels when compared with the atenolol treated-groups (p=0.001) in the heart tissue. The tumor necrosis factor-alpha level in plasma decrease in the treated groups when compared with SS+IR group (p=0.001). Histology analyses demonstrate pyknosis, edema, cellular vacuolization, presence of inflammatory infiltrate and band contraction in the heart tissue of the rats. Conclusion: Atenolol significantly reduce the degree of cardiac damage after intestinal ischemia-reperfusion.

Título: Hyperbaric oxygenation and the genic expression related to oxidative stress in the heart of mice during intestinal ischemia and reperfusion

Resumo: Abstract Purpose: To investigate the effects of hyperbaric oxygenation (HBO) on intestinal ischemia and reperfusion (IR) injury, we evaluated the expression of 84 genes related to oxidative stress and the antioxidant response in mouse hearts. Methods: Four groups were subjected to 60 minutes of intestinal ischemia followed by 60 minutes of reperfusion: IRG, ischemia and reperfusion group without HBO; HBO-IG, which received HBO during ischemia; HBO-RG, which received HBO during reperfusion; and HBO-IRG, which received HBO during ischemia and reperfusion. The control group (CG) underwent anesthesia and laparotomy and was observed for 120 minutes. The (RT-qPCR) method was applied. Genes with expression levels three times below or above the threshold cycle were considered significantly hypoexpressed or hyperexpressed, respectively (Student's t-test p<0.05). Results: Eight genes (9.52%) were hyperexpressed in the IRG. When the HBO groups were compared to the IRG, we found a decrease in the expression of eight genes in the HBO-IG, five genes in the HBO-RG, and seven genes in the HBO-IRG. Conclusion: The reduction in the expression of genes related to oxidative stress and antioxidant defense following HBO in mouse hearts resulting from intestinal IR injury was more favorable during the ischemic period than during the reperfusion period.

Resumo: O parasitismo acontece quando há associação de dois seres vivos onde um deles se desenvolve e prejudica de alguma forma o outro ser, chamado hospedeiro (El parasitismo ocurre cuando existe una asociación de dos seres vivos, donde uno de ellos se desarrolla y afecta de algún modo al otro, que se llama huésped). Alguns parasitos como Entamoeba histolytica/Entamoeba dispar, Giardia lamblia, Hymenolepis nana, Taenia solium, Ascaris lumbricoides, Trichuris trichiura e Enterobius vermicularis são transmitidos pela água e/ou (se transmiten por el agua o por) alimentos contaminados e afetam grande parte da população mundial no desempenho (y afectan a una gran parte de la población mundial en el desarrollo) de suas atividades físicas, mentais e sociais, expondo-a a diversas manifestações clínicas que podem gerar elevados níveis (pueden generar altos niveles) de morbidade. Considerando que, aproximadamente, 11% da população brasileira sofrem com algum tipo de deficiência intelectual e/ou múltipla e pouco se tem estudado sobre a ocorrência (y poco se ha estudiado acerca de la existencia) de enteroparasitos nesse grupo populacional, estudos parasitológicos laboratoriais foram realizados em 50 estudantes da Associação de Pais e Amigos dos Excepcionais (APAE) da cidade de São Mateus, Espírito Santo, Brasil. Do total de amostras analisadas 28 (56%) estavam positivas para um ou mais parasitos e comensais, sendo (uno o más parásitos y comensales, siendo) 5 (17.85%) por G. lamblia, 7 (25.00%) por E. histolytica/E. dispar/E. moschkovskii, 11 (39.28%) por Entamoeba coli, 6 (21.43%) por Balantidium coli, 2 (7.14%) por Endolimax nana e 3 (10.7%) por E. vermicularis. Considerando a elevada frequência de indivíduos parasitados torna-se necessário a busca (se hace necesaria la búsqueda) de investimentos em profilaxia e educação higiênico-sanitária, além da realização (además de la realización) constante de exames parasitológicos, garantindo uma melhor (garantizando una mejor) qualidade de vida à população da APAE de São Mateus.

Parasitism occurs when there is an association between two living beings, in which one develops and in some way harms the other being, called host. Some parasites such as Entamoeba histolytica/Entamoeba dispar, Giardia lamblia, Hymenolepis nana, Taenia solium, Ascaris lumbricoides, Trichuris trichiura and Enterobius vermicularis are transmitted by contaminated water and/or food and affect a large number of the world population in their physical, mental and social activities, exposing them to diverse clinical manifestations that can generate high levels of morbidity. Considering that approximately 11% of the Brazilian population suffers from some type of intellectual and/or multiple deficiency and little has been studied about the occurrence of enteroparasites in this population group, laboratory parasitological studies were performed in 50 students of the Associação de Pais e Amigos dos Excepcionais (APAE) of the city of São Mateus, Espírito Santo, Brazil. From the total of the analyzed samples, 28 (56%) were positive for one or more parasites and commensals, of which 5 (17.85%) were positive for G. lamblia, 6 (25.00%) for E. histolytica/E. dispar, 11 (39.28%) for Entamoeba coli, 7 (21.43%) for Balantidium coli, 2 (7.14%) for Endolimax nana and 3 (10.7%) for E. vermicularis. Considering the high prevalence of parasitic infection in this population, it is necessary to seek greater investment in prophylaxis and hygienic-sanitary education, in addition to constantly conducting parasitological examinations that might guarantee a better quality of life for the population of São Mateus APAE.

Resumo: Las duplicaciones intestinales quísticas son anomalías congénitas infrecuentes, con incidencia de 1:4500 autopsias y etiopatogenia incierta. Son estructuras quísticas, tubulares o diverticulares revestidas de mucosa gastrointestinal con pared de músculo liso, adheridas al tracto gastrointestinal, pero no suelen comunicar con la luz intestinal. Los quistes de duplicación gástrica representan el 7-9% de los quistes de duplicación intestinal. Pueden diagnosticarse intraútero mediante ecografía y la resonancia magnética fetal permite caracterizar el quiste y descartar patologías concomitantes. La ecografía posnatal revela una imagen quística de "doble pared". Los neonatos suelen permanecer asintomáticos, pero pueden presentar desde síntomas gastrointestinales inespecíficos hasta síntomas de obstrucción por efecto masa, torsión o infección. En pacientes asintomáticos, se aconseja el seguimiento clínico y estudios de imagen periódicos. El tratamiento es la resección quirúrgica electiva, de manera preferente, con técnicas mínimamente invasivas. Se presenta un caso con sospecha prenatal de quiste de duplicación enteral.

Intestinal cystic duplications are rare congenital anomalies, with an estimated incidence of approximately 1:4500 autopsies. The etiopathogenesis is uncertain. These duplications are cystic, tubular or diverticular structures lined with gastrointestinal mucosa. They share a common smooth muscle wall with the gastrointestinal tract but usually their lumens do not communicate with each other. Gastric duplication cysts represent 7-9% of the gastrointestinal tract duplication. They can be diagnosed prenatally by fetal ultrasound; magnetic resonance imaging characterizes the cyst and excludes other malformations. Postnatal ultrasound shows a characteristic double walled cyst. Newborns are usually asymptomatic, although nonspecific gastrointestinal symptoms, intestinal obstruction due to mass effect, volvulus or infection are described. In asymptomatic patients, clinical follow-up and periodic image controls are recommended. Elective surgical resection is the treatment of choice, using minimally invasive technique whenever possible. A case of prenatally suspected intestinal cystic duplication is presented.
