Título: Oral Candidal Carriage Among Patients with Oral Potential Malignant Disorders: A Case-Control Study

Resumo: Objective: To determine the prevalence of Candida species in the saliva of patients with clinically suspected oral potentially malignant disorders (OPMD) and healthy cohorts. Material and Methods: Unstimulated saliva was collected from patients with OPMD (n=100) and age and sex matched healthy subjects (n=170). The samples were inoculated onto Sabouraud Dextrose Agar and incubated for a week. The colonies of the isolates were enumerated using a colony counter. The isolates were identified using standard phenotypic methods. The significance of oral candidal carriage was calculated using Independent T test. Odds and Risk ratio was calculated using Pearson's Chi-square test. Results: Oral candida carriage was present in 51% of patients with OPMD while healthy cohorts had a prevalence of 20.6%. A good statistical significance was observed for the prevalence of oral candidal carriage for patients with OPMD in comparison to healthy cohorts (p=0.013). Significant Odds and risk ratio was observed for the prevalence of Candida species among OPMD. Majority of the isolates in both groups were C. albicans. Colony forming units were high among patients with OPMD. Conclusion: A significant association of oral candidal carriage to oral potentially malignant disorders in comparison to healthy cohorts was observed. Candidal species may be potent risk factor for transition of OPMD to oral Squamous Cell Carcinoma.

Título: Brush Biopsy Sample Quality: Preliminary Investigation of a Metal Brush Prototype

Resumo: Objective: To qualitatively investigate whether a prototype brush composed of metal bristles collects oral epithelial cells effectively for cytological evaluation of oral mucosal lesions. Material and Methods: Twenty patients with suspicious oral mucosal lesions were enrolled. Patients were asked to gargle with saline and to deposit the oral rinse into specimen cup. Then, oral mucosal cell samples were collected using a metal oral brush, via sweeping motion. Punch biopsy was performed for histological examination. All samples were evaluated with liquid based cytology (LBC) according to the cellularity, the depth of the epithelial layer, cellular integrity by an oral pathologist. Results: Oral rinse provided samples with 100% cellular integrity and cellularity, mostly from the intermediary layers. With metal brush, both inadequate cellularity and cellular integrity was observed in 25% of the cases. Cellular integrity was adequate in 65%, cellularity was adequate in 45% of the lesions. Samples were dominantly from the intermediary layers, but in one case, metal brush collected cells from the parabasal layer. Conclusion: The narrow spiral pitch and width of metal bristles may have resisted to release the cellular samples collected. With adjustment of the spiral pitch and diameter of metal brush bristles, its' efficacy could be enhanced.

Resumo: Objective: To compare the use of Fresh Frozen Amniotic Membrane (FFAM) and Buccal Pad of Fat (BPF) for reconstruction of oral mucosal defect after surgical excision of leukoplakia. Material and Methods: Twenty patients were randomly selected and divided into two groups. Group 1 use amniotic membrane graft and Group 2 BPF. Both groups were evaluated preoperatively and postoperatively. Incisal opening, epithelialization and fibrosis were evaluated after one month of surgery. Chi square and Student t tests were used. Results: According to the presence of smoking habits, the highest frequencies were for smoking (30%) and betel leaf areca nut with tobacco (30%). Regarding the diameter of oral leukoplakia, in 40% of the participants it was 2x3 cm2. In Group1, after one month of surgery preoperative and postoperative inter-incisal opening values were 44.20 ± 3.37 and 42.05 ± 3.47 (p<0.001). In Group 2, preoperative and postoperative inter-incisal opening values were 44.09 ± 3.32 and 43.01±3.38 (p>0.05). When FFAM was used complete epithelialization in 70% and incomplete epithelialization in 30% patients. When BPF was used the results were almost similar. Fibrosis occurred in 30% in Group 1. There were no complications like flap necrosis, infectiona and hematoma formation. Conclusion: Incisal opening was significantly better in Fresh Frozen Amniotic Membrane Group, epithelialization and fibrosis were almost same in both groups after surgical excision of oral leukoplakia.

Título: Compatibility of Clinical and Histopathological Diagnosis of Oral Lesions in Iranian Patients

Resumo: Objective: To determine compatibility between clinical diagnosis and the pathological reports of biopsies from oral lesions. Material and Methods: In this descriptive study, 1146 clinical files of patients referring to Tabriz Faculty of Dentistry from 2004 to 2016 were retrieved and evaluated. The kappa coefficient was calculated for each file for compatibility of clinical and pathological diagnosis. Results: In relation to clinical (40.2%) and pathological (39.2%) diagnosis, irritational lesions of soft tissues exhibited the highest frequency. In 72.3% of cases, the clinical and pathological diagnosis were compatible and in 27.7% of cases these diagnosis were not compatible. The highest compatibility rates were detected for irritational lesions of soft tissues (81.5%) and mucocutaneous lesions (76.9%). There was no compatibility for osseous malignant tumors, inflammatory tissues, granulation tissues, metastatic lesions and hematologic disorders. Conclusion: Approximately one-third of clinical and histopathological diagnosis were not compatible. Therefore, to reach a correct diagnosis, the clinical, radiographic and histopathological views should be evaluated simultaneously.

Resumo: Analisar resultados da recuperação de artigos sobre a temática neoplasia bucal e reabilitação bucal, publicados entre os anos de 1990 e abril de 2016, identificar a dispersão das publicações, examinar o padrão de autoria e citações na base de dados Scopus, a partir das Leis de Bradford e Lotka. Estudo bibliométrico, com abordagem descritiva e quantitativa. Realizada análise de artigos publicados em periódicos indexados na base de dados Scopus, utilizando-se os termos Mouth Neoplasms e Mouth Rehabilitation. Foram recuperados 28 artigos originais, publicados em 18 revistas, com distribuição irregular no período estudado. No momento da análise, identificou-se 222 citações com média de oito por documento. Medicina e odontologia se destacaram com publicações na temática. A Alemanha foi o país com maior produção científica. O estudo evidencia um quantitativo discreto de publicações e revela um campo que necessita do engajamento da enfermagem (AU).

The objectives of this study were to analyze the results of articles retrieved on the subject of mouth neoplasms and mouth rehabilitation, published between 1990 and April 2016, identify the dispersion of the publications, and examine authorship patterns and citations in the Scopus database, based on the Laws of Bradford and Lotka. This was a bibliometric study, with a descriptive and quantitative approach. An analysis of articles published in journals indexed in the Scopus database was performed, using the terms "Mouth Neoplasms" and "Mouth Rehabilitation". Twenty-eight original articles were retrieved, published in 18 journals, with irregular distribution in the period studied. At the time of analysis, 222 citations were identified with a mean of eight per document. The fields of medicine and dentistry stood out with publications on the theme. Germany had the largest scientific production. The study revealed a modest number of publications and the need for nursing engagement in this area of research (AU).

Analizar resultados de la recuperación de artículos sobre la temática neoplasia bucal y rehabilitación bucal, publicados entre los años 1990 y abril de 2016; identificar la dispersión de las publicaciones, examinar el estándar de autoría y citaciones en base de datos Scopus partiendo de las Leyes de Bradford y Lotka. Estudio bibliométrico, con abordaje descriptivo y cuantitativo. Se realizó análisis de artículos publicados en periódicos indexados en la base Scopus, utilizándose los términos Mouth Neoplasms y Mouth Rehabilitation. Pudieron recuperarse 28 artículos originales, publicados en 18 revistas, con distribución irregular en el período estudiado. Al momento del análisis de identificaron 222 citaciones, con promedio de ocho por documento. La Medicina y la Odontología se destacaron con publicaciones en la temática. Título: Intraosseous myofi broma of the jaw: Review of the literature / Miofibroma mandibular intraósseo: revisão de literatura

Resumo: The objective of this study was to perform a review of the pathological features and treatment of intraosseous myofi broma of the jaw. Electronic searches were performed of MEDLINE, via Pubmed (from 1950 to November 2012), EMBASE and Cochrane Wiley, on November 27th, 2012, with no language restrictions. The electronic search yielded 41 hits, of which 27 articles were considered relevant after evaluation of titles and abstracts and underwent eligibility assessment according to the eligibility criteria Of these, 10 did not meet one or more inclusion criteria and were rejected. The 17 articles selected included three retrospective studies, 13 case reports and one case series. These articles reported on a total of 24 cases of solitary myofi broma in the jaw. Intraosseous myofi bromas of the jaw are rare lesions and are part of the wide group of spindle-cell neoplasms and are generally observed in the posterior mandible. It is important to establish the appropriate treatment to avoid unnecessary aggressive therapy. Conservative surgery is the preferred option. Keywords: Myofi broma; Mandibular Neoplasm; Treatment.

O objetivo deste estudo foi realizar uma revisão de literatura sobre miofi broma intraósseo da mandíbula, suas características patológicas e tratamento. Foi realizada uma busca eletrônica nas base de dados MEDLINE via Pubmed (1950 a novembro de 2012), EMBASE e Wiley Cochrane, no dia 27 de novembro de 2012 sem restrição de linguagem. A busca eletrônica resultou em 41 citações, e 27 artigos foram considerados relevantes pela leitura do título e pela avaliação do resumo. Desses 27 artigos, 10 não atenderam um ou mais critérios de inclusão e foram excluídos. Entre os 17 artigos selecionados, havia três estudos retrospectivos, 13 relatos de caso e uma série de casos. Vinte e quatro casos de miofi broma solitário intraósseo da mandíbula foram identifi cados. Miofi bromas intraósseos dos maxilares são lesões raras que fazem parte do grupo de lesões neoplásicas fusiformes e são comumente encontrados na região posterior da mandíbula. É importante estabelecer o tratamento apropriado para evitar terapia agressiva desnecessária. Cirurgia conservadora é a opção desejada.

Resumo: Objetivo: analisar as tendências de concessão de auxílios-doença previdenciários por câncer bucal e de orofaringe no período de 2006 a 2013, no Brasil. Métodos: estudo de séries temporais com dados dos trabalhadores segurados pelo Instituto Nacional de Seguridade Social (INSS); utilizaram-se regressões generalizadas lineares de Prais-Winstein para o cálculo da variação percentual anual (VPA). Resultados: os benefícios previdenciários por câncer de boca e de orofaringe mostraram tendência crescente (VPA=9,0%; IC95% 1,4;17,4); benefícios para outras partes da boca, nasofaringe, orofaringe, assoalho da boca e palato mostraram aumento significativo; comércio (5,5%) e indústrias de transformação (5,2%) foram as atividades mais prevalentes; evidenciaram-se altas proporções de campos ignorados nos sistemas de informações (média de 72,9%). Conclusão: as tendências dos benefícios ocupacionais por câncer de boca e de orofaringe mostraram crescimento significativo.

Objetivo: investigar las tendencias en prestaciones de seguridad social para el cáncer oral y orofaríngeo en trabajadores asegurados por el Instituto Nacional de Seguridad Social (INSS) para el período 2006 a 2013, en Brasil. Métodos: estudio de series temporales con datos de los trabajadores asegurados por el INSS; se utilizaron regresiones lineales generalizadas de Prais-Winstein para calcular la variación porcentual anual (VPA). Resultados: los beneficios de cáncer de boca y faringe mostró un aumento creciente (VPA=9,0%; IC95% 1,4;17,4); beneficios a otras partes de la boca, nasofaringe, orofaringe, piso de la boca y paladar mostraron un aumento significativo; el comercio (5,5%) y las industrias manufactureras (5,2%) fueron las actividades más frecuentes; se presentaron fallas en los sistemas de informaciones (72,9% ignorada Group). Conclusion: tendencias de beneficios ocupacionales por cáncer oral y orofaríngeo mostraron un aumento significativo.

Objective: to analyze the trends in the concession of social security sick pay for oral and oropharyngeal cancer, from 2006 to 2013, in Brazil. Methods: time series study using data of workers insured by the Brazilian National Institute of Social Security (INSS); Prais-Winsten generalized linear regressions were used to calculate the annual percentage change (APC). Results: social security benefits for oral and oropharyngeal cancer presented significant increase (APC=9.0%; 95%CI 1.4; 17.4); benefits for other parts of the mouth, nasopharynx, oropharynx, floor of mouth and palate have also shown significant increase; the areas of trade (5.5%) and manufacturing (5.2%) were the most prevalent activities; there was a high proportion of fields in blank in the information systems (average of 72.9%). Conclusion: trends in occupational benefits for oral and oropharyngeal cancer showed significant increase.

Resumo: RESUMEN: Las lesiones pigmentadas malignas en la mucosa oral son de escasa frecuencia, pero de altas tasas de morbilidad y mortalidad, siendo de mal pronóstico a pesar de agresivos tratamientos que pueden combinar grandes resecciones quirúrgicas con quimioterapia y radioterapia. Dado este escenario, la conducta clínica asociada a la pesquisa de lesiones pigmentadas es la biopsia inmediata para la confirmación o descarte de patología maligna y con ello un inicio temprano de tratamiento. Este artículo trata de una paciente recibida en un hospital público aproximadamente seis meses posteriores a las primeras manifestaciones de una lesión pigmentada en mucosa maxilar, que luego de confirmado el diagnóstico de una lesión maligna fue derivada y manejada por un equipo oncológico del mismo centro.

ABSTRACT: Pigmented malignant oral mucosa lesions are infrequent, but with high rates of morbidity and mortality, and have a poor prognosis despite aggressive treatments that can combine large surgical resection with chemotherapy and radiotherapy. Given this scenario, the clinical conduct associated with pigmented lesions is the immediate biopsy to confirm or rule out the disease and thus the early initiation of treatment. This article is about a patient attended in a public hospital about six months after the first signs of an injury to maxillary mucosa, which then confirmed the diagnosis of a malignant lesion, who was referred and treated by a cancer team of the same center.

Resumo: Introducción: Un factor de riesgo para las neoplasias escamosas de faringe son las nitrosaminas, las que derivan de los nitratos salivales que provienen de la dieta y en especial del agua si los niveles están por sobre la norma. Objetivos: Conocer y comparar los niveles de nitratos y nitritos en agua y saliva de habitantes de la región precordillerana y en población urbana de Concepción. Material y método: Estudio prospectivo, tipo caso y control. Determinan niveles de nitratos y nitritos en agua de la zona precordillerana y en agua de Concepción y en saliva de habitantes de la zona rural y de grupo control de Concepción. Resultados: Se obtienen 10 muestras de agua del sector rural y 1 muestra de agua potable de Concepción. Nivel de nitratos en agua rural 23,29 mg/L, en agua urbana 3,6 mg/L (p <0,05). En saliva, grupo estudio (n =15) con nivel de nitritos de 9,58 mg/ml, grupo control (n =24) nivel de 3,8 mg/ml (p <0,05). Conclusión: En la zona rural hay niveles significativamente mayores de nitratos en el agua y nitritos en saliva. Los niveles en agua son superiores a normas nacionales e internacionales. Nuestros resultados resaltan la posible importancia de las nitrosaminas y sus precursores como elementos carcinogónicos en los carcinomas escamosos orofaríngeos.

Introduction: A risk factor for squamous carcinomas of the oropharynx are the nitrosamines, which are derived from salivary nitrate coming from diet and water, especially if the levels are above the allowed levels. Aim: To determine and compare the levels of nitrates and nitrites in drinking water and saliva of people at the foothills of our region and urban population of Concepcion. Material and method: A descriptive cross sectional study. Determination of levels of nitrates and nitrites in drinking water in the Andean foothills and Concepcion and in saliva of people from same locations. Results: We obtained 10 rural water samples and 1 from Concepcion drinking water. Nitrate levels in rural water were 23.29 mg/L and 3.6 mg/L (p <0.05) in urban water. In saliva, the study group (n =15) had 9.58 mg/ml nitrite in saliva, the control group (n =24) 3.8 mg/ml (p <0.05). Conclusion: In rural areas there are significantly higher levels of nitrates in drinking water and saliva. The water levels are higher than national and international standards. Our results confirm high amounts of nitrogen cycle products (nitrites and nitrates) in drinking water and in saliva so it could be considered as carcinogenic elements in oropharyngeal squamous cell carcinomas.

Resumo: O carcinoma epidermoide corresponde a 90% das lesões malignas da cavidade oral e orofaringe. Entre os fatores de risco para o câncer de boca, pode-se destacar o tabagismo e o etilismo. Além disso, vírus oncogênicos estão sendo relacionados ao desenvolvimento do câncer oral e de orofaringe. Entre estes vírus, destaca-se o papilomavírus humano. Avaliar o efeito carcinogênico do papilomavírus humano em cavidade oral e orofaringe. Uma busca sistemática de artigos científicos publicados entre janeiro de 2000 a maio de 2016 foi executada nas bases de dados PubMed/Medline, Scopus e Science Direct utilizando-se os descritores: Câncer Oral e de Orofaringe, Carcinoma Epidermoide, papiloma vírus Humano. Após análise completa das publicações, seis artigos preencheram todos os critérios de inclusão. Em nossa revisão sistemática, o subtipo 16 foi o mais frequentemente localizado em cavidade oral nos pacientes com carcinoma epidermoide em todos os estudos, sendo, de acordo com a literatura científica, um subtipo de alto risco para o desenvolvimento neoplásico. O papilomavírus humano pode apresentar-se como um fator etiológico para o desenvolvimento do câncer de orofaringe, assim como para um subgrupo de carcinoma epidermoide, essencialmente em casos de infecção pelo subtipo de risco papilomavírus humano 16.

The squamous cell carcinoma accounts for 90% of malignant lesions of the oral cavity and oropharynx. Among the risk factors for oral cancer can highlight smoking and alcohol consumption. In addition, oncogenic viruses are related to the development of oral cancer and oropharyngeal. To evaluate the carcinogenic effect of human papillomavirus in oral cavity and oropharynx. A systematic search of scientific articles published from January 2000 to May 2016 was performed in the databases PubMed / Medline, Scopus and Science Direct through the descriptors: Oral and oropharynx cancer, squamous cell carcinoma, Human Papilloma Virus. After complete analysis of publications, six articles met all inclusion criteria. In our systematic review, subtype 16 was most often located in the oral cavity in patients with squamous cell carcinoma in all studies, and according to the scientific literature a high risk subtype for neoplastic development. Human papilloma virus can be presented as an etiological factor for the development of oropharyngeal cancer as well as squamous cell carcinoma subgroup, mainly in cases of infection risk subtype of human papilloma virus 16.
