Título: Conocimiento, actitud y práctica de los médicos pediatras sobre factores preventivos de la salud oral en la primera infancia / Knowledge, attitude and practices of Pediatricians on preventive factors of oral Health in early Childhood

Resumo: Introducción: El médico pediatra tiene un papel fundamental en las primeras orientaciones sobre salud bucal. Objetivo: Determinar el nivel de conocimientos, actitudes y prácticas de los médicos pediatras que ejercen en Asunción y Central sobre factores preventivos en la salud oral en la primera infancia en el año 2013. Materiales y Métodos: Estudio observacional descriptivo de corte transverso, realizado mediante un cuestionario autoadministrado a los médicos pediatras que ejercen en Asunción y Central. Resultados: Se incluyó 92 médicos pediatras de los Servicios de Salud de la XVIII y XI Región Sanitaria, edad media de 29 ±4,9 años, 62% del sexo femenino. La región donde trabaja la mayoría es en el departamento central (77%). El promedio de los años de graduación es de 6,5 años; el 78% entre 1 a 5 años. El número de pacientes que atiende por día es de >20 en el 53% de los casos. Se observa que el 74% de los pediatras tiene práctica inadecuada con respecto al examen bucal, prescripción de flúor; el 49% tiene conocimiento suficiente sobre factores de riesgo para las principales enfermedades bucales; y el 100% presenta actitud favorable hacia su papel importante en la prevención de las enfermedades bucales. No se encuentra asociación entre la edad del pediatra y el nivel de conocimiento y práctica, tampoco se encontró asociación entre los años de práctica con las prácticas, mientras que se evidencia que menos años de práctica tienen mejores conocimientos (p=0,009); y que los pediatras que atienden menos pacientes en un día tienen mejores conocimientos (p=0,010) y prácticas (p=0,035). Conclusión: Se resalta la falta de coherencia entre los conocimientos sobre factores preventivos y la práctica en los médicos pediatras participantes

Título: Prevalence of Caries and Periodontal Disease Among Indonesian Pregnant Women

Resumo: Objective: To determine the extent of bleeding on probing, probing pocket depth and the level of oral hygiene and caries prevalence among pregnant women. Material and Methods: Random samples of pregnant women attending eight public health centers were examined. Their periodontal and caries statuses were examined using the Community Periodontal Index, Simplified Oral Hygiene Index and DMF-T. The good criteria of OHI-S ranged from 0 to 1.2, the fair from 1.3 to 3.0 and the poor from 3.1 to 6.0. Descriptive statistics were used to calculate the absolute and relative frequencies. Results: 84.7% had caries and the DMF-T index score was 4.34 (fair). 73% suffered gingival bleeding, 34% with 4-5 mm pocket depth, 2% with 6 mm or deeper pocket depth, while the majority had good oral hygiene. The prevalence of 4-5 mm probing pocket depth increased between the first and second trimesters (12.1% to 48.5%), before undergoing a slight decrease in the third trimester (39.4%). Gingival bleeding was found to be most prevalent in the third trimester. Pocket depth of 4-5 mm was found to be highest in the second trimester. Pocket depth of 6 mm or more was restricted to the third trimester. Oral hygiene scores increased in tandem with gestational age. Conclusion: The majority had caries, which strongly suggests that the awareness of the pregnant women regarding their oral hygiene remains limited.

Título: Prevalence and Molecular Analysis of Novel Newly Identified Periodontal Pathogens in Subgingival Plaque Samples of Saudi Patients with Chronic Periodontitis

Resumo: Objective: To evaluate the prevalence of novel newly identified periodontal pathogenic strains in subgingival plaque samples and relate it with bleeding on probing, probing pocket depth and age. Material and Methods: 268 chronic periodontitis patients with a mean age of 46.0 ± 6.0 years were included. The following microorganisms were evaluated: Campylobacter gracilis (Cg), Fusobacterium nucleatum (Fn), Porphyromonas gingivalis (Pg), and Tannerella forsythia (Tf). Full mouth examination was registered; the probing pocket depth and clinical attachment level were assessed at six sites per tooth. Dental subgingival plaque samples were taken in the deepest pocket per arch in the maxilla and mandible. DNA analysis was performed using DNA-strip technology. Pearson's correlation coefficient was used for statistical analysis. Results: Porphyromonas gingivalis and Tannerella forsythia were detected at high level of 80% and 82% respectively. F nucleatum revealed a rate of 94%. Bacterial load significantly increased by increasing mean probing depth of the examined sites. Pearson's coefficient was the highest for Pg (r=0.62, p=0.0001) and the lowest for Cg (r=0.08, p=0.04). The bacterial load significantly increased by increasing the number of bleeding sites; Pearson's coefficient varied from r= 0.14 for Pg (p=0.01) to r=0.39 for Tf (p=0.001). Conclusion: It was confirmed the presence of main putative periodontal pathogens detectable in Saudi periodontal subjects, also providing a comprehensive view for correlation of these putative periodontal pathogens with the increase in probing pocket depth to the presence and load of all the bacterial species.

Título: Comparing the Levels of Gingival Crevicular Fluid Prostaglandin E2 in Generalized Chronic Periodontitis Between Healthy and Type 2 Diabetes Patients: A Case-Control Study

Resumo: Objective: To compare the prostaglandin E2 (PGE2) levels of gingival crevicular fluid in generalized chronic periodontitis between healthy and type 2 diabetic patients. Material and Methods: 56 diabetic and non-diabetic participants with generalized chronic periodontitis were selected randomly. They were divided into two groups (G1: generalized chronic periodontitis patients with normal blood sugar; and G2: generalized chronic periodontitis patients with diabetes). Gingival crevicular fluid samples were obtained from both groups. The average of 2 samples per day were centrifuged in a laboratory at 2500 rpm and temperature of 4°C for 5 minutes and placed in a refrigerator at -20°C. The level of PGE2 was measured using ELISA and Abcam kit. Data were analyzed by Kolmogorov-Smirnov, Mann-Whitney U Test, Pearson and independent T tests. The significant amount was considered 0.05 in this test (α<0.05). Results: The mean level of PGE2 was significantly different in the two groups and the mean level of PGE2 in the control group was lower than the case group. There was no statistically significant relationship between PGE2 with pocket depth, fasting blood sugar (FBS) and HBA1C (p>0.05). Conclusion: PGE2 level of diabetic patient group with chronic generalized periodontitis was significantly more than non-diabetic group with generalized chronic periodontitis.

Título: Efficacy of Two Different Toothbrushes on Plaque Control: A Randomized Clinical Study

Resumo: Objective: This parallel, randomized, examiner-blind clinical trial aimed to evaluate the efficacy of two different toothbrushes (manual and sonic) on plaque control in adolescents. Material and Methods: This study enrolled 56 volunteers, randomly allocated to two different groups: group A (n = 28) for the manual toothbrush (Curaprox 5460 Ultra Soft®) and group B (n = 28) for the sonic toothbrush (Edel White®). Mean age was 17.2 ± 1.1 years. A calibrated periodontist performed a periodontal evaluation to assess the presence of plaque with the Turesky Modification of the Quigley Hein Plaque Index (PI) and the gingival inflammation with the Silness & Löe Gingival Index (GI). Adolescents received instructions about the mechanical control of plaque at baseline (T0), with a reexamination after 3 months (T1) and 6 months (T2). Non-parametric Mann Whitney test was used to compare the differences between the two toothbrush groups and the Friedman test was used for the comparisons between times. Results: There has been PI reduction concerning the study times (T0, T1 and T2, p<0.05), but not between the groups A and B (p>0.05). As for GI there has been no significant difference between the groups and between the study times (p>0.05). Conclusion: Both toothbrushes were efficient in the control of supragingival plaque (visible biofilm).

Resumo: Objective: To determine the effect of scaling and root planning treatment on levels of hs-CRP (C-reactive protein) in patients with risk of cardiovascular disease. Material and Methods: This research is an experimental research with one group pre- and post-test design. This research was performed to the periodontal patients who came to the clinic and have risk of cardiovascular disease. Medical evaluations included measurement of blood pressure and body mass index were performed. Blood samples were obtained from each subject after over night fasting, highsensitivity C-reactive protein was measured as an index of inflammation. Blood samples were analysed two times before treatment (scaling and root planning) and three weeks later. Statistical analysis used Paired t-test. The level of significance was set at 5%. Results: Means-CRP levels before and after treatment in scaling and root planning were 3.16 ± 2.37 and 2.18±1.56 (p=0.007). Conclusion: There are significant differences between hs-CRP levels before and after treatment.

Resumo: Scaling using ultrasonic dental equipment was performed on 20 teeth extracted from dogs with advanced periodontal disease, under clinical indication. The surgical extraction of the teeth, which already showed mobility, carried out by means of a lever, tried not to touch the radicular surface which would be assessed. The mesial surface of the cervical root region was used for verifying the effect of the ultrasonic scaling and the distal surface was maintained intact for control. After treatment, the teeth were sectioned, separating the region of interest, and these were fixed in glutaraldehyde, dried with a critical point dryer and metallized for observation under electronic scanning microscopy. The presence of plaque, biofilm and rod cells on the non-treated surface was quite characterized. On the surface submitted to treatment by ultrasound, it was possible to see areas with the removal of all the dental calculus, and areas with exposed naked dentine and showing open dentinal tubules. It was possible to observe also the occasional presence of bio-film and rod cells penetrating the dentinal tubules. No harm to the dentine was observed. Thus, ultrasonic scaling showed to be useful in the purpose of removing plaque and can be better developed. Future studies are necessary to establish a comparison with other methods of scaling.

A raspagem com ultrasom odontológico foi usado em 20 dentes extraídos de cães com doença periodontal avançada. A face mesial da raiz foi para a verificação da atuação do instrumento e a distal como controle. A avulsão cirúrgica dos dentes com o uso de alavanca procurou não alterar a superfície radicular a ser avaliada. Após tratamento, os dentes foram preparados para a microscopia eletrônica de varredura. Observou a presença de biofilme e bastonetes penetrando nos túbulos dentinários. A remoção de todo o cálculo dentário e áreas com exposição de dentina nua e com túbulos dentinários abertos. A raspagem fornece uma camada de smear layer. Não se verificou dano a dentina. A raspagem com ultrassom é útil e futuros estudos são requeridos para estabelecer comparativo com outros métodos de raspagem.

Resumo: A doença periodontal (DP) resulta de uma infecção polimicrobiana complexa, levando à destruição dos tecidos periodontais, como consequência da perturbação da homeostase entre a microbiota subgengival e os mecanismos de defesas do hospedeiro em indivíduos suscetíveis. A deficiência estrogênica (DE) é a causa mais comum de osteoporose. A osteoporose é definida como uma doença crônica, multifatorial, provenientes de uma desordem esquelética que promove fragilidade óssea pela redução de sua massa. Vários estudos experimentais têm demonstrado que a estimulação com Campo Eletromagnético Pulsátil (CEMP) pode promover a osteogênese e potencialmente aumentar a mineralização óssea e também, reduzir a inflamação aguda e crônica em tecidos moles e duros. Frente a isso, este estudo teve como objetivo, avaliar por meio da histomorfometria, imunoistoquímica e microtomografia computadorizada (MicroCT), a influência do CEMP na DP induzida em ratas ovariectomizadas e Sham. Para a pesquisa, foram utilizadas 60 ratas adultas (Rattus norvegicus, variação albinus, Wistar) com 3 meses de idade, pesando em torno de 300 gramas e em todos os animais a DP foi induzida. As ratas foram randomizadas em dois grupos experimentais, contendo 30 animais cada, classificados em ovariectomia simulada (Sham) e Ovariectomizada (Ovz), respectivamente. Os grupos foram divididos em dois subgrupos com 15 animais cada: Sham-S (n=15): não receberam terapia com CEMP e este foi nosso grupo controle. Sham-CEMP (n=15): receberam terapia com CEMP. Ovz­O (n=15): não receberam terapia com CEMP. Ovz­CEMP (n=15): receberam terapia com CEMP. As análises histomorfométrica, e MicroCT foram realizadas e os dados obtidos foram submetidos à análise de variância (ANOVA) e teste de Tukey, ambos com nível de significância convencional de 95% e não apresentaram nenhuma diferença estatisticamente significante. Na análise semiquantitativa para os biomarcadores RANKL e OPG, o subgrupo Ovz-O apresentou maior expressão do biomarcador RANKL e menor expressão do biomarcador OPG em relação aos outros subgrupos. Na análise quantitativa da expressão do biomarcador TRAP não foi encontrado nenhuma diferença estatisticamente significante. Apesar de não encontramos evidências significativas da terapia com CEMP na DP em ratas ovariectomizadas, o presente estudo nos sugere que o CEMP pode apresentar um efeito benéfico na remodelação óssea(AU)

Periodontal disease (PD) results from a complex polymicrobial infection, leading to the destruction of periodontal tissues as a consequence of the disturbance of homeostasis between the subgingival microbiota and the host defense mechanisms in susceptible individuals. Estrogen deficiency is the most common cause of osteoporosis. Osteoporosis is defined as a chronic, multifactorial disease from a skeletal disorder that promotes bone fragility by reducing its mass. Several experimental studies have shown that Pulsed Electromagnetic Field (PEMF) stimulation can promote osteogenesis and potentially increase bone mineralization and also reduce acute and chronic inflammation in soft and hard tissues. The aim of this study was to evaluate, through histomorphometry, immunohistochemistry and computerized microtomography (MicroCT), the influence of PEMF on PD induced in ovariectomized and Sham rats. For the research, 60 adult rats (Rattus norvegicus, albinus variant, Wistar) at 3 months of age, weighing around 300 grams were used and in all animals PD was induced. The rats were randomized into two experimental groups, containing 30 animals each, classified as simulated ovariectomy (Sham) and Ovariectomized (Ovz), respectively. The groups were divided into two subgroups with 15 animals each: Sham-S (n = 15): did not receive PEMF therapy and this was our control group. Sham-PEMF (n = 15): received PEMF therapy. Ovz-O (n = 15): did not receive PEMF therapy. Ovz-PEMF (n = 15): received PEMF therapy. The histomorphometric and MicroCT analyzes were performed and the data were submitted to analysis of variance (ANOVA) and Tukey's test, both with a 95% significance level and did not present any statistically significant difference. In the semiquantitative analysis for RANKL and OPG biomarkers, the Ovz-O subgroup showed higher expression of the RANKL biomarker and lower expression of the OPG biomarker in relation to the other subgroups. In the quantitative analysis of TRAP biomarker expression no statistically significant difference was found. Although we did not find significant evidence of PEMF therapy in PD in ovariectomized rats, the present study suggests that PEMF may have a beneficial effect on bone remodeling(AU)

Resumo: O objetivo deste estudo foi avaliar o grau de cooperação dos pacientes da Clínica de Periodontia do Curso de Odontologia da Universidade Federal do Maranhão durante a terapia periodontal de suporte (TPS). Avaliou-se a profundidade de sondagem (PS), recessão gengival (RG), nível de inserção clínica (NIC), mobilidade dentária, envolvimento de furca, índice de placa (IP) e índice de sangramento gengival (ISG) de 16 pacientes adultos em três fases: baseline, T1 e T2. Os indivíduos foram diagnosticados quanto à condição periodontal e responderam ao questionário sobre o impacto na rotina e qualidade de vida nos últimos seis meses pelo índice OHIP-14 (Oral Health Impact Profile). Os dados foram submetidos à análise estatística. Os pacientes foram classificados conforme o grau de cooperação em: colaboradores (75%), colaboradores irregulares (12,5%) e não colaboradores (12,5%). A PS e o NIC dos pacientes colaboradores durante as três fases não sofreram diferenças significativas. O ISG sofreu uma redução comparada ao baseline. A média do IP dos pacientes colaboradores sofreu uma pequena redução, ao contrário dos colaboradores irregulares. Não houve escores de impacto forte na avaliação do OHIP-14, demonstrando uma boa percepção da qualidade da saúde bucal pelos indivíduos. Os pacientes colaboradores tiveram um impacto fraco em relação à doença periodontal nas duas fases do estudo (T1 e T2). Os principais diagnósticos foram a gengivite associada ao biofilme (38,46%) e periodontite crônica avançada localizada (30,76%). Pode-se concluir que os pacientes, em sua maioria, mostraram-se colaboradores na TPS, mantendo sua condição periodontal estável ou progredindo para saúde periodontal.

Motivation and education in mouth health are basic tools to prevent periodontal diseases. Current study assesses patients´ cooperation level at the Periodontal Clinic of the Dentistry Course of the Universidade Federal do Maranhão during Basic Periodontal Therapy (BPT). Sounding depth (PS), gum recession (RG), level of clinical insertion, dental mobility, furca involvement, plaque index (IP) and gum blooding index (ISG) of six adult were assessed clinically at three phases: baseline, T1 and T2. The patients were diagnosed with regard to their periodontal condition and answered a questionnaire on the impact on routine and life quality during the last six months, using Oral Health Impact Profile (OHIP-14). Data underwent statistical analysis and patients were classified according to their cooperation level, namely, collaborators (75%), irregular collaborators (12.5%) and non-collaborators (12.5%). No significant differences existed in collaborators´ PS and NIC during the three phases. ISG decreased when compared to baseline. IP mean of collaborating patients showed a slight decrease when compared to that of irregular collaborators. Since no significant score impact existed in OHIP-14 assessment, there was a good perception by patients with regard to mouth health quality. Collaborating patients had a weak impact with regard to periodontal disease in the two phases (T1 and T2). Main diagnoses were gingivitis plus biofilm gum (38.46%) and local advanced chronic periodontitis (30.76%). Results show that most patients collaborated with BPT, with a stable or advanced periodontal condition in periodontal health.

Resumo: Las hormonas sexuales femeninas además de desempeñar un rol importante en la endocrinología reproductiva, regulan diversas funciones tisulares. En presencia de placa bacteriana (biofilm) asociada a cambios inflamatorios, estas hormonas influyen en el inicio y progreso de la enfermedad periodontal. En el presente estudio se exponen los aspectos morfofuncionales y fisiopatológicos actuales relacionados con dicha temática, con el fin de incrementar el nivel de conocimientos de los estomatólogos y de esta forma mejorar la calidad de la atención que se brinda a los pacientes.

Female sexual hormones besides playing an important role in the reproductive endocrinology, regulate diverse tissue functions. In presence of dental plaque (biofilm) associated to inflammatory changes, these hormones influence in the beginning and progress of periodontal disease. In this study the current morphofunctional and pathophysiological aspects related to that thematic are exposed, with the purpose of increasing the knowledge level of dentists and in this way improving the quality of care offered to patients
