Resumo: Vulvovaginal-gingival syndrome is characterized by erosions and desquamation of the vulva, vagina, and gingiva. We reported a case of a 32-year-old woman presenting with an 8-year history of damage to the vulval and perianal anatomy and limitation of mouth opening. The patient's symptoms were relieved after treatment with topical tacrolimus cream.

Resumo: La enfermedad periodontal es la segunda enfermedad oral más común encontrada en nuestra población. Las mujeres embarazadas han sido consideradas desde siempre un grupo de alto riesgo para la enfermedad periodontal y otros desórdenes orales y dentales. El objetivo de este estudio es determinar el porcentaje de gingivitis marginal y enfermedad periodontal presente en las mujeres embarazadas que acudieron al Hospital Materno Infantil Santísima Trinidad durante el periodo 2007 al 2008. Para lograr los objetivos propuestos, se realizó un estudio observacional descriptivo con componente analítico, de corte transversal, temporalmente retrospectivo. Fueron evaluados 34 pacientes con edades comprendidas entre 14 y 33 años. La media de edad fue de 22 años ± 4,7. El promedio de índice de dientes cariados, perdidos y obturados (CPO) fue de 11 ±6. El porcentaje de gingivitis hallado fue del 74,41% y de periodontitis de 17,65% se observó un pequeño porcentaje de pacientes sin problemas periodontales (2,94%). Con estos datos se puede concluir que la prevalencia de patología periodontal en la población estudiada fue elevada, observándose que la mayor parte de la misma presentaba gingivitis, representando un alto riesgo de evolución a formas más avanzadas de enfermedad periodontal por lo que se recomienda un programa de prevención y controles regulares desde los primeros estudios del embarazo

Resumo: Los adultos jóvenes constituyen gran parte de la población paraguaya y se considera que los conocimientos y prácticas sobre salud bucodental están muy relacionados al nivel de instrucción, el objetivo de este estudio fue evaluar el nivel de conocimientos y prácticas sobre salud bucodental de estudiantes de posgrado de la Carrera de Enfermería de la Facultad de Ciencias de la Salud de la Universidad Católica de Asunción (CE FCSUCA), en el 2018. Estudio observacional descriptivo de corte transversal con componente analítico en el que se incluyeron a todos los estudiantes de posgrado entre mayo y junio de 2018. La recolección de datos se realizó mediante un cuestionario sobre salud bucodental autoadministrado y de carácter anónimo. Los datos se asentaron en una planilla electrónica, y fueron analizados utilizando el Programa estadístico IBMSSPS 23.0. La muestra estuvo constituida por 166 estudiantes, en su mayoría del sexo femenino (75,3%), de entre 20 y 29 años (54,8%), cursando algún programa de especialización (57,8% y pertenecientes al área de la salud (94%). El 57,83% y el 55,42% de los estudiantes presentaron conocimientos y prácticas de salud bucodental de nivel regular, respectivamente. Se encontró asociación entre nivel de conocimiento y de práctica y entre el nivel de conocimiento y la edad. Se concluye que los niveles de conocimientos y prácticas sobre salud bucodental en los estudiantes de posgrado fueron deficientes por lo que se propone la implementación de estrategias educativas de concienciación en el área(AU)

Young adults constitute a large part of the Paraguayan population and considering that oral health knowledge and practices are closely related to the level of education, the objective of this study was to evaluate the level of knowledge and practices on oral health of postgraduate students of the Nursing Career of the Faculty of Health Sciences of the Catholic University of Asunción (CE FCSUCA), in 2018. The design was descriptive, cross-sectional with an analytical component in which all postgraduate students were included in May and June 2018. Data collection was carried out through a questionnaire on oral health self-administered and anonymous. The data were recorded in an electronic spreadsheet and analyzed using the IBMSSPS 23.0 Statistical Program. The sample consisted of 166 students, mostly female (75.3%), between 20 and 29 years old (54.8%), studying some specialization program (57.8%) and belonging to the area of Health (94%). 57.83% and 55.42% of the students presented knowledge and practices of oral health at the regular level, respectively. An association was found between the level of knowledge and practice and between the level of knowledge. It is concluded that the levels of knowledge and practices on oral health in the postgraduate students were insufficient, so the implementation of educational awareness strategies in the area is proposed(AU)

Resumo: ABSTRACT Objectives: The aim of this study is to investigate the prevalence and severity of gingival inflammation and associated risk indicators in South American adults. Material and Methods: Multi-stage samples totaling 1,650 adults from Porto Alegre (Brazil), Tucumán (Argentina), and Santiago (Chile) were assessed. The sampling procedure consisted of a 4-stage process. Examinations were performed in mobile dental units by calibrated examiners. A multivariable logistic regression model was utilized for associating variables as indicators of gingival inflammation (GI) (Gingival Index ≥0.5). Statistical significance was set at 0.05. Results: A total of 96.5% of the adults have GI. Regarding the severity of GI, 22.5% of participants examined have mild GI, 74.0% have moderate GI, and 3.6% have severe GI. The multivariate analyses identify the main risk indicators for GI as adults with higher mean of Calculus Index (OR=18.59); with a Visible Plaque Index ≥30% (OR=14.56); living in Santiago (OR=7.17); having ≤12 years of schooling (OR=2.18), and females (OR=1.93). Conclusions: This study shows a high prevalence and severity of gingival inflammation, being the first one performed in adult populations in three cities of South America.

Resumo: Abstract Objective: Single dose of systemic antibiotics and short-term use of mouthwashes reduce bacteremia. However, the effects of a single dose of preprocedural rinse are still controversial. This study evaluated, in periodontally diseased patients, the effects of a pre-procedural mouth rinse on induced bacteremia. Material and Methods: Systemically healthy individuals with gingivitis (n=27) or periodontitis (n = 27) were randomly allocated through a sealed envelope system to: 0.12% chlorhexidine pre-procedural rinse (13 gingivitis and 13 periodontitis patients) or no rinse before dental scaling (14 gingivitis and 15 periodontitis patients). Periodontal probing depth, clinical attachment level, plaque, and gingival indices were measured and subgingival samples were collected. Blood samples were collected before dental scaling, 2 and 6 minutes after scaling. Total bacterial load and levels of P. gingivalis were determined in oral and blood samples by real-time polymerase chain reaction, while aerobic and anaerobic counts were determined by culture in blood samples. The primary outcome was the antimicrobial effect of the pre-procedural rinse. Data was compared by Mann-Whitney and Signal tests (p<0.05). Results: In all sampling times, polymerase chain reaction revealed higher blood bacterial levels than culture (p<0.0001), while gingivitis patients presented lower bacterial levels in blood than periodontitis patients (p<0.0001). Individuals who experienced bacteremia showed worse mean clinical attachment level (3.4 mm vs. 1.1 mm) and more subgingival bacteria (p<0.005). The pre-procedural rinse did not reduce induced bacteremia. Conclusions: Bacteremia was influenced by periodontal parameters. In periodontally diseased patients, pre-procedural rinsing showed a discrete effect on bacteremia control.

Resumo: Abstract The major infectious diseases in Chile encompass the periodontal diseases, with a combined prevalence that rises up to 90% of the population. Thus, the population-based surveillance of periodontal diseases plays a central role for assessing their prevalence and for planning, implementing, and evaluating preventive and control programs. Self-report questionnaires have been proposed for the surveillance of periodontal diseases in adult populations world-wide. Objective This study aimed to develop and assess the content validity and reliability of a cognitively adapted self-report questionnaire designed for surveillance of gingivitis in adolescents. Material and Methods Ten predetermined self-report questions evaluating early signs and symptoms of gingivitis were preliminary assessed by a panel of clinical experts. Eight questions were selected and cognitively tested in 20 adolescents aged 12 to 18 years from Santiago de Chile. The questionnaire was then conducted and answered by 178 Chilean adolescents. Internal consistency was measured using the Cronbach's alpha and temporal stability was calculated using the Kappa-index. Results A reliable final self-report questionnaire consisting of 5 questions was obtained, with a total Cronbach's alpha of 0.73 and a Kappa-index ranging from 0.41 to 0.77 between the different questions. Conclusions The proposed questionnaire is reliable, with an acceptable internal consistency and a temporal stability from moderate to substantial, and it is promising for estimating the prevalence of gingivitis in adolescents.

Resumo: El término penfigoide de membranas mucosas designa un grupo de enfermedades vesículo-bulosas autoinmunes crónicas, que afectan predominantemente mucosas. El presente artículo describe dos casos clínicos de penfigoide de membranas mucosas con compromiso gingival y tuvo como objetivo resaltar la importancia de un correcto diagnóstico de la lesión y discutir la conducta terapéutica. Fueron realizados diversos exámenes complementares como, examen de Nikolsky, biopsia incisional, investigación de células LE y de anticuerpos antinucleares, además de la evaluación de médicos especializados en dermatología y oftalmología. Fue fundamental la substitución de dentífricos, la orientación nutricional y psicológica, el cuidado con la higiene, además del uso de corticoides tópicos. Hubo periodos de remisión y exacerbación del cuadro clínico durante el seguimiento, fue necesario el ajuste en la terapéutica y refuerzo en los cuidados con la higiene bucal. Es muy importante la interacción multidisciplinaria en la atención de estos casos para el control del tratamiento y seguimiento, además de reforzar todas las orientaciones y cuidados en lo que se refiere a la salud bucal y cautela en el uso de corticoides(AU)

The term Mucous Membrane Pemphigoid includes a group of chronic and autoimmune vesiculobullous diseases involving mainly the mucosa. The aim of present paper is to describe two clinical cases of Mucous Membrane Pemphigoid with gingival involvement to arrive to a proper diagnosis of the lesion and also to discuss the therapeutic behavior. Different complementary examinations were conducted including Nikolsky test, incisional biopsy, the LE cells study and of antinuclear antibodies, as well as the evaluation of dermatologists and ophthalmologists. It was necessary the replacement of toothpastes, the nutritional and psychological guiding, the hygiene care and the use of topic corticoids. There were periods of remission and exacerbation of clinical picture during follow-up being necessary the therapeutics fitting and the oral hygiene care reinforcement. It is important the multidisciplinary interaction in the care of these cases for treatment and follow-up control, as well as to reinforce all orientations and cares related to oral hygiene and the caution in the use of corticoids(AU)

Título: Oral lesions and retroviruses in shelter cats / Afecções orais e retroviroses em gatos de abrigo

Resumo: Oral lesions are common problems in feline medicine worldwide, and may be associated with different causes, such as infectious agents. There are only a few studies reporting the chief oral diseases and the results for retrovirus tests in shelter cats in Brazil, especially in the South region. This study aimed to identify the main inflammatory oral lesions in shelter cats and verify the test results for feline immunodeficiency virus (FIV) and feline leukemia virus (FeLV) infections. Forty-three felines from private shelters in the central region of Rio Grande do Sul state (RS) that presented clinically evident oral lesions, regardless of age, breed, sex, and neuter status, were used in this survey. Serological tests for FIV and FeLV were performed in all cats, and data regarding the rearing system were collected. Sixteen cats (37.2%) were reared in a free system, whereas 27 (62.8%) were kept under a restrict system. Of the 43 cats with oral lesions, 29 (67.44%) presented only one type of lesion, characterized as periodontitis (n=22, 51.16%), followed by gingivitis (n=6, 13.95%), and stomatitis (n=1, 2.32%). Concomitant stomatitis and periodontitis were found in the 14 remaining cats (100%). With respect to the test results for retrovirus infections, nine (20.93%) of the 43 felines were positive for FIV alone. Co-infection with both viruses was observed in seven cats (16.28%). No cat was seropositive for FeLV valone. None of the six cats that presented gingivitis was positive for FIV and FeLV; one cat with stomatitis was positive for FIV and FeLV; of the 22 cats with periodontitis, six (27.27%) were FIV positive and two (9.09%) were FIV/FeLV positive; and of the 14 cats that presented stomatitis and periodontitis, three (21.43%) were FIV positive and four (28.57%) were FIV/FeLV positive. As for diagnosis, 28 cats (65.1%) presented solely periodontal disease (PD), one cat (2.32%) had feline chronic gingivostomatitis (FCG) alone, and 14 (32.5%) had both PD and FCG. The results obtained show that the main oral lesions found in shelter cats in the central region of RS were gingivitis, stomatitis, and periodontitis. Periodontitis, in association or not with stomatitis, was the most frequently observed oral cavity lesion in FIV- and/or FeLV-positive cats. Other factors may contribute to installation of inflammatory oral diseases in shelter cats because most cats with oral cavity lesions tested negative for retrovirus infections.(AU)

As afecções orais são problemas comuns em medicina felina em diferentes locais do mundo e podem estar relacionadas a diferentes causas, como agentes infecciosos. Poucos estudos foram encontrados no Brasil sobre o levantamento das principais doenças orais e dos resultados de testes para retrovírus em gatos de abrigos, principalmente na região Sul. Diante disso, o objetivo deste artigo foi identificar as principais afecções orais inflamatórias em gatos de abrigos e verificar os resultados dos testes para o vírus da imunodeficiência felina (FIV) e o vírus da leucemia felina (FeLV). Foram incluídos 43 felinos provenientes de abrigos privados localizados na região central do Rio Grande do Sul (RS) que apresentavam lesões orais clinicamente evidentes, independente de idade, raça, gênero e estado reprodutivo. Em todos os gatos foram realizados testes sorológicos para FIV e FeLV e obtidas informações referentes ao sistema de criação. Em 16 gatos (37,2%), o sistema de criação era livre, enquanto em 27 (62,8%) era restrito. Dos 43 gatos com lesões orais, em 29 (67,44%) foi verificado somente um tipo de lesão, caracterizado como periodontite (n=22, 51,16%), seguido de gengivite (n=6, 13,95%) e estomatite (n=1, 2,32%). Lesões concomitantes de estomatite e periodontite foram encontradas nos 14 gatos (100%) restantes. Quanto aos resultados dos testes para retrovírus, nove (20,93%) dos 43 felinos testados, foram positivos somente para FIV. Em sete gatos (16,28%) foi observada coinfecção pelos dois vírus. Em nenhum gato foi observado soropositividade somente para FeLV. Dos seis gatos com gengivite, nenhum foi positivo para FIV e FeLV; um gato com estomatite foi positivo para FIV e FeLV; dos 22 gatos com periodontite, seis (27,27%) foram FIV positivos e dois (9,09%) FIV/FeLV positivos; e dos 14 com estomatite e periodontite, três (21,43%) foram FIV positivos e quatro (28,57%) FIV/FeLV positivos. Quanto ao diagnóstico, em 28 gatos (65,1%) foi observada somente doença periodontal (DP), em um (2,32%) somente gengivoestomatite crônica felina (GECF) e em 14 gatos (32,5%) DP e GECF. Diante dos resultados obtidos, pode-se concluir que as principais lesões orais encontradas em gatos de abrigos da região central do RS foram gengivite, estomatite e periodontite; a periodontite associada ou não a estomatite foi a lesão oral mais frequente nos gatos positivos para FIV e/ou FeLV. Acredita-se que outros fatores possam contribuir na instalação de doenças orais em gatos de abrigos, já que houve predomínio de gatos com resultados negativos nos testes para os retrovírus.(AU)

Resumo: Desde hace algunos años se observa en nuestras universidades de ciencias médicas estudiantes chinos de forma masiva, evento sin precedentes en nuestro país. Para el dominio de todo lo referente a su salud bucal y al proceso salud-enfermedad de los mismos. Objetivo: identificar las principales enfermedades bucales en estudiantes chinos para que de esta forma se logre una calidad en la atención de los mismos. Métodos: se realizó una investigación descriptiva transversal. Se tomó una muestra de 185 estudiantes chinos. Se le realizó un examen bucal y facial y una entrevista para obtener las variables de interés en la investigación. Resultados: las gingivitis y las maloclusiones fueron las enfermedades bucales más prevalecientes; la caries dental fue la que menos se manifestó. Tanto el sexo masculino como el femenino prevalecieron como enfermos y el origen social rural aportó los únicos pacientes discapacitados deficientes. Conclusiones: la higiene bucal es el factor de riesgo más relacionado con las Gingivitis y Caries Dental, así como el origen social rural determinó una peor condición epidemiológica(AU)

Recently, Chinese students are being massively received in our medical universities. This is an unprecedented event in our country. For mastering everything about their oral health and their health-disease process, a stomatologic study was conducted on them. Objective: to identify the main oral diseases in these subjects to provide quality health care for them. Methods: a descriptive study was conducted, taking a sample of 185 Chinese students. They had oral and facial examination and an interview for this investigation variables was applied. Results: gingivitis and malocclusions were the most prevalent oral diseases, dental caries manifested the least. Both male and female subjects prevailed as patients. Rural social background provided the only poor disabled patients. Conclusions: oral hygiene is the risk factor most related to gingivitis y dental caries. The rural social origin determined a worse epidemiological condition(AU)

Resumo: El granuloma telangiectásico se considera como una lesión benigna no neoplásica de la mucosa gingival de tipo reactivo asociado a trauma. Durante varios años tuvo diferentes nombres dentro de los que destacan granuloma piógeno del embarazo o gravídico, hemangioma capilar lobular y épulis. Histológicamente se caracteriza por una proliferación vascular, infiltrado inflamatorio crónico y tejido de granulación. El objetivo del presente caso es describir las características clínicas e histológicas de un granuloma telangiectásico en cavidad oral y su plan de tratamiento. Llega a consulta paciente femenina de 50 años de edad, refiere presentar sangrado gingival en repetidas ocasiones en zona posterior de la arcada dentaria superior, durante el cepillado o con el consumo de algunos alimentos. Clínicamente se observa prótesis provisional desadaptada en órganos dentarios 11 al 15. Al retirar la prótesis es notorio un aumento de volumen en la encía interdental, con aspecto eritematoso, que sangra con facilidad. El reporte de anatomía patológica mostró una lesión con engrosamiento del epitelio escamoso, formación de nuevos vasos sanguíneos e infiltrado inflamatorio crónico. Diagnosticado como granuloma telangiectásico. La lesión fue eliminada quirúrgicamente por alargamiento coronal con reducción ósea. Ocho días después de la cirugía se evidencia proceso de cicatrización sin ninguna complicación. La lesión presentó recidiva transcurrido seis meses. Ello nos lleva a destacar la importancia del control de factores locales, como la presencia de prótesis desadaptadas y el acúmulo de placa bacteriana luego de la eliminación quirúrgica de este tipo de lesiones(AU)

Telangiectatic granuloma is a non-neoplastic benign lesion of the gingival mucosa, reactive type, associated with trauma. For several years, it had different names among which we can mention the pyogenic granuloma gravidarum occurring during pregnancy, the lobular capillary hemangioma and the epulis. Histologically, it is characterized by a vascular proliferation, chronic inflammatory infiltrate and granulation tissue. The aim of this case is to describe the clinical and histological characteristics of the telangiectatic granuloma of the oral cavity and its treatment plan. A 50-year-old female patient comes to the consultation reporting repeated gingival bleeding on the posterior area of the upper dental arch when brushing or consuming some food. Clinically, unadjusted provisional prosthesis was observed in the dental organs from 11 to 15. After removing the prosthesis, an increase in the volume of the interdental gum of erythematous type which bleeds easily was obvious. The pathologic report showed a lesion with swelling of the flaky epithelium, formation of new blood vessels and chronic inflammatory infiltrate. The diagnosis was telangiectatic granuloma. The lesion was surgically removed by crown lengthening with osseous reduction. Eight days after surgery, a healing process without complications was seen. 6 months later, the lesion relapsed. This fact leads us to outline the importance of the control of local factors, such as the presence of unadjusted prosthesis and the accumulation of dental plaque after the surgical removal of this type of lesions(AU)
