Resumo: Abstract This study aimed at assessing the relationship between dental pain and the reason for using dental services and oral health quality of life in people aged 50 to 74 years in southern Brazil. A cross-sectional population-based study was conducted with 720 individuals aged 50 to 74 years, living in three health districts in the city of Porto Alegre. Dental impacts on daily life and sociodemographic data were assessed using structured interviews. The Oral Impacts on Daily Performance - OIDP instrument was used to measure oral impacts. The information was analyzed by Poisson regression with robust variance adjustment, taking into account cluster sampling. Dental pain was present in 32.5% of those reporting an oral impact on their daily activities. Dental pain most frequently affected talking (37.6%), cleaning teeth and gums (37.0%) and enjoying the companionship of people (36.5%). After adjustments to the multivariate analysis, the reason for dental visits due to dental pain was found to have a high impact on daily activities [RP 1.68 (1.11 - 2.54].

Título: Acutely infected teeth: to extract or not to extract?

Resumo: Abstract: Not only laymen but also dentists generally believe that extraction of acutely infected teeth should be avoided until the infection subdues by using systemic antibiotics. The aim of this study was to compare perioperative complications in routine extractions of acutely infected teeth with extractions of asymptomatic teeth. This prospective study was performed with 82 patients. Severe pain on percussion of the relevant tooth was considered as basic criteria for acute infection. The acutely infected teeth were labeled as the study group (n = 35) and the asymptomatic teeth as the control group (n = 47). The extractions were done using standard procedures. The amount of anesthetic solution used and duration of extractions were recorded. Postoperative severe pain and exposed bone with no granulation tissue in the extraction socket were indications of alveolar osteitis (AO). The level of statistical significance was accepted as 0.05. Symptoms that could indicate systemic response, including fever, fatigue, and shivering were not found. There was no statistically significant difference between groups in terms of AO, amount of anesthetic solution used, and duration of extraction. The presence of an acute infection characterized by severe percussion pain is not a contraindication for tooth extraction. Infected teeth should be extracted as soon as possible and the procedure should not be postponed by giving antibiotics.

Resumo: Abstract: The aim of this study was to describe and evaluate access to oral health services among adolescents enrolled in public schools of Campina Grande, Paraíba, Brazil, a large-size municipality in the Northeast of Brazil. An observational, descriptive, analytical, quantitative, and cross-sectional study was carried out through a school survey, in which four validated questionnaires were applied to 438 adolescents aged 12 to 19 years. Data were processed using the SPSS statistical software version 20.0, with bivariate analysis and multivariate analysis through Poisson regression. About 90.9% of adolescents reported having visited the dentist at least once; however, when considering the last 6 months, this percentage fell to 48.4%. Adolescents used private services (50.2%) or the public service (49.8%). About 70.6% of interviewees reported dental treatment (61.1%) as the main reason for seeking the service. Through multivariate analysis, it was observed that the demand for the service was higher among female adolescents (60.5%, p <0.001); the other variables did not present statistically significant differences. In conclusion, the access to oral healthcare services reported by adolescents was good, but there is still a considerable part of this population with no access. Variables that presented significant associated with dental services were gender and toothache history, but only gender remained significant in the multivariate model.

Título: Factors associated with toothache among Brazilian adults: a multilevel analysis

Resumo: Abstract The aim of this study was to evaluate the factors associated with toothache in the adult population of Minas Gerais, Brazil. Individual data from a population sample (age 35 to 44 years) were collected from a secondary database of the SB Minas survey. Sampling was carried out by clusters and with multiple drawing stages. The eligibility criteria were to reside in areas chosen for the research, be within the age group, and accept to participate in the research. The individual variables assessed by a questionnaire and dental exams were sex, income, race/skin color, root caries, periodontal condition, need for dental treatment, and last dental appointment. The contextual variables, assessed by municipal indexes, were Human Development Index (HDI), illiteracy, unemployment, half minimum wage, quarter minimum wage, oral health team coverage, access to individual health care, and supervised tooth brushing average. The dependent variable was toothache in the past six months. A descriptive analysis was made using the Statistical Package for the Social Sciences and Hierarchical Linear and Nonlinear Modeling Software was used to perform the multilevel analyses for individual and contextual levels. An association was found between toothache and low income (OR = 2.00; 95%CI = 1.32-3.13), dental caries (OR = 1.86; 95%CI = 1.22-2.86), periodontal condition, and living on a quarter of the minimum wage or less (OR = 1.03; 95%CI = 1.00-1.08). Clinical and social factors were associated with toothache, reinforcing the need to improve public polices in oral health focused on the adult population.

Resumo: Abstract Dental pain is usually managed by clinical interventions and pharmacological coadjuvants such as NSAIDs. However, its perception and modulation is mediated by different nociceptive mechanisms and these strategies can be insufficient. The multimodal analgesia refers to the use of 2 or more analgesic drugs that attenuate or blockade different mechanisms of pain, obtaining a greater clinical effect. Within this concept, pharmacological synergism plays a leading role, combining different molecules in lower doses to diminish also side effects. Since there are no standard prescriptions to be use in all the patients, multimodal approaches allow the clinician to make responsible effective combinations, individualizing analgesia as the pathway to success.

Resumen El dolor dental generalmente se trata con intervenciones clínicas y coadyuvantes farmacológicos como los AINEs. Sin embargo, su percepción y modulación está mediada por diferentes mecanismos nociceptivos y estas estrategias pueden ser insuficientes. La analgesia multimodal se refiere al uso de 2 o más fármacos analgésicos que atenúan o bloquean diferentes mecanismos de dolor, obteniendo un mayor efecto clínico. Dentro de este concepto, el sinergismo farmacológico juega un papel importante, combinando diferentes moléculas en dosis más bajas para disminuir también los efectos secundarios. Dado que no hay prescripciones estándar para ser usadas en todos los pacientes, los enfoques multimodales permiten al clínico realizar combinaciones responsables y eficaces, individualizando la analgesia como el camino hacia el éxito.

Resumo: Se realizó un estudio observacional, descriptivo y transversal para caracterizar el comportamiento de las urgencias estomatológicas en los niños de los círculos infantiles del área de salud del Policlínico Universitario 27 de Noviembre del municipio Marianao, La Habana. El universo estuvo constituido por 450 niños de ambos sexos, matriculados en los tres círculos infantiles del área de salud, durante el curso escolar 2006-2007. La muestra estuvo representada por 175 niños con diagnóstico de lesiones por caries dental y sus secuelas, en los cuales se tuvo en cuenta edad, sexo, frecuencia de asistencia a la consulta de urgencias y severidad de la caries. La frecuencia de asistencia a consulta de urgencia siguió un patrón de más de una vez en todas las edades excepto el grupo de 4 años, en el que la mayoría asistió solamente una vez. El 22,2 por ciento de los niños de 5 años necesitaron el servicio de urgencia en dos ocasiones. El dolor fue la causa más frecuente de asistencia a la consulta de urgencia, y se manifestó, solamente en las edades de 4 y 5 años, con predominio de severidad grado 1, y de ellos el sexo masculino estuvo más afectado. La asistencia a consulta de urgencias aumentó con la edad, con predominio del sexo masculino, quienes acudieron más de una vez por la presencia de dolor, a pesar de predominar caries de severidad grado 1(AU)

A cross-sectional, descriptive and observational study was conducted to characterize the behavior of Stomatology urgencies in children of Nurseries from 27 de Noviembre polyclinic health area of Marianao municipality in La Habana, Cuba. Universe included 450 boys and girls, registered in the three nurseries of health area diagnosed with dental caries analyzing age, sex, frequency of urgency consultation assistance and caries's severity. In parents a questionnaire was administered. From the 175 boys and girls affected of dental caries, there were 56 with a four years involvement due to caries being the more frequent to urgency consultation for a 10,9 percent and the highest frequency was twice, in boys and girls aged 5 for a 5,7 percent. A total of 17 children visited more frequent the urgency consultation with two visits for a 9,7 percent. Boys and girls aged 5 were more frequent to above mentioned consultation due to pain followed by those aged 4. There was predominance of I degree severity for a 68,1 percent and in these suffered of pain for a 27,6 percent. There is an increasing assistance to urgency consultation increase according to age with predominance of male sex, requiring this type of serve often due to pain despite predominance of 1 degree severity of caries(AU)

Resumo: Abstract Objective: To investigate the impacts of toothache on the daily activities of children aged 5 to 9 years. Material and Methods: A cross-sectional analytical study was conducted among 465 children (220 boys and 245 girls) aged between 5-9 years, using the Child Dental Pain Questionnaire (Child-DPQ), in the city of Kerman. They were categorized into two groups based on the clinical status: (1) untreated dental caries and (2) no dental caries or treated dental caries. For data analysis, the Chi-square, Mann-Whitney test, multiple logistic regressions, Spearman's correlation coefficient and t-test were used. Results: Regarding the severity of pain, about 28.3% reported mild pain, 30.1% moderate pain, and 21.9% very severe pain. The prevalence of reported dental pain increased by increasing the number of reported dental visits and more strongly among those having dmft>1. Toothache in children caused to stop eating (73.3%) and to sleep (63.8.1%) and provoked a dental visit in more than 40%. Conclusion: The toothache stopped many children eating and sleeping and provoked a dental visit in more than 40%. Among the children under study, 55.9% had to miss school.

Resumo: Al realizar un diagnóstico endodóntico se presentan signos y síntomas que generan confusión en éste. Ante esta situación, es nuestra intención realizar una investigación documental (libros, revistas, internet y entrevistas) donde se describan dichos signos y síntomas. Autores consultados mencionan algunas patologías como periodontitis apical aguda, oclusión traumática, cefalea en racimo, absceso Periodontal y neuralgia del trigémino entre otras, que presentan una sintomatología similar a las encontradas en una odontalgia por lo que se puede complicar el diagnóstico. Al finalizar la investigación documental se concluye que el dolor dental es el síntoma más común que crea confusión al diagnosticar, pero realizando un diagnóstico exhaustivo, no habrá duda al momento de diferenciar entre una pulpalgia de otra patología.

When making an endodontic diagnosis, signs and symptoms are presented that generate confusion in it. Faced with this situation, it is our intention to carry out a documentary investigation (books, magazines, internet and interviews) where these signs and symptoms are described. Authors consulted mention some pathologies such as acute apical periodontitis, traumatic occlusion, cluster headache, Periodontal abscess and trigeminal neuralgia, among others, which present symptoms similar to those found in odontalgia, which can complicate the diagnosis. At the end of the documentary research, it is concluded that dental pain is the most common symptom that creates confusion when diagnosing, but making an exhaustive diagnosis, there will be no doubt when differentiating between a pulpalgia from another pathology.

Resumo: Objetivos: analizar y describir los requerimientos de atención quirúrgica y no quirúrgica de urgencia en la población infantil y juvenil en un servicio de urgencias de Argentina durante el aislamiento social preventivo y obligatorio (ASPO). Materiales y métodos: se realizó un estudio observacional y descriptivo de análisis retrospectivo durante el período inicial de 93 días de ASPO, comprendido entre 20 de marzo de 2020 hasta el 20 de junio de 2020. Se registraron los pacientes menores de 17 años que acudieron al servicio de urgencias del Hospital Odontológico de la Universidad de Buenos Aires y se caracterizó aquellos que requerían distintos tipos de tratamientos. Resultados: se atendieron 4654 pacientes durante el periodo estudiado, de los cuales 116 eran menores de 17 años. Se realizaron 142 prestaciones. A 63 piezas dentarias se les realizó la exodoncia. A otras 63, algún tipo de tratamiento endodóntico. 11 piezas requirieron de restauraciones directas, y en 5 se realizaron otros tipos de tratamientos. Conclusión: fue importante la presencia de recurso humano calificado en el área de la cirugía bucal y pediátrica en los servicios de guardia, considerando el incremento sustancial de necesidad de tratamiento suscitado durante el ASPO (AU)

Objective: to analyse and describe the requirements for emergency surgical and non-surgical care in the paediatric population in an emergency department in Argentina during Preventive and Mandatory Social Isolation. Materials and Methods: an observational, descriptive, retrospective study was conducted during the initial period of 93 days of Preventive and Mandatory Social Isolation, from March 20, 2020 to June 20, 2020. Patients under 17 years of age who attended the emergency department of the Dental Hospital of the University of Buenos Aires were registered and those requiring different types of treatment were characterised. Results: 4654 patients were seen during the period studied, 116 of whom were under 17 years of age. A total of 142 services were performed. Sixty-three teeth were exodontically extracted. A further 63 underwent some form of endodontic treatment. 11 teeth required direct restorations and 5 required other types of treatment. Conclusion: the presence of qualified human resources in oral and paediatric surgery in the on-call services was important, considering the substantial increase in the need for treatment during the ASPO (AU)

Título: #Dentalpain: what do the brazilian Instagram® users want to mean?

Resumo: Aim: The aim of this study was to evaluate what brazilian Instagram® users manifest when they use #dordedente (#dentalpain in portuguese). Methods: The content of 1,000 publications related to dental pain was evaluated. Through print screens posts were collected in two periods (2015 and 2020) and organized into files. Variables related to characteristics of users, publication's type, cause of pain, impact on daily life and actions take to relieve pain were collected. Data analysis was realized through the qualitative methodology of conventional content analysis and a descriptive analysis (95% CI) was performed. Results: In the first assessment period 76% of the posts referred to self-declaration of pain. Regarding to the users' characteristics, 54% were female, 20.4% male, and the remaining 25.6% were clinic/ company profiles, or it was not possible to identify the user's gender. Apparently, 34.4% of users were adults, 18.6% were adolescents and only 1.2% were children. On the other hand, in the second evaluation the vast majority (99.6%) were clinic/company profiles. In addition, 90.2% were informative posts and only 3 posts (0.6%) were related to self-declaration of pain. Conclusions:The social network Instagram® serves as an efficient means of communication for informative and advertising purposes being an interesting alternative for the dissemination of health information, and as an instrument of health surveillance
