Resumo: Introducción: Los trastornos hipertensivos son la primera causa de muerte materna en los países desarrollados y la tercera causa de muerte materna en los países en vías de desarrollo. La preeclampsia es una enfermedad de origen desconocido y multifactorial cuyo tratamiento definitivo es el parto, además de ser causal de repercusiones sobre la madre y el recién nacido. Objetivo: El objetivo general del estudio fue determinar la Prevalencia de preeclampsia en embarazadas en el servicio de ginecología y obstetricia del hospital central del instituto de previsión social, 2017. Materiales y métodos: Estudio observacional descriptivo retrospectivo de corte transversal, con muestreo no probabilístico de casos consecutivos. Fueron incluidas todas las embarazadas con preeclampsia que acudieron al Servicio de Ginecología y Obstetricia del Hospital Central de Instituto de Previsión Social en el periodo comprendido entre los meses de enero a diciembre del año 2017. Resultados: Se realizó un estudio observacional descriptivo retrospectivo en 375 pacientes que acudieron al Hospital Central del Instituto de Previsión Social en el año 2017. Los resultados arrojaron que el 38,4% presentó preeclampsia de las cuales 63,2% presentaba en preeclampsia leve y 36,8% presentaba preeclampsia severa Entre los factores de riesgo el que apareció en mayor cantidad fue la Hipertensión Arterial Crónica. En cuanto a las complicaciones maternas se presentan en mayoría Hepáticas, Neurológicas y Renales. Conclusión: Los resultados de este estudio realizado en un centro de referencia nacional y de alta complejidad coinciden con estudio previo, determina datos importantes acerca de la prevalencia de preeclampsia.

Título: Evaluation of the profile of inflammatory cytokines, through immunohistochemistry, in the skin of patients with allergic contact dermatitis to nickel in the acute and chronic phases

Resumo: Abstract: Background: Allergic contact dermatitis to ion nickel (Ni+2) is an inflammatory dermatosis, common in industrialized countries. It involves the activation of nickel-specific T-cells, followed by proliferation and induction of a mixed profile of both proinflammatory and regulatory cytokines, suggesting that several T-cell subtypes (helper - Th and cytotoxic - Tc) are involved. A broader understanding of the cytokine profile may lead to new therapeutic approaches. Objectives: This study aimed to analyze the cytokines TNF-α, INF-γ, IL-2, IL-4, IL-10, IL-13, IL-17 and IL-23 using the immunohistochemistry technique in order to try to identify their prevalence in chronic and acute eczema of patients with allergic contact dermatitis to Ni+2. Methods: We performed an immunohistochemical study for eight cytokines in 20 patients with Ni+2 allergic contact dermatitis, biopsied at the site of chronic eczema, triggered by the patient's daily contact with Ni+2, and at the site of acute eczema caused by nickel sulfate, 48 hours after applying the contact test. Results: The stained samples showed positive results for the eight cytokines studied. TNF-α, IFN-γ, IL-4, IL-13 and IL-17 had a higher prevalence in chronic eczema, IL-2 and IL-23 in acute eczema, and IL-10 presented a similar prevalence in both acute and chronic eczema. However, these prevalences were statistically significant only for IL-4 and IL-13. Study Limitations: Small sample size. Conclusions: In chronic and acute eczema, we observed the presence of a mixed cytokine profile of the T cell subtypes (Th/Tc), suggesting that the responses are expressed at the same time.

Resumo: Abstract: Background: Diseases caused by melanized fungi include mycetoma, chromoblastomycosis and phaeohyphomycosis. This broad clinical spectrum depends on the dynamic interactions between etiologic agent and host. The immune status of the host influences on the development of the disease, as, an exemple. phaeohyphomicosis is more frequently observed in immunocompromised patients. Objectives: Examine the histological inflammatory response induced by Fonsecaea pedrosoi in several different strains of mice (BALB/c, C57BL/6, Nude and SCID, and reconstituted Nude). Methods: Fonsecaea pedrosoi was cultivated on agar gel and a fragment of this gel was implanted subcutaneously in the abdominal region of female adult mice. After infection has been obtained, tissue fragment was studied histopathologically. Results: There were significant changes across the strains, with the nodular lesion more persistent in Nude and SCID mice, whereas in immunocompetent mice the lesion progressed to ulceration and healing. The histopathological analysis showed a significant acute inflammatory reaction which consisted mainly of neutrophils in the initial phase that was subsequently followed by a tuberculoid type granuloma in immunocompetent mice. Study limitations: There is no a suitable animal model for chromoblastomycosis. Conclusions: The neutrophilic infiltration had an important role in the containment of infection to prevent fungal spreading, including in immunodeficient mice. The fungal elimination was dependent on T lymphocytes. The re-exposure of C57BL/6 mice to Fonsecaea pedrosoi caused a delay in resolving the infection, and appearance of muriform cells, which may indicate that re-exposure to fungi, might lead to chronicity of infection.

Resumo: Abstract: Background: Urticarias are frequent diseases, with 15% to 20% of the population presenting at least one acute episode in their lifetime. Urticaria are classified in acute ( ≤ 6 weeks) or chronic (> 6 weeks). They may be induced or spontaneous. Objectives: To verify the diagnostic and therapeutic recommendations in chronic spontaneous urticaria (CSU), according to the experience of Brazilian experts, regarding the available guidelines (international and US). Methods: A questionnaire was sent to Brazilian experts, with questions concerning diagnostic and therapeutic recommendations for CSU in adults. Results: Sixteen Brazilian experts answered the questionnaire related to diagnosis and therapy of CSU in adults and data were analyzed. Final text was written, considering the available guidelines (International and US), adapted to the medical practices in Brazil. Diagnostic work up in CSU is rarely necessary. Biopsy of skin lesion and histopathology may be indicated to rule out other diseases, such as, urticarial vasculitis. Other laboratory tests, such as complete blood count, CRP, ESR and thyroid screening. Treatment of CSU includes second-generation anti-histamines (sgAH) at licensed doses, sgAH two, three to fourfold doses (non-licensed) and omalizumab. Other drugs, such as, cyclosporine, immunomodulatory drugs and immunosuppressants may be indicated (non-licensed and with limited scientific evidence). Conclusions: Most of the Brazilian experts in this study partially agreed with the diagnostic and therapeutic recommendations of the International and US guidelines. They agreed with the use of sgAH at licensed doses. Increase in the dose to fourfold of sgAH may be suggested with restrictions, due to its non-licensed dose. Sedating anti-histamines, as suggested by the US guideline, are indicated by some of the Brazilian experts, due to its availability. Adaptations are mandatory in the treatment of CSU, due to scarce or lack of other therapeutic resources in the public health system in Brazil, such as omalizumab or cyclosporine.

Resumo: Este estudo teve por objetivo verificar os fatores associados à prática de atividade física de lazer em idosos com diabetes tipo 2. Realizou-se um estudo transversal de base populacional, com 204 idosos com diabetes tipo 2, cadastrados em 4 unidades da saúde da família de Passo Fundo, Rio Grande do Sul. Utilizou-se questionários as variáveis sociodemográficas e de saúde, e o nível de atividade física o questionário internacional de atividade física IPAQ. Foram realizadas análises brutas e multivariadas por regressão de Poisson, com intervalos de confiança de 95% a um p≤0.050. Após análise ajustada para a prática de atividade física de lazer se manterão associadas significativamente as variáveis: faixa etária de 60 a 69 anos (RP: 1,96; IC95%: 1,06-3,62), não ter apresentador dor (RP: 7,20; IC95%: 3,42-15,16) e não ter sofrido quedas nos últimos 6 meses (RP: 1,88; IC95%: 1,01-3,52).

This study aimed to verify the factors associated witch the practice of physical activity of leisure in elderly people with type 2 diabetes. A population-based cross-sectional study was carried out with 204 elderly people with type 2 diabetes, enrolled in 4 health units of the Family of Passo Fundo, Rio Grande do Sul. The sociodemographic and health variables questionnaires were used, and the physical activity level was the International IPAQ physical activity questionnaire. Gross and multivariate analyses were performed by Poisson regresseion, with confidence intervals of 95% at a p≤0.050. After the adjusted analysis for the practice of leisure physical activity, the following variables will be significantly associated: age range from 60 to 69 yeras (RP: 1,96; 95%CI: 1,06-3,62), no presenter pain (RP: 7,20; IC95%: 3,42- 15,16) and did not suffer falls in the last 6 months (RP: 1,88; IC95%: 1,01-3,52).

Resumo: Abstract Vegetative chronic genital herpes is an atypical presentation of herpes simplex 2 that it is usually seen in patients coinfected with human immunodeficiency virus. Clinically, it is characterized by extensive ulcers that evolve to chronification and hypertrophic pseudotumor forms. Antiviral drugs are recommended for the treatment, and acyclovir is the most used one. Foscarnet is the treatment of choice to resistant cases, although treatment failure has been reported. We report a male patient, previously diagnosed with human immunodeficiency virus who developed vegetative chronic genital herpes resistant to acyclovir and successfully treated with imiquimod.

Resumo: En 1978, la Organización Mundial de la Salud definió la atención primaria de la salud como pilar fundamental de la salud global. En los últimos años, a las causas habituales de mortalidad (infecciosas, carenciales) se sumaron las provocadas por enfermedades crónicas y degenerativas. Es posible brindar atención de alta complejidad a pacientes sin recursos adecuados para su supervivencia luego del egreso. El sistema de salud no supo adaptarse a este cambio y mantuvo un modelo fragmentado, reactivo y centrado en la enfermedad.La "atención primaria selectiva" permite reorientar las bases de la atención primaria según las características de la población. Un grupo de acciones específicas, dirigidas a una población con vulnerabilidades particulares, potencia las virtudes de la atención primaria de la salud. Se describe el modelo de atención "Cuidados Especiales Ambulatorios Pediátricos" implementado en el Centro de Salud y Acción Comunitaria N.º 5 de la Ciudad de Buenos Aires

In 1978, the World Health Organization defined primary health care as the cornerstone of global health. In recent years, the usual causes of mortality (infections, deficiency diseases) now include chronic and degenerative diseases. It is now possible to provide tertiary care to patients without adequate resources for their survival after discharge. The health system has not been able to adapt to such change and has maintained a fragmented, reactive, disease-centered model."Selective primary care" is useful to redirect primary care bases according to the characteristics of the population. A set of specific measures, targeted at a population with specific vulnerabilities, leverages the power of primary health care. Here we describe the "Outpatient Pediatric Special Care" model of care implemented at the Center for Health and Community Action no. 5, in the Autonomous City of Buenos Aires.

Título: Serum amyloid A levels in acute and chronic urticaria

Resumo: Abstract: Background: Serum amyloid A is an acute-phase protein. There is no available data regarding serum amyloid A levels in patients with acute and chronic urticaria (CU). Objective: To investigate the association between serum amyloid A and urticaria. Methods: This was a case-control study of 81 patients who visited our Hospital between June and December 2016 with a diagnosis of urticaria. Eighty healthy controls (HC) who visited for routine health examination and physical checkups were recruited. Serum amyloid A and C-reactive protein levels were measured by automated methods. Results: Serum amyloid A and C-reactive protein levels were significantly higher in AU (Serum amyloid A: 207.1 (6.7-439.0) mg/L; C-reactive protein: 16.0 (0.2-90.0) mg/L) and CU (Serum amyloid A: 6.5 (2.5-35.8) mg/L; C-reactive protein: 1.0 (0.1-16.0) mg/L) compared with HC (Serum amyloid A: 5.04 (2.0-9.1) mg/L; C-reactive protein: 1.2 (0.1-5.6) mg/L), and in AU compared with CU (all P<0.05). There were no differences between the CU and HC group. In CU, Serum amyloid A levels in those with moderate/severe urticaria (median, 16.4 (9.7-35.8) mg/L) were higher than in those with mild urticaria (median, 5.7 (2.5-9.5) mg/L) and HC (all P<0.05). Serum amyloid A and C-reactive protein levels exceeded the normal lab range in 90.7% and 72.1% patients with AU compared with 28.9% and 13.2% patients with CU, respectively. Significant positive correlations were found between serum amyloid A and C-reactive protein (r = 0.562, P < 0.001). Study limitations: There was no comparison between active disease and remission. Conclusion: There was an association between serum amyloid A levels and urticaria. Higher serum amyloid A levels were associated with AU and more severe CU. Serum amyloid A may help to identify CU patients earlier.
