Resumo: Introduction: beeswax alcohols, consisting in a reproducible mixture of six primary fatty alcohols from 24 to 34 carbon atoms purified from Apis mellifera beeswax, has been shown to produce antioxidant and gastroprotective effects. This substance is used to manufacture Abexol® 50 mg tablets, the finished form used in clinical studies and in routine practice. Objective: to determine the stability of 50 mgâ€'Beeswax alcohols tablets (Abexol®) packed in highâ€'density polyethylene flasks. Methods: samples from three batches of Abexol® 50 mg tablets packed in white high density polyethylene flasks (Rainbow & Nature, Sydney, Australia) were put into cardboard boxes and kept under climatic conditions of the zone IV (30±2 ºC, 70±5 percent of relative humidity) for five years. Results: all parameters tested were within specifications throughout the whole study: appearance (white round tablets with intact surfaces), average weight (initial average weight±7,5 percent), total content of the six fatty alcohols (50±3.75 mg), disintegration time (<15 min), hardness (˃3 kg/cm2) and microbiological content (≤1 000 bacteria/g and ≤100 fungi/g, absence of E. coli,S. aureus, Pseudomonas, Salmonella and C. albicans). This result is consistent with the data of a previous stability study of Abexol® 50 mg tablets manufactured in Cuba and packed in blisters of polyvinyl chloride and aluminum. Conclusions: the results of the present study support that Abexol®50mg tablets packed in white high density polyethylene flasks and stored at the conditions of the climatic zone IV have a shelf life of 5 years(AU)

Introducción: los alcoholes de la cera de abejas constituyen una mezcla reproducible de seis alcoholes grasos primarios de 24 a 34 átomos de carbono purificados de la cera de Apis mellifera. Esta sustancia, con efectos antioxidantes y gastroprotectores, es empleada para la elaboración de las tabletas de Abexol® con dosis de 50 mg de alcoholes, forma farmacéutica terminada utilizada en los ensayos clínicos y en la práctica de rutina. Objetivo: determinar la estabilidad de las tabletas con 50 mg de alcoholes de cera de abejas (Abexol®) en frascos de polietileno de alta densidad. Métodos: muestras de tres lotes de tabletas de Abexol® con dosis de 50 mg envasadas en frascos de polietileno de alta densidad ( Rainbow & Nature, Sydney, Australia) se pusieron en cajas de cartón y se mantuvieron en las condiciones de la zona climática IV (30±2 ºC, 70±5 por ciento de humedad relativa) durante cinco años. Resultados: los parámetros evaluados se mantuvieron dentro de sus especificaciones de calidad durante todo el estudio: apariencia (tabletas blancas redondas con superficies enteras), peso promedio (inicial±7,5 por ciento), contenido total de los seis alcoholes grasos (50±3,75 mg), tiempo de desintegración (<15 min), dureza (˃3 kg/cm2) y contenido microbiológico (≤1 000 bacterias/g y ≤100 hongos/g, ausencia de E. coli, S. aureus, Pseudomonas, Salmonella y C. albicans). Este resultado es consistente con los datos de un estudio previo de estabilidad de tabletas de Abexol® con dosis de 50 mg, fabricadas en Cuba y envasadas en blísteres de cloruro de polivinilo y aluminio. Conclusiones: los resultados del presente estudio sustentan que las tabletas de Abexol® con dosis de 50 mg, envasadas en frascos de polietileno de alta densidad y almacenadas en las condiciones de la zona climática IV, presentan un tiempo de vida útil de cinco años(AU)

Resumo: This paper analyses some aspects of the trajectory of the Argentinian physician and sociologist Juan César García (1932-1984) in the field of Latin American Social Medicine. Three dimensions constituting his basic orientations are highlighted: the elaboration of systematic and reflective social thought; a critical attitude in questioning teaching and professional practices; a commitment to the institutionalization and dissemination of health knowledge.

O artigo analisa aspectos da trajetória de Juan César García (1932-1984), médico e sociólogo argentino, no campo da medicina social latino-americana. Destaca três dimensões que constituem as suas orientações básicas no campo da saúde: a elaboração de um pensamento sobre o social, sistemático e reflexivo; uma atitude crítica na problematização do ensino e das práticas profissionais; um compromisso com a institucionalização e divulgação do saber sanitário.

Resumo: Introducción: el D004, nuevo extracto lipídico purificado a partir de los frutos de la palma real cubana (Roystonea regia [Kunth] F. Cook), ha mostrado ser efectivo en modelos experimentales de hiperplasia prostática, y ha mostrado efectos antioxidantes y antiiflamatorios. Objetivos: identificar los alcoholes que pudieran estar presentes en el ingrediente activo D004 utilizando la cromatografía de gases acoplada a espectrometría de masa. Métodos: muestras de 9 lotes de D004 se sometieron a saponificación con una disolución de KOH/EtOH y las fracciones insaponificables fueron extraídas con n-hexano. Los alcoholes se analizaron como derivados trimetilsilil, obtenidos por reacción con N-metil, N-trimetilsilil trifluoroacetamida. La cuantificación se basó en el empleo de 1-eicosanol como patrón interno. Resultados: el D004 presenta un contenido total de alcoholes entre 0,18 y 5,10 por ciento. Esta fracción está compuesta principalmente por 1-octacosanol (52,94 por ciento), 1-triacontanol (25,91 por ciento) y 1-hexacosanol (14,40 por ciento); y presenta como componentes minoritarios: 1-docosanol, 1-tricosanol, 1-tetracosanol, 1-pentacosanol, 1-heptacosanol, 1-nonacosanol, 1-dotriacontanol, 1-tetratriacontanol, 1-tetradecanol, 1-pentadecanol, 1-hexadecanol, 1-octadecanol y 1-eicosanol, los seis últimos en niveles de trazas. Conclusiones: se identificaron y se cuantificaron mediante cromatografía de gases acoplada a espectrometría de masa los alcoholes grasos en el ingrediente activo D004. Los componentes mayoritarios fueron 1-octacosanol, 1-triacontanol y 1-hexacosanol

Introduction: D004, a new lipid extract purified from the Cuban royal palm (Roystonea regia [Kunth] F. Cook) fruits, has been shown to be effective in experimental models of prostate hyperplasia, and to exhibit antioxidant and anti-inflammatory effects. Objectives: to identify the fatty alcohols that could be present in D004 active ingredient using gas chromatography-mass spectrometry. Methods: samples of nine batches of D004 were submitted to saponification with KOH/EtOH solution and the unsaponifiable fractions were extracted with n-hexane. The alcohols were analyzed as trimetilsilil derivatives, which were obtained by reaction with MSTFA. The quantification was carried out by the internal standard method using 1-eicosanol. Results: the total content of alcohols in D004 ranged from 0,18 to 5,10 percent. This fraction was mainly composed of 1-octacosanol (52,94 percent), 1-triacontanol (25,91 percent), and 1-hexacosanol (14,40 percent); whereas the minority components were 1-docosanol, 1-tricosanol, 1-tetracosanol, 1-pentacosanol, 1-heptacosanol, 1-nonacosanol, 1-dotriacontanol 1-tetratriacontanol, 1-tetradecanol, 1-pentadecanol, 1-hexadecanol, 1-octadecanol and 1-eicosanol were minor components, being the last six alcohols present in trace levels. Conclusions: fatty alcohols were identified and quantified in D004 active ingredient by gas chromatography-mass spectrometry. The major components were 1-octacosanol, 1-triacontanol and 1-hexacosanol

Título: Protective effect of policosanol on atherosclerotic lesions in rabbits with exogenous hypercholesterolemia

Resumo: Policosanol is a mixture of higher aliphatic alcohols purified from sugar cane wax, with cholesterol-lowering effects demonstrable in experimental models and in patients with type II hypercholesterolemia. The protective effects of policosanol on atherosclerotic lesions experimentally induced by lipofundin in rabbits and rats and spontaneously developed in stumptail monkeys have been described. The present study was conducted to determine whether policosanol administered orally to rabbits with exogenous hypercholesterolemia also protects against the development of atherosclerotic lesions. Male New Zealand rabbits weighing 1.5 to 2 kg were randomly divided into three experimental groups which received 25 or 200 mg/kg policosanol (N = 7) orally for 60 days with acacia gum as vehicle or acacia gum alone (control group, N = 9). All animals received a cholesterol-rich diet (0.5 percent) during the entire period. Control animals developed marked hypercholesterolemia, macroscopic lesions and arterial intimal thickening. Intima thickness was significantly less (32.5 +/- 7 and 25.4 +/- 4 µm) in hypercholesterolemic rabbits treated with policosanol than in controls (57.6 +/- 9 µm). In most policosanol-treated animals, atherosclerotic lesions were not present, and in others, thickness of fatty streaks had less foam cell layers than in controls. We conclude that policosanol has a protective effect on the atherosclerotic lesions occurring in this experimental model.

Resumo: A Síndrome de Sjögren-Larsson é uma doença autossômica recessiva rara com distribuiçäo universal. Consiste em ictiose, displegia espástica e retardo mental causado por um defeito enzimático na oxidaçäo do álcool-graxo. Nós relatamos dois casos e fazemos uma revisäo da literatura respectiva. As duas crianças tinham atividade deficiente da NAD oxidorredutase. Foram estudados os lípides de membrana das células plasmáticas e eritrócitos. Bons resultados foram obtidos em um dos pacientes quando submetido a dieta na infância precoce o que se correlacionou com diminuiçäo do álcool-graxo no plasma. Entretanto näo obtivemos melhora clínica no outro paciente cujo tratamento teve início tardio.Terapia com etretinato foi necessária para controlar os sintomas cutâneos neste segundo paciente.

Resumo: Policosanol is a mixture of higher aliphatic primary alcohols isolated from sugar cane wax, whose main component is octacosanol. An inhibitory effect of policosanol on platelet aggregation and cerebral ischemia in animal models has been reported. Thus, the objective of the present study was to evaluate the effect of policosanol on cerebral ischemia induced by unilateral carotid ligation and bilateral clamping and recirculation in Mongolian gerbils. Policosanol (200 mg/kg) administered immediately after unilateral carotid ligation and at 12- or 24-h intervals for 48 h significantly inhibited mortality and clinical symptoms when compared with controls, whereas lower doses (100 mg/kg) were not effective. Control animals showed swelling (tissue vacuolization) and necrosis of neurons in all areas of the brain studied (frontal cortex, hippocampus, striatum and olfactory tubercle), showing a similar injury profile. In the group treated with 200 mg/kg policosanol swelling and necrosis were significantly reduced when compared with the control group. In another experimental model, comparison between groups showed that the brain water content of control gerbils (N = 15) was significantly higher after 15 min of clamping and 4 h of recirculation than in sham-operated animals (N = 13), whereas policosanol (200 mg/kg) (N = 19) significantly reduced the edema compared with the control group, with a cerebral water content identical to that of the sham-operated animals. cAMP levels in the brain of control-ligated Mongolian gerbils (N = 8) were significantly lower than those of sham-operated animals (N = 10). The policosanol-treated group (N = 10) showed significantly higher cAMP levels (2.68 pmol/g of tissue) than the positive control (1.91 pmol/g of tissue) and similar to those of non-ligated gerbils (2.97 pmol/g of tissue). In conclusion, our results show an anti-ischemic effect of policosanol administered after induction of cerebral ischemia, in two different experimental models in Mongolian gerbils, suggesting a possible therapeutic effect in cerebral vascular disorders

Resumo: The effect of D002, a defined mixture of higher primary alcohols purified from bee wax, on in vivo and in vitro lipid peroxidation was studied. The extent of lipid peroxidation was measured on the basis of the levels of thiobarbituric acid reactive substances (TBARS). When D002 (5-100 mg/kg body weight) was administered orally to rats for two weeks, a partial inhibition of the in vitro enzymatic and non-enzymatic lipid peroxidation was observed in liver and brain microsomes. Maximal protection (46 percent) occurred at a dose of 25 mg/kg. D002 behaved differently depending on both the presence of NADPH and the integrity of liver microsomes, which suggests that under conditions where microsomal metabolism was favored the protective effect of D002 was increased. D002 (25 mg/kg) also completely inhibited carbon tetrachloride- and toluene-induced in vivo lipid peroxidation in liver and brain. Also, D002 significantly lowered in a dose-dependent manner the basal level of TBARS in liver (19-40 percent) and brain (28-44 percent) microsomes. We conclude that the oral administration of D002 (5, 25 and 100 mg/kg) for two weeks protected rat liver and brain microsomes against microsomal lipid peroxidation in vitro and in vivo. Thus, D002 could be useful as a dietary natural antioxidant supplement. More studies are required before these data can be extrapolated to the recommendation for the use of D002 as a dietary antioxidant supplement for humans

Resumo: Policosanol is a natural mixture of higher primary aliphatic alcohols isolated and purified from sugar cane (Saccharum officinarum, L.) wax, whose main component is octacosanol. Policosanol (5-200 mg/kg) orally administered for 4 weeks to normocholesterolemic New Zealand rabbits significantly reduced total cholesterol and low density lipoprotein cholesterol (LDL-C) serum levels in a dose dependent manner. Serum triglyceride levels of treated and control animals were significantly different, but the reduction observed was not dose-dependent. High density lipoprotein cholesterol (HDL-C) levels remained unchanged. Results indicate that the reduction in total cholesterol values induced by policosanol is mainly mediated through a decrease in LDL-C levels
