Título: Nickel, chromium and cobalt: the relevant allergens in allergic contact dermatitis. Comparative study between two periods: 1995-2002 and 2003-2015

Resumo: Abstract: Background: Metals are common agents of allergic contact dermatitis, occupational or not, with decreasing incidence over the last years in some countries that have regulated the amount of nickel in objects. Objectives: To analyze and compare with previous studies the profile of metal sensitization between 2003-2015. Methods: Patients who underwent patch testing between 2003-2015 were evaluated retrospectively regarding the sensitization rates to metals, the associations between them, the relationship with profession and epidemiology. Results: Of the 1,386 patients tested, 438 (32%) had positive test to some metal, similar results to the 404/1,208 (33%) of the previous study (1995-2002) performed at the same service (p=0.32). The frequency of nickel (77%), cobalt (32%) and chromium (29%) changed slightly (p=0.20). Most cases of sensitization to chromium were related to the occupation (64%), in contrast to nickel and cobalt (p<0.0001). There was a predominance of females among those sensitized to metal in both studies (p=0.63) and the age group of 20-49 years old (p=0.11); the number of fair-skinned individuals increased (p<0.001), as well as the lesions in the cephalic segment (50.5%; p<0.0001) and hands (45%; p<0.0001), which are not the most frequent location anymore. The number of cleaners decreased (39% vs. 59%; p<0.0001), which still lead in front of bricklayers/painters, which increased (14% vs. 9%; p=0.013). The frequency of wet work reduced (65% vs. 81%; p<0.0001). Study limitations: The study included a single population group; only patients with positive tests to metals were considered - the others were not evaluated for the possibility of false negatives. Conclusion: The sensitization to metals, occupational or not, has been significant over the last 21 years, with few epidemiological changes.

Resumo: Abstract: Children's products are considered safe by the general population and doctors. Labels with terms such as "hypoallergenic" or "dermatologically recommended and tested" denote trust and credibility with the idea that they can be used by any individual. Patients with allergic contact dermatitis may be sensitive to allergens present in any product, including children's. There is insufficient knowledge about allergens in these products in our country. We evaluated 254 children's products, and at least one allergen was present in 236 (93%) of them. The indication of a topical product should be careful and based on contact tests.

Título: Allergic contact dermatitis to shoes: contribution of a specific series to the diagnosis

Resumo: Abstract: Background: In our country, the Brazilian Standard Series is the most used for the etiological diagnosis of allergic contact dermatitis to shoes. However, there is no assessment of the usefulness of specific allergens for shoes. Objectives: To measure the improvement in diagnostic accuracy of allergic contact dermatitis to shoes with the use of a specific complementary series in patch testing and describe the characteristics of the affected population, such as gender, location of lesions, time of evolution, and the most common allergens. Methods: This retrospective study evaluated the results of 52 patients with suspected shoe dermatitis subjected to patch tests with the standard and specific series to quantify the gain in diagnostic accuracy. Results: Among the 52 suspected cases, 29 cases (56%) were confirmed. In 13 (45%) cases the diagnosis was determined through the specific series, which results in an 81% increase in the number of diagnoses. Study limitation: Small sample size. Conclusions: Women were more commonly affected, with a mean time for the final diagnosis of 45 months, and the most common localization was the dorsum of the feet. There was an increase in diagnostic accuracy with the introduction of new haptens in the patch test of patients with suspected shoes dermatitis.

Resumo: La implicación de los hongos en las reacciones alérgicas se conoce desde hace mucho tiempo. Más de 80 géneros de hongos se han asociado con síntomas de alergias del tracto respiratorio. Se resume la clasificación taxonómica de los géneros de hongos que más se han relacionado con enfermedades alérgicas respiratorias y el mecanismo fisiopatológico de daño al huésped causado por hongos en asma, rinitis, sinusitis, micosis pulmonares y pneumonitis por hipersensibilidad, y se presentan las bases de la nomenclatura de los determinantes alergénicos fúngicos según su caracterización bioquímica. El uso de alérgenos recombinantes de hongos permite aclarar el perfil alergénico que presentan algunos pacientes polisensibilizados y reconocer la sensibilización a alérgenos potencialmente peligrosos, como es el caso de alérgenos alimentarios, e identificar la presencia de éstos en diversas fuentes; además, sirven para estudiar la reactividad cruzada entre alérgenos de la misma especie o de otras especies, y con éstos podría superarse la limitación de los extractos fúngicos convencionales los cuales son muy variables en cuanto a potencia y contenido alergénico. Se comentan la complejidad y las indicaciones de la inmunoterapia específica para desensibilización de pacientes hiperreactivos a hongos alergénicos. (AU)

The involvement of fungi in allergic reactions has been known for a long time. More than 80 genera of fungi have been associated with symptoms of respiratory allergies. This review summarize the taxonomic classification of genera of fungi that have been linked to the most prevalent allergic diseases and also commented the pathophysiological mechanisms of damage to the host caused by fungi in asthma, rhinitis sinusitis and pulmonary fungal hypersensitivity pneumonitis and presents the bases of nomenclature of fungal allergenic determinants according to their biochemical characterization. The use of recombinant allergens helps clarify the profile fungal allergen present in some patients polysensitized and recognize their presence in various sources. This also serves well to study cross-reactivity between allergens of the same species or other species. With the use of recombinant allergens could overcome the limitation of the conventional fungal extracts which are very variable in terms of potency and allergen content. We comment of the complexity and indications of specific immunotherapy for desensitization of hyperreactive patients to allergenic fungi. (AU)

Resumo: Abstract: BACKGROUND: Patients with oral sensitivity are common in our practice. Allergic contact dermatitis is one of the most frequent etiologies. OBJECTIVES: Evaluate oral contact dermatitis using the Brazilian standard series and complementary dental series in patients using dental prostheses, with or without oral complaints. Determine specific dental Brazilian series. METHODS: Patients using dental prostheses with or without oral complaints realized patch tests. Brazilian standard series and complementary dental series were used according to ICDRG recommendations. The results were analysed according to age, sex, race, atopic conditions and symptoms associated. RESULTS: From 54 patients tested, 34 (63%) were positive at least to one substance. Nineteen had oral complaints, such as burning mouth, itch or oral erythema. There was no association between atopic condition and tests results. Without the oral series, just 23(42,6%) patients had a positive result. Using the Brazilian standard series with the complementary dental series we improved the positivity of the patch test to 47%. CONCLUSION: In patients using prostheses and with oral complaints, patch tests with Brazilian standard series with complementary dental series improve the tests positivity.

Título: Incidence of allergic contact sensitization in central Chinese subjects with chronic urticaria

Resumo: Abstract BACKGROUND: Chronic urticaria (CU) can be provoked by a wide variety of causes. Some studies suggest contact sensitization may play a role in the disease. OBJECTIVE: To investigate the incidence and distribution characteristics of allergic contact sensitization in central Chinese subjects with CU, and assess contact allergen avoidance measures in managing CU. METHODS: Patch tests were performed, following the recommended standard procedure, with 20 selected allergens, in line with the European baseline series. All subjects with positive results were prescribed appropriate avoidance measures for the sensitizing substances, while subjects with negative results served as the control group. CU severity was assessed daily from week1 to week4 and for each subject, applying the Urticaria Activity Score. RESULTS: 42.9% (233/543) of subjects with CU showed positive reactions to one or more contact allergen(s). Potassium dichromate, benzene mix and carba mix were more common in male patients, while nickel sulfate was more frequent in females. The positive rates for different allergens varied with age and occupation. The median (interquartile range) severity scores at week 1 were 20 (14-21) and 15 (14-27) for the allergen avoidance group and control group, respectively (P>0.05); and 12 (7-15) and 14 (12-17) at week 4 (P<0.001). CONCLUSION: The incidence of allergic contact sensitization in CU patients was high, and appropriate contact allergen avoidance measures benefitted CU management. Contact allergens may play a role in the pathogenic mechanism of CU and patch tests are an option for CU patients.

Resumo: The presence of milk, egg and soy proteins was evaluated in thirteen commercial products, which were premixes to make cream caramel, cake, soy "milanesas", "tortas fritas", pizza, gnocchi and complementary foods. Samples were analysed using SDS-PAGE, immunoblotting and ELISA methods. The electrophoretic method and immunoblotting were useful to confirm the presence of milk, egg and soy proteins in four samples that declared them as ingredients. In other samples, both methods showed negative results for some of these proteins, although they were also declared as ingredients. This suggests that those proteins were not added as ingredients in these products. The ELISA kits detected very low concentration of the allergenic proteins in four products with precautionary phrases and also in five samples that made no reference to them in their labels. ELISA methods are useful to detect cross contamination due to their high sensitivity. The food industry should be responsible for the declaration of milk, egg and soy proteins in their food labels.

Se evaluó la detección de proteínas de leche, huevo y soja en trece productos comerciales, correspondientes a premezclas para preparar flanes, bizcochuelo, milanesas de soja, tortas fritas, pizza, ñoquis y productos en polvo a base de harinas para niños pequeños. Las muestras fueron analizadas por SDS-PAGE, inmunoblotting y métodos de ELISA. El método electroforético e inmunoblotting resultaron útiles para confirmar la presencia de las proteínas alergénicas en algunas muestras que las declaraban como ingredientes. En cuatro muestras que también declaraban como ingredientes alguna de estas proteínas, tanto con SDS-PAGE como con Inmunoblotting, los resultados fueron negativos, sugiriendo que dichas proteínas no fueron agregadas como ingredientes en dichos alimentos. Los kits de ELISA permitieron la detección de concentraciones muy bajas de estos alérgenos en cuatro productos que presentaban frases de advertencia e incluso en otros cinco que no tenían ninguna declaración de alérgenos. Los métodos de ELISA son útiles para detectar contaminaciones cruzadas, dada su elevada sensibilidad. Se concluye que resulta necesario una declaración responsable de estas proteínas alergénicas, en los rótulos de este tipo de alimentos por parte de los fabricantes.

Resumo: Dermatophagoides farinae (Der f), one of the main species of house dust mites, produces more than 30 allergens. A recently identified allergen belonging to the alpha-tubulin protein family, Der f 33, has not been characterized in detail. In this study, we used bioinformatics tools to construct the secondary and tertiary structures and predict the B and T cell epitopes of Der f 33. First, protein attribution, protein patterns, and physicochemical properties were predicted. Then, a reasonable tertiary structure was constructed by homology modeling. In addition, six B cell epitopes (amino acid positions 34-45, 63-67, 103-108, 224-230, 308-316, and 365-377) and four T cell epitopes (positions 178-186, 241-249, 335-343, and 402-410) were predicted. These results established a theoretical basis for further studies and eventual epitope-based vaccine design against Der f 33.

Resumo: Se evaluó la detección de trazas de soja en sistemas modelo (SM) de galletitas y extrudidos que contenían 5000, 250, 100, 50, 25, 10, 5 y 0 ppm de concentrado de proteína de soja (CPS). Se analizaron, además, dos lotes diferentes de ocho clases de galletitas comerciales y dos tipos de extrudidos que declaraban la presencia de soja a través de la frase "Contiene ..." o a través de frases de advertencia. La determinación de proteínas de soja se realizó utilizando tres kits ELISA (R-Biopharm, Neogen-Veratox y Romer). Solo el kit de R-Biopharm permitió la cuantificación de soja a partir de 25 ppm de CPS en galletitas y a partir de 5 ppm de CPS en productos extrudidos. Los otros dos kits resultaron menos sensibles. De acuerdo con los resultados obtenidos en el análisis de los SM de galletitas y de productos extrudidos con diferentes kits comerciales se concluye que debe estudiarse el desempeño de cada kit para cada alergeno y para cada matriz en particular. Esto permitiría establecer cuál es el método más adecuado para el control de los productos comerciales a los fines de evaluar la correcta declaración de alergenos en los respectivos rótulos.

Detection of traces of soy was evaluated in cookies and extruded product model systems (MS) containing 5000, 250, 100, 50, 25, 10, 5 and 0 ppm of soy protein concentrate (SPC). Besides, two different batches of eight different types of commercial cookies and two types of extruded products declaring soy presence through either the statement: "Contains…" or precautionary phrases were analyzed. Soy protein determination was carried out using three different commercial ELISA kits (R-Biopharm, Neogen-Veratox and Romer). Only the R-Biopharm kit allowed quantification of soy from 25 ppm of SPC in cookies and from 5 ppm of SPC in the extruded products. The other two kits had lower sensitivity. According to the results obtained in the analysis of cookies and extruded product MS with different commercial kits, it can be concluded that the performance of every kit should be studied for all allergens and each particular food matrix. This would enable establishing the most suitable method for commercial products control, in order to assess the correct allergen labeling.

Analisaram-se, também dois lotes diferentes de oito tipos de biscoitos comerciais e dois tipos de extrusados que declaravam a presença de soja através da frase "Contém.... ou através de frases de advertência. A determinação de proteínas de soja foi realizada utilizando três kits ELISA (R-Biopharm, Neogen-Veratox e Romer). Apenas o kit de R-Biopharm permitiu a quantificação de soja a partir de 25 ppm de CPS em biscoitos e de a partir de 5 ppm de CPS em produtos extrusados. Os outros dois kits resultaram menos sensíveis. De acordo com os resultados obtidos na análise dos SM de biscoitos e de produtos extrusados com diferentes kits comerciais conclui-se que é preciso estudar o desempenho de cada kit para cada alergeno e para cada matriz em particular. Isto permitiria estabelecer qual é o método mais adequado para o controle dos produtos comerciais viando avaliar a correta declaração de alergenos nas respectivas etiquetas.

Resumo: Background: Prosopis, or mesquite (Prosopis juliflora (Sw.) DC.), was introduced in Saudi Arabia several decades ago and is heavily used in street, roadside, and park plantations. It shows great adaptation to the prevailing climatic conditions such as high temperature, severe drought, and salinity and spreads naturally in many parts of the Kingdom. This research was conducted to isolate allergen proteins and biogenic amines from the pollen grains of P. juliflora genotypes in Saudi Arabia from two regions, namely Al-Qassim and Eastern regions. Results: The results showed that 18 different allergen proteins were detected in P. juliflora genotypes, with molecular weight ranging from 14 to 97 kDa. Moreover, P. juliflora genotypes from the two studied regions contained eight biogenic amines, namely histamine, tyramine, tryptamine, ß-phenylethylamine, butricine, codapherine, spermidine, and spermine. All genotypes from the Al-Qassim region were found to contain all eight amines, while in the Eastern region, histamine was absent in three genotypes, spermine was absent in six genotypes, and spermidine was absent in three genotypes. Genotypes B23, E20, and E21 had the lowest biogenic amine quantity. Conclusions: All identified proteins from mesquite trees from both regions (Eastern and Al-Qassim) cause allergies in patients who are sensitive to pollen grains. Bioamines, except histamine and tyramine, were recorded at varying concentrations in different genotypes.
