Título: Evaluation of Canine Transpositions and Related Dental Anomalies in a Turkish Pediatric Population

Resumo: Abstract Objective: To investigate the prevalence and characteristics of canine transpositions and to evaluate associated dental anomalies in a pediatric population. Material and Methods: This study was carried out by evaluating the panoramic radiographs of 11.750 patients ranging in ages from 6 to 17 presented to a pedodontic clinic over 5 years. The patients with canine transposition were included in the study. Age at diagnosis, gender, classification and location of transposition, presence of other dental anomalies were recorded. Differences were analyzed using the Chi-squared test. Results: Canine transposition was detected in 73 teeth from 69 patients (0.6%). Transpositions were more common in girls than in boys (55.1%). Transposed canine teeth were more frequently observed unilaterally (89%). The most commonly found transposition type was type 2 (42%). A total of 49.3% of 69 patients had retained primary canine teeth, 20.3% had congenitally missing teeth, and 11.6% had peg-shaped, microdont lateral incisors. Impacted teeth excluding third molars and taurodontism were also observed. Conclusion: Although the canine transpositions are generally asymptomatic in children in the period of growth and development, the importance of early diagnosis is crucial to prevent various clinical problems.

Resumo: Abstract Objective: To analyze the prevalence and factors associated with malocclusions in preschool children. Material and Methods: A census, epidemiological study, with a cross-sectional, descriptive and analytic sample, was carried out with children aged between 4 and 6 years, matriculated in preschools in the town of Aiquara, Bahia, Brazil. Malocclusion in the deciduous dentition was determined through the application of the index proposed by WHO, with the Foster and Hamilton criteria also being incorporated. The data was tabulated in duplicate in a Microsoft Excel spreadsheet and following correction, was analyzed using the SPSS, STATA and PAST Software. In order to identify the associated factors, we opted for Multiple Poisson regression analysis and Principal Component Analysis (PCA). Results: A total of 148 children were examined and the general prevalence of malocclusions was 69.59%, with 57.5% in the group of 4-year-old, 76.92% those aged 5 years old and 72.46% among children aged 6 years old. In terms of primary canines, 66.2% were identified as class I, 25.7% as class II and 8.1% as class III. Normal overjet was 41.2%, increased overjet 34.5%, edge to edge bite 17.6% and anterior crossbite 6.8%. The normal overbite 51.4%, reduced 20.9% and deep 8.1%. Of the individuals, 20.9% had posterior crossbite. In the Poisson regression analysis and PCA, a statistical association between the malocclusions and dental caries, duration of use of pacifiers, onychophagia and thumb sucking, was identified. Conclusion: The prevalence of malocclusion in preschool children in the small town of Aiquara is high and the associated factors identified are capable of control and prevention.

Título: Radiographic Characteristics of Soft Tissue Calcification on Digital Panoramic Images

Resumo: Abstract Objective: To assess the prevalence of soft tissue calcifications and their panoramic radiographic characteristics. Material and Methods: This descriptive retrospective study evaluated 2027 panoramic radiographs. The type and location of calcifications and the age and gender of patients were evaluated by two radiologists. Data were analyzed via SPSS and the Chi-square, Fisher's exact and Kappa tests were used to compare the categorical demographic variables among the groups. The confidence interval was set to 95% and p<0.05 was considered statistically significant. Results: The prevalence of calcified stylohyoid ligament was 11.24%. This value was 3.99% for tonsillolith, 1.33% for calcified carotid plaque, 0.69% for antrolith, 0.39% for calcified lymph node, 0.29% for phleboliths, and 0.19% for sialoliths. The prevalence of these conditions had no significant association with gender or age (p=0.102). The prevalence of bilateral calcified stylohyoid ligament, tonsillolith, and a calcified carotid plaque was significantly higher (p<0.001). The most prevalent type of calcified stylohyoid ligament, according to O'Carroll's classification, belonged to types 1, 4, 3 and 2 (p<0.001). The most commonly observed radiographic pattern was multiple, well-defined tonsilloliths (75.3%, p<0.001). Conclusion: The prevalence of soft tissue calcifications on panoramic radiographs was relatively low in this Iranian population. The most calcifications were respectively calcified stylohyoid ligament, tonsillolith, calcified carotid plaque, antrolith, calcified lymph node, phleboliths and sialoliths. Calcified stylohyoid ligament, tonsillolith and calcified carotid plaque were more bilaterally. Thereby panoramic imaging can help in primary assessment, epidemiologic and screening evaluation of these calcifications.

Título: Combination of Ledipasvir and Sofosbuvir for Treatment of Hepatitis C Virus Genotype 1 Infection: Systematic Review and Meta-Analysis

Resumo: ABSTRACT Background and aim. The combination of Sofosbuvir (SOF) and Ledipasvir (LDV) has been lead to considerable enhancement of treatment of hepatitis C virus (HCV) genotype 1 infection. A meta-analysis of the currently available studies was undertaken with the aim to evaluate the antiviral efficacy of SOF/LDV therapy for 12 or 24 weeks with or without Ribavirin (RBV) in patients with HCV genotype 1 infection. Material and methods. In this meta-analysis, we searched databases including PubMed, Scopus, Science Direct and Web of Science using appropriate keywords. All papers which evaluated the efficacy of combination therapy of SOF/LDV with or without RBV for 12 or 24 weeks among patients with HCV genotype 1 infection were included. Results. The 20 published articles were assessed for eligibility and finally 10 articles pooling 2248 participants were included in this meta-analysis. Pooled SVR12 for four SOF/LDV regimens were 95% (95%CI = 93%-97%) for 12 weeks of treatment with SOF/LDV, 97% (95%CI = 95%-98%) for 24 weeks of treatment with SOF/LDV, 96% (95%CI = 94%-97%) for 12 weeks of treatment with SOF/ LDV/RBV and 98% (95%CI = 97%-99%) for 24 weeks of treatment with SOF/LDV/RBV. Only in treatment regimen of SOF/LDV for 12 weeks, cirrhosis had a significant effect on the SVR12 (OR = 0.21, 95%CI = 0.07-0.66). Furthermore, NS5A resistance-associated substitutions at baseline were associated with decrease in the rate of SVR (OR = 0.31, 95%CI = 0.2-0.5). Conclusions. The Interferon-free regimen of SOF/LDV for 12 or 24 weeks with or without RBV is highly effective for treatment of patients with HCV genotype 1 infection.

Título: Interaction of Gender and Hepatitis C in Risk of Chronic Renal Failure After Liver Transplantation

Resumo: ABSTRACT Background. Chronic renal failure (CRF) is a significant cause of morbidity and mortality in post-liver transplantation (LT) recipients. The risk factors associated with the development of renal dysfunction are not clearly elucidated. Objectives. To examine the risk factors in the development of CRF in these patients. Material and methods. Retrospective case-cohort of liver transplant patients without baseline kidney dysfunction who developed chronic renal failure during their follow-up. Results. Of 370 patients, 254 met the inclusion criteria. 30% (76) of these patients had CRF of which 57% (43) were male. Age, estimated glomerular filtration rate (eGFR) at discharge, and HCV infection were found to be risk factors for CRF post-LT. The odds ratio of developing CRF was 1.4 (0.6-3.3) in males with HCV, 1.6 (0.7-3.9) in females without HCV and 4.4 (1.5-13.2) among females with HCV when compared to men without HCV. Conclusions. In this cohort of LT receipients of a major Canadian city, age, eGFR, and HCV infection were risk factors for CRF. Female gender and HCV increased this odds by a factor of more than 4.

Título: Clinical Impact of Portal Vein Thrombosis Prior to Liver Transplantation: A Retrospective Cohort Study

Resumo: ABSTRACT Introduction. To identify the impact of portal vein thrombosis (PVT) and associated medical and surgical factors on outcomes post liver transplant (LT). Material and methods. Two analyses were performed. Analysis One: cohort study of 505 consecutive patients who underwent LT (Alberta) between 01/2002-12/2012. PVT was identified in 61 (14%) patients. Analysis Two: cohort study of 144 consecutive PVT patients from two sites (Alberta and London) during the same period. Cox multivariable survival analysis was used to identify independent associations with post-LT mortality. Results. In Analysis One (Alberta), PVT was not associated with post-LT mortality (log rank p = 0.99). On adjusted analysis, complete/occlusive PVT was associated with increased mortality (Hazard Ratio (HR) 8.4, p < 0.001). In Analysis Two (Alberta and London), complete/occlusive PVT was associated with increased mortality only on unadjusted analysis (HR 3.7, p = 0.02). On adjusted analysis, Hepatitis C (HR 2.1, p = 0.03) and post-LT portal vein re-occlusion (HR 3.2, p = 0.01) were independently associated with increased mortality. Conclusion: Well-selected LT patients who had PVT prior to LT had similar post-LT outcomes to non-PVT LT recipients. Subgroups of PVT patients who did worse post-LT (complete/occlusive thrombosis pre-LT, Hepatitis C or post-LT portal vein re-occlusion) warrant closer evaluation in listing and management post-LT.

Título: Análisis de la cumplimentación de certificados médicos por estudiantes y profesionales de odontología / Analysis of filling out dental certificates by dental students and professionals

Resumo: Resumen: Este estudio tuvo el objetivo de analizar y comparar la cumplimentación de certificados médicos por estudiantes del cuarto año del curso de Odontología de una universidad pública brasileña y por profesionales de cursos de especialización específicos del área. Entre 2014 y 2016, se dejó a los encuestados una hoja de papel en blanco en cuyo encabezamiento sólo constaba la palabra "Certificado", a fin de que se lo elaboraran de la manera que juzgaran más adecuada. Se utilizó test para comparación de dos proporciones y Test de Chi- Cuadrado con nivel de significación del 5%. Participaron de la inves tigación 336 individuos, de los cuales 167 (49,7%) eran estudiantes y 169 (50,3%), profesionales de cursos de especialización de la Facultad de Odontología de Araçatuba. Solamente 73 (21,7%) individuos ela boraron el certificado correctamente. Entre los errores de los estu diantes se destaca la inserción de la Clasificación Estadística Interna cional de Enfermedades sin el consentimiento del paciente, y, de los profesionales, la falta del Documento Nacional de Identidad. Hubo diferencia estadística significativa entre las categorías respecto a la correcta cumplimentación del certificado (p=0). Se concluyó que una parte considerable de los encuestados rellena los certificados odonto lógicos incorrectamente, con menores errores entre los estudiantes de Odontología.

Abstract: The objective of this study was to analyze and compare the filling of the dental certificates by the students of the fourth year of the den tistry course of a Brazilian public university and professionals of specialization courses in the area. The respondents were given a blank sheet written "Certificate", for them to fill out. The period analyzed was from 2014 to 2016. The test was used to compare two proportions and the chi-square test, with a significance level of 5%. A total of 336 individuals participated in the study, of which 167 (49.7%) were fourth year students of the Dentistry course and 169 (50.3%) professionals of specialization courses of the School of Dentistry of Araçatuba. Only 73 (21.7%) correctly filled the certificate. Among the errors, we emphasize the students' insertion of the International Statistical Clas sification of Diseases without the patient's consent and among profes sionals the lack of the General Registry. There was a significant statis tical difference between the categories regarding the correct filling of the certificate (p = 0). We concluded that the dental certificates are filled incorrectly by a considerable portion of the respondents, with minor errors among dentistry students.

Resumo: Resumen: En este trabajo se realizó un análisis psicométrico de la Escala de Satisfacción Familiar de Olson y Wilson en una muestra de 274 trabajadores de la ciudad de Arequipa, con la finalidad de valorar su estructura factorial y ofrecer un instrumento válido y confiable para evaluar la funcionalidad familiar en contextos laborales. Los resultados indican que la dimensión de Cohesión cuenta con un índice de confiabilidad de a = 0,858 y la dimensión de Adaptabilidad de a = 0,848. Asimismo el análisis factorial confirmatorio revela indicadores de bondad de ajuste adecuados, que confirman la estructura bifactorial de la prueba. También se ofrecen baremos con percentiles en tres niveles (bajo, moderado y alto) de la funcionali dad familiar, que permitirán valorar el sistema familiar como factor de riesgo psicosocial en el trabajo.

Abstract: In this work we made a psychometric analysis about Olson and Wilson's Family Satisfaction Scale in a sample of 274 workers from Arequipa City, in order to value its factor structure and offer a valid and reliable instrument to test the family functionability in organi zational contexts. The results indicate that the dimension of Cohesion has an index of reliability of a= 0.858, and the dimension of Adaptability has a reliability index of a= 0.848. Moreover, the confirmatory factor analysis reveal indicators of adequate good fit ness index, which confirm the bifactorial structure of the test. Finally, we show percentiles in three levels, about the family functionability, so they could allow the evaluation of family as a factor of psychosocial risk at work.

Título: Self-Perceived Preparedness to Work Independently Among Graduating Dental Students in Saudi Arabia: Traditional Versus Integrated Curriculum

Resumo: Abstract Objective: To investigate the perceived preparedness of graduating dental students to work independently and to compare these perceptions between Traditional and Integrated curriculums of dentistry in Saudi Arabia. Material and Methods: A pretested self-administered questionnaire was filled by 258 dental students from four dental institutions across the country. The questionnaire covered 10 domains consisting of sixty suitable questions. Students were asked to mark their responses at one of five levels of performance for each of the selected skills ranging from 0 (not yet learned) to 4 (can do it confidently). Mean scores for each subscale of the Questionnaire were calculated by individual items and for all comparison, p≤0.05 was considered statistically significant. Results: 253 graduating students were included in the final analysis, out of which 108 belongs to the Integrated curriculum group and 145 from Traditional curriculum. Overall scores of Traditional curriculum students (176 ± 4.8) were more compared to Integrated curriculum students (161 ± 3.2), which are statistically significant. Integrated curriculum students had more self-perceived skill scores in Orthodontics, Conservative/Operative Dentistry & Endodontics, Dental Public Health and Drug & Emergency management group of clinical skills. Overall, perceived preparedness to practice dentistry for all participants is 60.5% and traditional curriculum students expressed more preparedness (68%) than Integrated curriculum (53%). Conclusion: Overall, preparedness to practice independently is unsatisfactory compared to other countries. The Ministry of Education (KSA) should take necessary actions to bridge the gap between training dentists and to be competent practitioners.

Título: Prevalence of Dental Fluorosis in a City without Fluoridation in its Water Supply: Effect of Sampling

Resumo: Abstract Objective: To determine the prevalence of dental fluorosis in a population not exposed to fluoridated water and to evaluate the effect of the sampling technique on this measurement. Material and Methods: This observational, cross-sectional and quantitative study was conducted in the city of Mariana, Brazil. The participants were selected based on age, so that they had access to fluoridated dentifrice during the formation of the permanent upper teeth. Based on a nominal survey of schoolchildren aged 14 to 19, a probabilistic sample of 304 such children was clinically examined. Dental fluorosis was measured by the Thylstrup-Fejerskov index. The prevalence of fluorosis identified in a previous study, conducted in 2011 in the same city, using the same methodology but employing convenience sampling, was used as a control to compare the effect of the two sampling techniques. Results: The prevalence of dental fluorosis was 3% based on the probabilistic sampling of the subjects. In the previous study, the prevalence of fluorosis was 7% (p<0.05) based on convenience sampling of the same age group. Both studies detected a Thylstrup-Fejerskov fluorosis index value of 1, corresponding to only a mild degree of dental fluorosis. Conclusion: The convenience sampling technique seems to have affected the prevalence of dental fluorosis identified, with a 4% difference between the two studies. The prevalence of dental fluorosis attributed to the use of fluoridated dentifrice in a population not exposed to fluoridation of the water supply was considered low.
