Resumo: El núcleo supraquiasmático (NSQ) es el principal reloj biológico de los mamíferos y sincroniza la actividad de la glándula pineal al ciclo luz-oscuridad a través de una vía polisináptica. El efecto de asa de retroalimentación neuroendocrina se lleva a cabo por la melatonina. El presente trabajo pretende demostrar que la glándula pineal modula la sensibilidad a la luz en el NSQ. Se utilizaron ratas Wistar, y se asignaron a 3 grupos: grupo A (falsa pinealectomía -sham-, sin luz), grupo B (falsa pinealectomía -sham- + luz) y grupo C al cual se le realizó la pinealectomía + luz, después de la manipulación se sacrifican para realizar inmunohistoquímica para c-Fos y al final conteo celular por técnica de estereología. Se obtuvo una reducción del 46,8% del promedio de células inmunorreactivas a c-Fos en el grupo C en comparación del grupo B. Este trabajo muestra que la sensibilidad a la luz está modulada por la actividad de la glándula pineal.

The suprachiasmatic nucleus (SCN) is the main and major biological clock in mammals and is responsible for the synchronization of the pineal gland to the light/darkness cycle through a polysynaptic pathway. The neuroendocrine feedback loop effect is carried out by melatonin. This study was carried out to demonstrate that the pineal gland adjusts the sensibility to light in the suprachiasmatic nucleus. Wistar rats were allocated in 3 groups: Group A (sham pinalectomy, without light), group B (sham pinealectomy + light) and group C which underwent real pinalectomy + light. After the intervention the animals were slain to perform immunohistochemistry for c-Fos and cell counting by stereology technique. A 46.8% average reduction in c-Fos immunoreactive cells was achieved in-group C as compared with group B. The present work shows that sensibility to the light is modulate by the activity of the pineal gland.

Resumo: Los marcapasos cardíacos han sido parte del arsenal terapéutico para el manejo de una variedad de bradiarritmias durante muchas décadas y con la evolución de los generadores de estimulación se ha reducido su tamaño, incrementado la longevidad, así como su complejidad. Los más recientes ofrecen diagnósticos del ritmo y estado de la enfermedad, con varios algoritmos diseñados para ofrecer un tiempo de entrega de la estimulación de impulsos y aun los más recientes presentan diagnósticos indicativos de la fisiopatología. El uso de la estimulación ha aumentado en forma significativa de 46,7/100.000 en 1993 a 61,6/100.000 en 2009, con el porcentaje de dispositivos de cámara doble, pasando del 62% al 82% durante este período, con alrededor de 20.000 reemplazos al año en los Estados Unidos. El trazado en el electrocardiograma varía de acuerdo con el tipo de dispositivo, número y colocación de electrodos, así como su configuración. Es importante que el médico se encuentre familiarizado con estos hallazgos, así como las principales causas del mal funcionamiento, por ello se presenta esta revisión donde se muestran las características principales de función de los marcapasos, así como su normal interpretación en el electrocardiograma y las principales causas de mal funcionamiento...(AU)

Cardiac [artificial] pacemakers have been part of the therapeutic arsenal for the treatment of a variety of bradyarrhythmias during many decades. With the development of pulse generators, pacemakers are now smaller in size and have increased device longevity and complexity. The latest pacemakers have the ability to detect rhythm and provide cardiovascular disease states. Several algorithms were designed to offer a time to deliver a stimulus pulse. Even more modern pacemakers are capable to provide diagnoses which indicate cardiovascular physiopathology. The use of stimulation has increased significantly from 46.7/100.000 in 1993 to 61.6/100.000 in 2009, showed by the dual chamber device use rate which has increased from 62 to 82% during this period, with around 20.000 upgrades performed yearly in the United States. The electrocardiographic traces vary according to the type of device, number and location of electrodes, as well as their configuration. Doctors must be familiar with these findings and have an understanding of the main causes of malfunction. This review was undertaken for that purpose showing the main features of pacemaker functioning, as well as, the correct interpretation of electrocardiographic traces and the main causes of malfunction...(AU)

Título: Complicaciones en el implante de marcapasos endocavitarios definitivos / Complications in definitive endocavitary pacemakers implantation

Resumo: Introducción: La progresiva disponibilidad de tecnología diagnóstica para detectar enfermedades del sistema eléctrico de conducción cardiaca, sumado a la eficiente aparatología, han sido motivos para una mayor indicación de implante de marcapaso. Al tratarse de un procedimiento invasivo, requiere de los cuidados habituales de un procedimiento quirúrgico, dado esto no se encuentra exento de complicaciones, dado al acto quirúrgico en si, como a factores propios del paciente. Objetivos: Detallar las posibles complicaciones del procedimiento, analizar sus posibles causas y describir las terapéuticas que se decidieron y proponer conductas preventivas eventuales. Material y métodos: Se revisaron los marcapasos implantados en el HAC entre Enero de 2010 hasta diciembre de 2014, incluyendo los recambios de generador y los cardiodes fibriladores, contando un total de 184 casos, de los cuales 16 casos presentaron un total de 11 complicaciones 5.97% siendo el 73% hombres, y un promedio de edad de 52 años con extremos de 28 a 94 años y ningún óbito consecuencia inmediata del procedimiento. Resultados: De los 184 casos evaluados, se obtuvo un total de 16 casos, de los cuales presentaron 13 complicaciones, que se dividieron en: hemopericardio (3 casos), lesión por decúbito que se subdividieron en a-predecúbito (2 casos), b-decúbito completo (3 casos), infección (1 caso), neumotórax (1 caso), rotura del catéter (1 caso), desplazamiento (2 casos) y asistolia (3 casos), de cada una de las complicaciones presentadas, se evaluó su diagnóstico y los procedimientos empleados para su resolución. Conclusión: La colocación de marcapasos, es un acto quirúrgico de moderada magnitud que puede presentar severas complicaciones con cierto riesgo de vida para el paciente, se debe tener el entrenamiento adecuado del equipo quirúrgico, no solo para su realización, sino además para poder detectarlas y poder contar con las herramientas para resolver las eventuales complicaciones.

Introduction: The progressive availability of diagnostic technology to detect diseases of electrical cardiac conductionsystem, coupled with efficient appliances have been grounds forfurther indication of pacer implantation. As an invasive procedure,requires regular care a surgical procedure, since it is not without complications, since the surgery itself, as factors specific to thepatient.Objectives: List the potential complications of the procedure,analyze possible causes and describe the therapeutic whodecided and propose possible preventive behaviors.Materials and methods: Pacemakers implanted in the HAC fromJanuary 2010 until December 2014 reviewed, including sparesgenerator and defibrillators, counting a total of 184 cases, ofwhich 16 cases had a total of 11 complications 5.97% and the73% men, mean age 52 years with extremes of 28 to 94 yearsand no death immediate consequence of the procedure. Results: Hemopericardio (3 cases), decubitus ulcer which were subdividedinto a-predecúbito (2 cases), b-prone: Of the 184 cases studied, atotal of 16 cases, of which presented 13 complications, which were divided into was obtained full (3 cases), infection (1 case), pneumothorax (1 case), catheter fracture (1 case), travel (2cases) and asystole (3 cases) of each of the presentedcomplications, diagnosis was assessed and the procedures forresolution. Conclusion Pacemaker implantation is a surgical act ofmoderate magnitude that can present severe complications withsome risk of life for the patient, you must have the proper trainingof the surgical team, not only for its implementation, but also todetect and to have with the tools to resolve any complications.

Resumo: Introduction: The Mood Rhythm Instrument (MRI) is a questionnaire developed to assess the circadian rhythm of mood-related behaviors. The aim of this study was to translate this instrument from Brazilian Portuguese into Spanish. Methods: The translation process consisted of forward translation, adjustment, back translation, back translation review and harmonization. Results: Comparing the initial Spanish translation and the final Spanish version, there were no semantic differences and the items were not changed. Conclusions: The Spanish version of the MRI is ready to be tested in a Spanish population. In the future, assessing and comparing mood-related behaviors in transcultural studies will be possible(AU)

Alterations in circadian rhythms are associated with the development of metabolic disorders, such as obesity and metabolic syndrome. Moreover, the availability and consumption of highly palatable foods rich in fat and the exposure to stressful situations may contribute to the incidence and severity of these chronic diseases, leading to changes in energy balance, alterations in lipid metabolism and adipogenesis. Resumo: Mujer de 70 años de edad, con historia de un electrodo de estimulación cardiaca abandonado, que genera: un trastorno de la deglución y una deformidad estética que la hacen consultar en múltiples oportunidades. A pesar de que se planteó la opción del retiro quirúrgico, se logró una extracción endovascular por la tracción de un electrodo. Describimos la utilidad del eco intracardiaco y el catéter ENSnare®, para la extracción de los electrodos en este tipo de casos.

70-year-old woman with a history of an abandoned pacing lead who developed a deglutition disorder and an aesthetic deformity that made her consult in multiple specialties. Even though surgical removal was considered, endovascular extraction was accomplished with the traction of the lead. We describe the usefulness of the intracardiac echocardiography and the ENSnare® catheter for this type of lead extractions.

Resumo: El síndrome de Twiddler es una complicación infrecuente de los dispositivos de estimulación eléctrica cardíaca; a pesar de su rareza merece ser tenido en cuenta al evaluar pacientes con sospecha de disfunción del marcapasos. Presentamos el caso de un paciente de 62 años portador de marcapasos biventricular quien ingresó al Hospital Universitario San Vicente Fundación por insuficiencia cardíaca descompensada debida a síndrome de Twiddler. Los síntomas se resolvieron con el implante de una nueva terminal de resincronización cardíaca.

Twiddler´s syndrome is a rare complication of cardiac conduction devices; despite its low frequency, it should be considered when evaluating patients with suspicion of pacemaker´sdysfunction. We report the case of a 62 year old man who consulted Hospital Universitario San Vicente Fundación, in Medellín, Colombia, with decompensated heart failure due to dislodgement of the left ventricle lead of his recently implanted biventricular pacemaker. Thepatient improved with the placement of a new device.

Resumo: In the State of Amazonas, Brazil, urban expansion together with precarious basic sanitation conditions and human settlement on river banks has contributed to the persistence of waterborne and intestinal parasitic diseases. Time series of the recorded cases of cholera, typhoid fever, hepatitis A and leptospirosis are described, using data from different levels of the surveillance systems. The sources for intestinal parasitosis prevalence data (non-compulsory reporting in Brazil) were Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana (LILACS) and the annals of major scientific meetings. Relevant papers and abstracts in all languages were accessed by two independent reviewers. The references cited by each relevant paper were scrutinized to locate additional papers. Despite its initial dissemination across the entire State of Amazonas, cholera was controlled in 1998. The magnitude of typhoid fever has decreased; however, a pattern characterized by eventual outbreaks still remains. Leptospirosis is an increasing cause of concern in association with the annual floods. The overall prevalence of intestinal parasites is high regardless of the municipality and the characteristics of areas and populations. The incidence of hepatitis A has decreased over the past decade. A comparison of older and recent surveys shows that the prevalence of intestinal parasitic diseases has remained constant. The load of waterborne and intestinal parasitic diseases ranks high among the health problems present in the State of Amazonas. Interventions aiming at basic sanitation and vaccination for hepatitis A were formulated and implemented, but assessment of their effectiveness in the targeted populations is still needed.

Título: The application of multiplex fluorimetric sensor for the analysis of flavonoids content in the medicinal herbs family Asteraceae, Lamiaceae, Rosaceae

Resumo: BACKGROUND: The aim of our research work was to quantify total flavonoid contents in the leaves of 13 plant species family Asteraceae, 8 representatives of family Lamiaceae and 9 plant species belonging to familyRosaceae, using the multiplex fluorimetric sensor. Fluorescence was measured using optical fluorescence apparatus Multiplex(R) 3 (Force-A, France) for non-destructive flavonoids estimation. The content of total flavonoids was estimated by FLAV index (expressed in relative units), that is deduced from flavonoids UV absorbing properties. RESULTS: Among observed plant species, the highest amount of total flavonoids has been found in leaves ofHelianthus multiflorus (1.65 RU) and Echinops ritro (1.27 RU), Rudbeckia fulgida (1.13 RU) belonging to the family Asteraceae. Lowest flavonoid content has been observed in the leaves of marigold (Calendula officinalis) (0.14 RU) also belonging to family Asteraceae. The highest content of flavonoids among experimental plants of family Rosaceae has been estimated in the leaves of Rosa canina (1.18 RU) and among plant species of family Lamiaceae in the leaves of Coleus blumei (0.90 RU). CONCLUSIONS: This research work was done as pre-screening of flavonoids content in the leaves of plant species belonging to family Asteraceae, Lamiaceae and Rosaceae. Results indicated that statistically significant differences (P > 0.05) in flavonoids content were observed not only between families, but also among individual plant species within one family.
