[Ab] Resumo:Background: Baseline clinical variables are prognostic for overall survival (OS) in patients with castration-resistant prostate cancer (CRPC). Their prognostic and predictive value with agents targeting bone metastases, such as radium-223, is not established. Patients and methods: The radium-223 ALSYMPCA trial enrolled patients with CRPC and symptomatic bone metastases. Prognostic potential of baseline variables was assessed using Cox models. Percentage changes in biomarker levels from baseline were evaluated during the trial period; changes from baseline to week 12 were evaluated for association with OS and surrogacy. Results: Eastern Cooperative Oncology Group performance status, total alkaline phosphatase (tALP), lactate dehydrogenase (LDH), and prostate-specific antigen (PSA) at baseline were associated with OS (P ≤ 0.0003) in the intent-to-treat population (radium-223, N = 614; placebo, N = 307). tALP declined from baseline within 4 weeks after beginning radium-223, by week 12 declining in 87% of radium-223 and 23% of placebo patients (P < 0.001). LDH declined in 51% and 34% (P = 0.003), whereas PSA declined in 27% and 14% (P = 0.160). Mean tALP change from baseline was 32.2% decrease with radium-223 and 37.2% increase with placebo. Radium-223 patients with tALP decline from baseline to week 12 (confirmed ≥3 weeks from week 12) had 55% lower risk of death (hazard ratio = 0.45; 95% CI 0.34-0.61) versus those with no confirmed tALP decline. Proportional treatment effect (PTE) values for tALP, LDH, and PSA changes from baseline at week 12 as OS surrogate markers were 0.34 (95% CI: 0-0.746), 0.07 (95% CI: 0-0.211), and 0 (95% CI: 0-0.082), respectively. Conclusions: Significant tALP declines (versus placebo) occurred as early as 4 weeks after beginning radium-223 therapy. tALP or LDH declines at 12 weeks correlated with longer OS, but did not meet statistical surrogacy requirements. Dynamic changes in tALP and LDH during radium-223 treatments may be useful to monitor, but do not serve as surrogates for survival.

[Ab] Resumo:Register for new drugs in cancer care provides a picture of how the drugs are used in the daily clinical practice Today, an increasing number of cancer drugs are approved before traditional well-controlled phase 3 studies have been conducted and in many registration studies there is no participation of Swedish departments. This article describes the general experience of a caregiver initiated systematic follow-up of new cancer drugs that shows the possibility of obtaining a picture of the drug's use in routine care. From the register "New Pharmaceuticals in Cancer care", registrations from Stockholm-Gotland region are reported. The structure of the registry can be used with advantage in other therapeutic areas than cancer and can be supplemented with data from national and regional registers as well as quality registers including patient experiences. The knowledge is important to many actors in health care and can contribute to an evidence based, patient-safe and equal healthcare in accordance with current guidelines.

[Ab] Resumo:BACKGROUND/AIM: Prostate cancer frequently causes bone metastases and skeletal events that impair quality of life (QoL) and survival. The alpha emitter radium-223 is a new drug that improves treatment in men with castration-resistant prostate cancer (CRPC) and bone metastases. Our aim was to evaluate the effectiveness of radium-223. SUBJECTS AND METHODS: In this retrospective study we enrolled 48 subjects. Pain reduction, alkaline phosphatase (ALP), time to first symptomatic skeletal event, and QoL were the variables we evaluated. RESULTS: Radium-223 was well tolerated, with a manageable toxicity profile and a modest objective response rate. A considerable difference in serum ALP levels before and after treatment was observed, with a significant correlation between pain relief and QoL, which showed a value of R2 to 0.44 with a slope of 1.50 (p = 0.0021). CONCLUSIONS: Radium-223 showed a clinical benefit, with a reduction in pain symptoms in 58% of patients. Radium-223 was shown to be an effective and well-tolerated therapeutic option in patients with metastatic CRPC progressing after docetaxel plus prednisone treatment.

[Ab] Resumo:The present work deals with estimation and distribution of natural radionuclides in 40 soil samples collected from Wadi Al-Rummah (Qassim part) Central Saudi Arabia. Estimations were carried out using NaI (Tl) gamma-ray spectrometer. Activity concentrations ranged from 5.3 to 45.0 Bq kg-1, from 4.3 to 33.8 Bq kg-1 and from 38 Bq kg-1 to 273.1 Bq kg-1 for 226Ra, 232Th and 40K, respectively. The average activity concentrations of 226Ra, 232Th and 40K in soil samples was 15.5 Bq kg-1, 14.1 Bq kg-1 and 143.1 Bq kg-1. The results of this study were compared with other studies around the world. Radiation hazard parameters such as radium equivalent activity, annual dose, external hazard were calculated and compared with the recommended levels quoted from International Commission on Radiological Protection (ICRP-60) and United Nations Scientific Committee on the Effects of Atomic Radiation UNSCEAR reports.

[Ab] Resumo:A soil-to-plant transfer factor (TF) is an important parameter that could be used to estimate radionuclides levels in medicinal plants. This work reports concentrations of natural radionuclides ( Ra and K) and TFs in six Thai medicinal plants grown in central Thailand using an HPGe gamma ray spectrometer. Either root, leaf, or flower parts of each medicinal plant were selected for use in the investigation according to their practical uses in traditional medicine. The results showed that due to K being essential in plants, K had higher arithmetic means of activity concentrations and geometric means of TFs (geometric standard deviations in parentheses) of 610 ±â€¯260 Bq kg dry weight (DW) and 2.0 (1.4), respectively, than Ra, which had the activity concentrations and TFs of 4.8 ±â€¯2.6 Bq kg DW and 0.17 (1.8), respectively. The results also showed that the leaves of medicinal plants had higher activity concentrations and TFs than root and flower parts, probably due to higher metabolic activities in leaves. Furthermore, there was good agreement between the results from the current work and other similar reports on medicinal plants. The information obtained from this work could strengthen knowledge of natural radionuclides in plants and particularly increase available TF data on Thai medicinal plants.

[Ab] Resumo:Cherenkov Luminescence Imaging (CLI) is a new method to image radioactive therapeutic and diagnostic agents, primarily in preclinical studies. This study used Geant4 and Python to generate the predicted Cherenkov light production as a function of time for a set of isotopic chains of interest for targeted alpha therapy: Ra, Pb, and Tb. All are shown to produce substantial Cherenkov light, though time delays between initial decays and the production of Cherenkov light requires caution in interpreting CLI.

[Ab] Resumo:We examined the spatial distributions of Ra, Ra, Cs, and Cs concentrations (activities) in seawater off the western and southern Korean Peninsula in July 2014. Radium-228 (and Ra) concentrations in water samples varied widely from 5 to 14 mBq/L (2-4 mBq/L), showing a negative correlation with salinity, particularly at the surface off the western Korean Peninsula. This indicates that the seawaters in this area are fundamentally comprised of Ra-poor and high-saline Kuroshio Current water and Ra-rich and low-saline water (e.g., continental shelf water), with various mixing ratios. Although Fukushima Dai-ichi Nuclear Power Plant (FDNPP)-derived Cs was below the detection limit (<0.08 mBq/L) in waters off the western Korean Peninsula, low level Cs (0.1-0.2 mBq/L) was detected in waters off the southern Korean Peninsula accompanied by higher Cs concentrations (1.6-1.9 mBq/L) relative to that off the western Korean Peninsula. Combined with the lower radium concentrations, the detection of Cs is explained by mixing of FDNPP-derived radiocesium-contaminated Kuroshio Current water.

[Ab] Resumo:Located in the south-western part of Brazil, the state of Rio de Janeiro is geotectonically contained within a complex structural province that resulted in the amalgamation of the Western Gondwana Paleocontinent. To undertake an extensive radiological characterization of this complex geological province and investigate the influence of bedrock, soil type and soil chemical-physical characteristics on natural radionuclide levels in soils, 259 surface soil samples were collected that encompassed the main soil types and geological formations throughout the state. Gamma spectrometry analysis of the samples resulted in median values of 114 Bq.kg for K, 32 Bq.kg for Ra and 74 Bq.kg for Ra. The median value for Ra was similar to the world median value for soils, the K value was well below the worldwide value, and that for Ra exceeded the world median value. The intense weathering caused by the high rainfall rates and high temperatures may be responsible for the low levels of K in the soils, of which the strongly acidic and clayey soils are markedly K-depleted. A soil from a high-grade metamorphic rock (granulite) presented the lowest Ra (18 Bq.kg ) content, whereas the highest levels for Ra (92 Bq.kg ) and Ra (139 Bq.kg 1) were observed in a young soil enriched in primary minerals (Leptsol). A lowland soil (Gleysol) showed the highest median of K (301 Bq.kg ). Strongly acidic soils tended to present high amounts of Ra, and sandy soils tended to contain low levels of Ra. The external radiation dose indicates that the state has a background radiation level within the natural range.

[Ab] Resumo:Prostate cancer is the first cancer in men and has a specific tropism to bones. This tropism provided the rationale to develop bone targeting radiopharmaceutical agents, such as strontium, samarium and more recently the Radium-223, an alpha-emitter. In a phase III trial, ALSYMPCA, Radium-223 not only improved pain relief, but also impacted on overall survival. Despite an approval by both FDA and EMA, prescription of this agent remains limited by the lack of refund, especially in France. Radium-223 is currently evaluated in several clinical trials (combination with chemotherapy, radiotherapy or hormone therapy) in order to optimize the therapeutic sequences in metastatic bone prostate cancer and argues for being incorporated into the current therapeutic arsenal.
