[Ab] Resumo:This in vitro study aimed to evaluate the volume of polymerization shrinkage (VS), gap (VG), and void (VV) using computerized microtomography (µCT) in bulk fill resin composites and conventional class I restorations, and to establish a correlation between these factors. Class I cavities (4 x 5 x 4 mm), C-factor = 4.2, were performed on caries-free human third molars and randomly divided into five groups (n = 6): FSI (Filtek Supreme XTE incremental insertion); FSS [(Filtek Supreme XTE single insertion(SI)]; TBF [(Tetric Bulk Fill: SI and manual filling (MF)]; SFM (Sonic Fill: SI/MF); and SFS (SonicFill: SI and sonic filling). The teeth were scanned and analyzed by µCT at T0, after filling the cavity with resin, and at T1, after polymerization for VG and VV, and for VS (T1-T0). There was statistically significant difference in VS in µCT for the FSI and FSS groups and between SFS and FSS as well as some difference in VV for FSI and bulk fill resin composites and no difference in VG between the conventional technique and bulk fill composites. Bulk fill resin composites presented similar VS and gap formation to those of incrementally inserted conventional resin composites. There is a moderate and weak positive correlation between polymerization shrinkage and gap formation and void, respectively. The final gap formation was more dependent on the initial gap than on polymerization shrinkage or void volume.

[Ab] Resumo:General dental practice is increasingly being recognised as the ideal situation for the conduct of clinical trials into the longevity of restorations. The aim of this study was to investigate the survival of 64 nanofilled resin composite (Filtek Supreme XTE) restorations placed principally in loadbearing cavities using a Universal dentine bonding agent (Scotchbond Universal), in five UK dental practices by members of the UK-based practice-based research group, the PREP Panel. A split mouth design was used, comprising patients who required two restorations, with one of the restorations receiving a total etch approach using phosphoric acid and the other being placed using a self-etch approach. The results indicated good performance of the restorations examined, with no difference, in terms of marginal characteristics, between the restorations which received total etching and those which did not.

[Ab] Resumo:BACKGROUND: Cyclosporine (CsA) is one of the immunosuppressive drugs, whose pharmacokinetic characteristics vary greatly among individuals. The published data reveal conflicting effects of the polymorphism of MDR1 exon 12 SNP C1236T on the pharmacokinetics of cyclosporine.This study aims to conduct a meta-analysis to investigate the effect of SNP C1236T on the pharmacokinetics of cyclosporine. METHODS: A literature retrieval was conducted to find the relevant papers in databases including PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure (CNKI), Wan Fang Database (Wan Fang), Chinese Biomedical Literature Database (CBM), VIP Database for Chinese Technical Periodicals (VIP) electronic source for published studies until January 2017. The pharmacokinetic parameters, including C0 (trough blood concentration), C2 (whole-blood levels at 2 hours after drug intake), Cmax (the maximum concentration), and daily dose were extracted and a meta-analysis was performed by RevMan 5.3. RESULTS: A total of 11 papers concerning 1361 individuals were included in the meta-analysis. As for dose adjusted C0, the results showed difference between subjects carrying CC genotypes and TT genotypes (MD: 6.76, 95% CI [2.38, 11.14], P = .02]. As for C2, the results showed significant difference between subjects carrying CC genotypes and CT genotypes (MD: -18.50, 95% CI [-35.49, -1.52], P = .03), as well as CC genotypes and TT genotypes (MD: -19.01, 95% CI (-35.85, -2.16), P = .03). As for Cmax, daily dose, and C0, the overall results showed no major influence. CONCLUSIONS: MDR1 C1236T polymorphism may have a minor effect on cyclosporine pharmacokinetics in transplantation patients.

[Ab] Resumo:OBJECTIVE: Bioactive glasses and surface pre-reacted glass-ionomer (sPRG) filler possess cariostatic properties owing to ion release. Many studies investigated potential cariostatic effects; few studies evaluated the surface stability and the structural changes their surfaces undergo in acidic conditions. METHODS: The surface resistance against acid attack and the surface receptiveness for bacterial adhesion and biofilm formation of a sPRG-filled (Beautifil ll, Shofu) and conventional glass-filled (Herculite XRV Ultra, Kerr) resin-based composite (RBC), and a conventional glass-ionomer cement (GIC; Fuji IX GP Extra, GC) were examined. Specimens (n=3) were immersed in distilled water or lactic acid (pH 4.0) for 3 days. Bacterial growth and biofilm formation were recorded using optical density and SEM. RESULTS: Upon 3-day immersion in lactic acid, the surface of the sPRG-filled RBC revealed multiple holes, while virtually no change in surface integrity was observed for the conventional RBC and GIC. Bacterial growth measurements revealed that none of the materials inhibited Streptococcus mutans (p<0.05). Remarkably, cross-sectional SEM revealed that S. mutans had penetrated the etch pits induced by lactic acid in/around the sPRG filler. Ion-release measurements revealed that sPRG-filled RBC released boron and fluoride, while GIC only released fluoride. However, the concentration of ions released by both materials appeared not sufficient to inhibit bacterial growth. Moreover, the structural surface change and resultant increased surface roughness appeared to have promoted biofilm formation. SIGNIFICANCE: While having bioactive potential through ion release, the stability of surface integrity of bioactive materials is a key-parameter to be assessed with regard to their cariostatic potential.

[Ab] Resumo:OBJECTIVE: Establish the release kinetics of new chlorhexidine particles incorporated in a dental resin, and with the application of ultrasound. METHODS: Spherical chlorhexidine particles (SCP) were synthesized (5wt%), freeze dried and incorporated into UDMA-HEMA resins. Chlorhexidine diacetate (CDP) (5wt%) was similarly incorporated in separate resins. Resin discs were immersed in deionized water, and a release profile established (650h). Ultrasound was used to trigger chlorhexidine (CHX) release from the resin discs at specific durations (10-30s) and time intervals (1-425h). Chlorhexidine content was determined by UV-vis absorption. The chlorhexidine particles/polymer composites were characterized using TGA, SEM, and confocal microscopy. RESULTS: SCP exhibited structures with high chlorhexidine content (90-95%), and a Mean (SD) diameter of 17.2 (2.5)µm which was significantly (p<0.001) smaller than the CDP crystals at 53.6 (33.7)µm. The SCP discs had a lower (7.7%) CHX release compared to the CDP group (16.2%). Ultrasonication of the resin discs with increasing durations (10-30s) resulted in higher drug release rates. CDP release rates (CHX) over 650h were: 23.5% (10s), 42.6% (20s), 51.2% (30s), and for SCP (CHX) were; 9.8% (10s), 12.3% (20s), and 14.0% (30s). SEM/confocal microscopy revealed CDP discs exhibited dissolution associated with the particle surface and SCP from the interior. SIGNIFICANCE: Chlorhexidine spheres incorporated in a dental resin demonstrated a responsive and lower CHX release. Ultrasound enhanced CHX release and is useful in clinical situations where the drug is required on demand to treat severe or persistent infections.

[Ab] Resumo:OBJECTIVES: To determine the interfacial fracture toughness (iFT) and micro-tensile strength (µTBS) of composite cements bonded to dentin. METHODS: Fifty feldspar ceramic blocks (Vita Mark II, Vita Zahnfabrik) were luted onto dentin using two self-adhesive (G-CEM LinkAce, GC; SpeedCEM, Ivoclar Vivadent), two self-etch (Multilink Primer & Multilink Automix, Ivoclar Vivadent; Scotchbond Universal & RelyX Ultimate, 3 M ESPE), and one etch-and-rinse (Excite F DSC & Variolink II, Ivoclar Vivadent) composite cement (n=10). After 48h in 100% relative humidity at 37°C, one half of each tooth was sectioned in sticks with a chevron notch at the cement-dentin interface and tested in a 4-point bending test setup (iFT). The remaining half of the tooth was sectioned in micro-specimens and stressed in tension until failure (µTBS). The mode of failure was determined with a stereomicroscope at 50× magnification. Data were submitted to Weibull analysis and Pearson's correlation (α=0.05). RESULTS: At 10% probability of failure, no significant differences could be found using iFT, while the etch-and-rinse composite cement Variolink II presented a significantly higher µTBS at this level. At 63.2% probability of failure, the self-adhesive composite cement G-CEM LinkAce revealed a significantly lower µTBS and iFT, and the self-etch cement Multilink Automix also revealed a significantly lower µTBS than all other cements. The correlation found between iFT and µTBS was moderate and not significant (r =0.618, p=0.11). SIGNIFICANCE: Overall, the etch-and-rinse and 'universal' self-etch composite cements performed best. The micro-tensile bond strength and interfacial fracture toughness tests did not correlate well.
