[Ab] Resumo:This study aimed to systematically review the literature to address the question regarding the influence of different materials in the clinical and radiographic success of indirect pulp treatment in primary teeth. A literature search was carried out for articles published prior to January 2017 in PubMed/MEDLINE, CENTRAL, Scopus, TRIP and ClinicalTrials databases; relevant articles included randomized clinical trials that compared materials used for indirect pulp treatment in primary teeth. Two reviewers independently selected the studies and extracted the data. The effects of each material on the outcome (clinical and radiographic failures) were analyzed using a mixed treatment comparisons meta-analysis. The ranking of treatments according to their probability of being the best choice was also calculated. From 1,088 potentially eligible studies, 11 were selected for full-text analysis, and 4 were included in the meta-analysis. In all papers, calcium hydroxide liner was used as the control group versus an adhesive system, resin-modified glass ionomer cement or placebo. The follow-up period ranged from 24 to 48 months, with dropout rates of 0-25.7%. The material type did not significantly affect the risk of failure of the indirect pulp treatment. However, calcium hydroxide presented a higher probability of failure. In conclusion, there is no scientific evidence showing the superiority of any material used for indirect pulp treatment in primary teeth.

[Ab] Resumo:OBJECTIVE: To evaluate the effect of semiconductor laser irradiation on root canal sealing after routine root canal therapy (RCT). METHODS: Sixty freshly extracted single-rooted human teeth were randomly divided into six groups (n = 10). The anatomic crowns were sectioned at the cementoenamel junction and the remaining roots were prepared endodontically with conventional RCT methods. Groups A and B were irradiated with semiconductor laser at 1W for 20 seconds; Groups C and D were ultrasonically rinsed for 60 seconds as positive control groups; Groups E and F without treatment of root canal prior to RCT as negative control groups. Root canal sealing of Groups A, C and E were evaluated by measurements of apical microleakage. The teeth from Groups B, D and F were sectioned, and the micro-structures were examined with scanning electron microscopy (SEM). One way ANOVA and LSD-t test were used for statistical analysis (α = .05). RESULTS: The apical sealing of both the laser irradiated group and the ultrasonic irrigated group were significantly different from the control group (p<0.5). There was no significant difference between the laser irradiated group and the ultrasonic irrigated group (p>0.5). SEM observation showed that most of the dentinal tubules in the laser irradiation group melted, narrowed or closed, while most of the dentinal tubules in the ultrasonic irrigation group were filled with tooth paste. CONCLUSION: The application of semiconductor laser prior to root canal obturation increases the apical sealing of the roots treated.

[Ab] Resumo:To evaluate the influence of rewetting solutions on bond strength to root dentin of conventional gutta-percha (GP) or niobium phosphate glass-based gutta-percha (GNb) associated with a bioceramic sealer. The root canals of 80 human mandibular premolars were prepared using nickel-titanium instruments and irrigation with sodium hypochlorite and EDTA. The teeth were randomly divided into four groups according to the gutta-percha used: GNb or GP associated with EndoSequence BC Sealer (BC) and the solution for rewetting dentin before filling (distilled water; phosphate buffer saline solution - PBS; simulated body fluid - SBF; or no solution). The root canals were filled with a single cone using warm vertical condensation. Micropush-out bond strengths associated with the filling materials in slices from middle root thirds was determined 30 days after root filling. The failure mode was analyzed with stereoscopic lens. The data were statistically analyzed by two-way ANOVA and Holm-Sidak test (p < 0.05). There was significant difference in the types of gutta-percha (p < 0.001) and in the different rewetting solutions (p = 0.003). The interaction between gutta-percha and rewetting solutions was not significant (p = 0.53). The SBF solution provided an increase in bond strength for both gutta-percha solutions. The GNb+BC (3.42 MPa) association increased bond strength when compared with GP+BC (2.0 MPa). The use of SBF as a dentin rewetting solution increased bond strength in the groups studied. Association of GNb with bioceramic sealer was beneficial, increasing the bond strength to dentin when compared with the association with GP.

[Ab] Resumo:To assess the physicochemical properties of AH Plus, GuttaFlow 2, GuttaFlow BioSeal, and MM Seal, five samples of each root canal sealer were evaluated to determine their setting time (ST), dimensional change (DC), solubility (SL), flow (FL), and radiopacity (RD) according to American National Standards Institute/American Dental Association (ANSI/ADA) Specification 57. The distilled and deionized water obtained from the SL test were subjected to atomic absorption spectrometry to observe the presence of Ca2+, K+, and Na+ ions. Statistical analysis was performed by using one-way ANOVA and Tukey-Kramer tests (p < 0.05). The following results were obtained: ST (min) (AH Plus 463.6 ± 13.22; GuttaFlow 2 24.35 ± 2.78; GuttaFlow Bioseal 17.4 ± 0.55; MM Seal 47.60 ± 4.39), DC (%) (AH Plus 0.06 ± 0.12; GuttaFlow 2 -26.06 ± 1.24; GuttaFlow Bioseal 2.10 ± 1.47; MM Seal 8.47 ± 2.41), SL (%) (AH Plus 0.41 ± 0.21; GuttaFlow 2 5.13 ± 4.11; GuttaFlow Bioseal 3.03 ± 1.05; MM Seal 0.94 ± 0.17), FL (mm) (AH Plus 36.42 ± 0.40; GuttaFlow 2 36.44 ± 0.05; GuttaFlow Bioseal 35.4 ± 0.03; MM Seal 52.75 ± 0.60), and RD (mmAl) (AH Plus 7.52 ± 1.59; GuttaFlow 2 6.85 ± 0.14; GuttaFlow Bioseal 7.02 ± 0.18; MM Seal 3.32 ± 0.90). ST, DC, SL, FL, and RD showed statistical differences among the root canal sealers (p < 0.05). As AH Plus showed the lowest DC and SL values (p < 0.05), the findings indicate that this sample is the only sealer conforming to ANSI/ADA standards.

[Ab] Resumo:BACKGROUND: Alkaline pH is responsible for antibacterial activity and the stimulation of periapical tissue healing. It neutralizes the acidic environment of inflammatory tissues in the periapical region of the teeth and favors bone repair by activating tissue enzymes. OBJECTIVES: The aim of this study was to evaluate and compare in vitro the pH of 8 root canal filling materials (sealers and points) -AH Plus Jet (AH), Apexit Plus (AP), Endomethasone N (END), Epiphany (EP), GuttaFlow (GF), gutta-percha (G), Resilon (R), Tubliseal (T). MATERIAL AND METHODS: 0.1 g of each material (n = 6) was placed in dialysis tubes and immersed in 20 mL of deionized water. The control contained deionized water (pH 6.6) with an empty tube. The pH values were recorded immediately after immersion (baseline) and after 1, 2, 24, 48, 120, and 192 h with a pH-meter. Data were statistically analyzed using the Student's -t test and 1-way analysis of variance (p < 0.05). RESULTS: Nearly all the materials had pH significantly higher than the control (p < 0.05). There were significant differences in the pH between the materials tested at each time point (p < 0.001). The highest pH was exhibited by EP, followed by AP and AH. The lowest pH was shown by GF, G and R. CONCLUSIONS: Among the materials studied, only EP, AP and AH Plus were able to elevate the pH level that would allow inactivation of microorganisms in the root canals and promote healing of inflamed periapical tissues. However, the low alkalizing potential of G and R can be modified by the concomitant application of sealers producing alkaline pH.

[Ab] Resumo:OBJECTIVE: To investigate the effect of thermal conductivity on the apical sealing ability of different dental gutta-percha cones during the warm vertical condensation obturation. METHODS: Four kinds of dental gutta-percha cones were used in this study: Bio-GP points (BP, B&L, Korea), large-tapered gutta-percha (DP, DENTSPLY, America), PROTAPER Universal gutta-percha points (DPP, DENTSPLY, America) and mtwo gutta-percha points (VP, VDW, Germany). Volume differences method was used to determine the main components and the thermal conductivity determinator was used to measure the thermal conductivity of these dental gutta-percha cones. Furthermore, 20 cones randomly selected from each kind of dental gutta-percha cones. And 10 cones of each dental gutta-percha cones, which were cut out the part of 4 mm in length from the apical end, were heated (200 °C) from the upper end without compression, and the temperature of the gutta perchacones surface was monitored in time by the infrared thermal imager during the whole heating process. In addition, the rest of the 10 cones of each dental gutta-percha cones were used to obturate the root canal in the transparent root canal resin model using warm vertical condensation technique. The cross-sectional surface was observed by stereomicroscope (× 40) at 1 mm and 3 mm from the working length and the gutta-percha obturation area proportion was measured and analyzed. The data were analyzed by one-way ANOVA. RESULTS: The proportion of inorganic fillers (80.90%±1.14 %) (P<0.05) and the thermal conductivity (2.247±0.002) of DP was significantly higher than BP (79.28%±3.88%, 1.179±0.003), DPP (68.46%±5.09%, 0.604±0.001), VP (78.86%±1.87%, 1.150±0.001) (P<0.05). During the thermal obturation without compression at the setting temperature (200 °C), DP could achieve 65 °C beyond 1 mm from the heating point, and BP, DPP and VP only reached 65 °C within 1 mm. After warm vertical condensation obturation, all the groups showed increased gutta-percha obturation area proportion. At the position of WL-3, DP (96.89%±0.03%) showed significantly higher proportion of gutta-percha obturation area than BP (95.47%±0.06%), DPP (95.21%±0.03%) and VP (95.15%±0.03%) (P<0.05). CONCLUSION: DP contains more inorganic fillers, possesses higher thermal conductivity, and leads a better apical sealing ability than BP,DPP and VP at the position of WL-3 during warm vertical condensation obturation.

[Ab] Resumo:OBJECTIVE: To compare the effect of four different techniques on removal of vapor lock in the apical region of curved root canals. METHODS: Forty simulated resin root canals with 45° curvature were prepared using WaveOne Primary, then the apical foramen were sealed with soft wax. The teeth were divided randomly into 4 groups thereafter (n=10). Contract solution was injected into the canals using a 30 G side-vented needle and scanned with cone-beam CT (CBCT) to identify the volume of the vapor lock. Four different techniques including photon-induced photoacoustic streaming (PIPS) laser-activated irrigation, gutta-percha cone technique, ultrasonic irrigation, and sonic irrigation were used to remove the vapor locks in the root canals. The residual volume of the vapor lock was identified again using CBCT scanning data. Accordingly, the reduction rates of the vapor lock were calculated. Furthermore, the initial and residual vapor lock length was calculated. The data were analyzed by using the One-way ANOVA analysis and Kruskal-Wallis H test at a significance level of P<0.05. RESULTS: There was no significant difference in the initial vapor lock volume (P>0.05). Residual volume of the vapor lock for PIPS laser-activated irrigation was 0 mm(3), and that for gutta-percha cone technique was (0.02±0.07) mm3, significantly lower than those of ultrasonic and sonic irrigation, the values being (0.20±0.09) mm(3) and (0.23±0.06) mm(3) (P<0.001), respectively. The reduction rates of the vapor lock of PIPS laser-activated irrigation and gutta-percha cone technique were 100.00% (100.00%, 100.00%) and 100.00% (77.66%, 100.00%), respectively, significantly higher than those of ultrasonic irrigation [70.37% (56.41%, 91.43%)] and sonic irrigation [63.54% (51.47%, 74.00%), P<0.001]. The length of the residual vapor lock for PIPS laser-activated irrigation was 0 mm, and that for gutta-percha cone technique was (0.15±0.47) mm, significantly lower than those of ultrasonic and sonic irrigation, values being (2.21±0.09) mm and (2.34±0.08) mm (P<0.001), respectively. The length of the residual vapor locks in the ultrasonic and sonic group remained approximately the same as the distance between the working tip and the apical foramen. CONCLUSION: PIPS laser activated irrigation and gutta-percha cone technique could remove the vapor lock from the apical region of curved canals effectively.

[Ab] Resumo:OBJECTIVES: This study evaluated the intensity and duration of postoperative pain after the removal of root canal filling material in retreatment procedures of upper incisor teeth with chronic apical periodontitis, using different techniques. MATERIALS AND METHODS: One hundred and thirty-five patients requiring retreatment of upper incisor teeth with chronic apical periodontitis were included in the study. The patients were assigned to three groups of 45 patients, according to the method used to remove old canal filling material. In group 1, canal filling material was removed using hand files. In group 2, the canal filling material was removed with ProTaper universal retreatment (PTUR) instruments. In group 3, Reciproc instruments were used to remove canal filling material. Teeth were then medicated with calcium hydroxide and sealed using temporary filling material. The presence of postoperative pain was assessed after 6, 12, 24, 48 and 72 h, 7 days, and finally after 10 days. RESULTS: In all time intervals, except for 72 h, 7 days and 10 days, group 1 participants reported more intense postoperative pain than those in groups 2 and 3 (p = 0.02). In all time intervals, there was no difference in the pain scores between groups 2 and 3 (p = 0.08). In all groups, the intensity of postoperative pain decreased over time. The required time to remove canal filling material was less for the Reciproc group compared to the hand and ProTaper retreatment groups (p = 0.032). CONCLUSIONS: Hand files caused greater postoperative pain after non-surgical endodontic retreatment (NSER) of upper incisor teeth with chronic apical periodontitis compared to the ProTaper retreatment and Reciproc files.

[Ab] Resumo:We have shown that Computed Microtomography (CMT) is able to map the internal distribution of the filler particles in ProTaper , Lexicon , and GuttaCore materials, and explain the differences in their tensile and ductility properties, prior to mechanical manipulation. Working of uncrosslinked ProTaper and Lexicon samples resulted in a five-fold increase in ductility and the tensile elongation at break. CMT mapping of the internal structure showed that large, periodic, striations formed across the interior of the sample corresponding to the formation of regions with low filler particle density. In contrast to metals which harden upon working, this migration of particles away from the high stress regions resulted in stress softening, as predicted by the Mullins effect. The results indicate that CMT is an effective method for 3-D visualization of the internal particle distribution which permits the determination of structure-property relationships and facilitates the design of new materials.
