[Ab] Resumo:Objective: To evaluate solubility, dimensional stability, filling ability and volumetric change of root-end filling materials using conventional tests and new Micro-CT-based methods. Material and Methods: 7. Results: The results suggested correlated or complementary data between the proposed tests. At 7 days, BIO showed higher solubility and at 30 days, showed higher volumetric change in comparison with MTA (p<0.05). With regard to volumetric change, the tested materials were similar (p>0.05) at 7 days. At 30 days, they presented similar solubility. BIO and MTA showed higher dimensional stability than ZOE (p<0.05). ZOE and BIO showed higher filling ability (p<0.05). Conclusions: ZOE presented a higher dimensional change, and BIO had greater solubility after 7 days. BIO presented filling ability and dimensional stability, but greater volumetric change than MTA after 30 days. Micro-CT can provide important data on the physicochemical properties of materials complementing conventional tests.

[Ab] Resumo:BACKGROUND: Alkaline pH is responsible for antibacterial activity and the stimulation of periapical tissue healing. It neutralizes the acidic environment of inflammatory tissues in the periapical region of the teeth and favors bone repair by activating tissue enzymes. OBJECTIVES: The aim of this study was to evaluate and compare in vitro the pH of 8 root canal filling materials (sealers and points) -AH Plus Jet (AH), Apexit Plus (AP), Endomethasone N (END), Epiphany (EP), GuttaFlow (GF), gutta-percha (G), Resilon (R), Tubliseal (T). MATERIAL AND METHODS: 0.1 g of each material (n = 6) was placed in dialysis tubes and immersed in 20 mL of deionized water. The control contained deionized water (pH 6.6) with an empty tube. The pH values were recorded immediately after immersion (baseline) and after 1, 2, 24, 48, 120, and 192 h with a pH-meter. Data were statistically analyzed using the Student's -t test and 1-way analysis of variance (p < 0.05). RESULTS: Nearly all the materials had pH significantly higher than the control (p < 0.05). There were significant differences in the pH between the materials tested at each time point (p < 0.001). The highest pH was exhibited by EP, followed by AP and AH. The lowest pH was shown by GF, G and R. CONCLUSIONS: Among the materials studied, only EP, AP and AH Plus were able to elevate the pH level that would allow inactivation of microorganisms in the root canals and promote healing of inflamed periapical tissues. However, the low alkalizing potential of G and R can be modified by the concomitant application of sealers producing alkaline pH.

[Ab] Resumo:BACKGROUND: Various obturation techniques have been evaluated for better filling of the root canals in primary teeth using different methods. Spiral Computed Tomography (SCT) is a new revolution in the pediatric endodontics for assessment of quality of the obturation from 3 dimensions. OBJECTIVES: To evaluate the efficiency of 5 different obturation methods in delivering the filling material into the canals of primary teeth using Spiral Computed Tomography scan. STUDY DESIGN: A total of 50 canals of primary teeth were prepared, divided into 5 groups with 10 canals in each group and obturated with Zinc Oxide Eugenol cement using 5 different obturation techniques such as Local anesthetic syringe, Tuberculin syringe, Endodontic plugger, hand held Lentulo-spiral, and Lentulo-spiral mounted on slow speed hand piece. The pre and post obturation volume and finally the Percentage of Obturated Volume (POV) were calculated using SCT scan for each group. The data obtained was statistically analyzed using One-way Analysis of Variance (ANOVA) and Tukey's post-hoc test. RESULTS: Lentulo-spiral hand held showed highest POV value followed by Lentulospiral mounted to hand piece, Tuberculin syringe and Endodontic plugger; whereas Anesthetic syringe had least POV (P < 0.05). CONCLUSION: Lentulo-spiral hand held is the best obturating technique among the 5 groups evaluated as the canals of this group showed maximum percentage of filled material. However, a further study with large sample size is highly essential.

[Ab] Resumo:AIM: This in vitro study evaluated the efficacy of three different obturation techniques with regards to quality of two filling pastes-Ca(OH) /iodoform syringe paste and zinc oxide eugenol paste in primary molars. METHODS: Root canals of 45 extracted primary molars were prepared and randomly divided into three groups of 15 teeth each. Group A-canals were filled with Ca(OH) /iodoform syringe paste, Group B-zinc oxide eugenol paste with handheld lentulo spiral and Group C-zinc oxide eugenol paste with rotary lentulo spiral. The quality of filled root canals was evaluated with conventional radiography in antero-posterior and lateral dimensions. RESULTS: There were significant differences between all groups in the presence of voids (p = 0.03) and length of filling (p = 0.002). Half of the sampled teeth in handheld lentulo spiral group had voids in the filled canals, while 80% of the teeth filled with Ca(OH) /iodoform syringe resulted in overfilling of the paste. CONCLUSIONS: All three delivery methods for the obturation of primary molars' root canals showed inherent limitations in terms of voids and quality of filling. Voids are inevitable and were present in all the techniques. Overfilling was more frequently seen with the syringe method, while the lentulo spiral technique, both handheld and rotary, showed a better quality of filling.

[Ab] Resumo:Although formaldehyde is well known to cause type 4 hypersensitivity, immunoglobulin E (IgE)-mediated hypersensitivity to formaldehyde is rare. Here, we report a case of recurrent generalized urticaria after endodontic treatment using a para-formaldehyde (PFA)-containing root canal sealant and present a review of previous studies describing cases of immediate hypersensitivity reactions to formaldehyde. A 50-year-old man visited our allergy clinic for recurrent generalized urticaria several hours after endodontic treatment. Prick tests to latex, lidocaine, and formaldehyde showed negative reactions. However, swelling and redness at the prick site continued for several days. The level of formaldehyde-specific IgE was high (class 4). Thus, the patient was deemed to have experienced an IgE-mediated hypersensitivity reaction caused by the PFA used in the root canal disinfectant. Accordingly, we suggest that physicians should pay attention to type I hypersensitivity reactions to root canal disinfectants, even if the symptoms occur several hours after exposure.

[Ab] Resumo:AIM: This study was performed to assess the clinical and radiographic success rates of a formocresol and zinc oxide eugenol (ZOE) primary molar root canal therapy (RCT) technique. The effects of this treatment on the permanent successors and on exfoliation times were also investigated. MATERIALS AND METHODS: Study design: the retrospective study included 161 patients with 211 primary molars treated by RCT by a single operator in a private paediatric dental office in the Toronto area. Data were coded and entered into a Microsoft Excel database and analysis undertaken using SPSS software. Predominantly non-parametric tests were used to evaluate statistical differences (p < 0.05). RESULTS: A clinical success rate of 90% (190/211) and a radiographic success rate of 77.3% (136/176) were obtained. Following RCT in a primary molar, enamel defects were found in 6.8% (7/103) of premolars, all of which occurred in first premolars, and in patients treated at a mean age of 54.1 months (p < 0.005). Treated molars exfoliated on average 7.6 months sooner than contralateral teeth (p < 0.005). CONCLUSION: This formocresol and ZOE RCT is a viable treatment for necrotic primary molars and yielded very high clinical and acceptable radiographic success rates.

[Ab] Resumo:INTRODUCTION: This study aimed at evaluating and comparing the antibacterial activity of six types of dental luting cements on Streptococcus mutans and Lactobacillus acidophilus using the agar diffusion test (ADT) and the direct-contact test (DCT). The antibacterial activity in ADT was measured based on the diameter of the zone of inhibition formed, whereas in DCT the density of the bacterial suspension was measured. The lower the density of the suspension, the more antibacterial activity the cement possesses. MATERIALS AND METHODS: Agar diffusion test was carried out on the bacteria. After an incubation period of 24 hours, the plates were checked for the presence of zone of inhibition. In DCT the cement was mixed and applied. Once the cement was set, bacterial suspension and brain-heart infusion medium was poured and incubated for 24 hours. After 24 hours, the plate was placed in the enzyme-linked immunosorbent assay plate reader, which measured the optical density of the fluid. The first set of data was recorded approximately 1 hour after incubation. Overall, three sets of data were recorded. Additional experiments were performed on set test materials that were allowed to age for 24 hours, 1 week, 1, 3, and 6 months. RESULTS: When using ADT only two cements zinc oxide eugenol (ZOE) and zinc polycarboxylate (ZPC) cement showed antibacterial activity against the test organisms. When using DCT, all cements showed some amount of antibacterial activity. Zinc oxide eugenol and ZPC cement showed highest amount of antibacterial activity against S. mutans and L. acidophilus respectively. CONCLUSION: Within the limitations of study, ZOE cement and ZPC cement were most effective against the tested microorganisms followed by the newer resin cement. The glass ionomer cement was the weakest of all. CLINICAL SIGNIFICANCE: Patients with high caries index can be treated more effectively using the abovementioned cements.

[Ab] Resumo:INTRODUCTION: The aim was to compare the solubility, radiopacity, and setting times of a tricalcium silicate-containing (BioRoot RCS; Septodont, St Maur-des-Fossés, France) and a mineral trioxide aggregate-containing sealer (MTA Fillapex; Angelus, Londrina, Brazil) with an epoxy resin-based sealer (AH Plus; Dentsply DeTrey, Konstanz, Germany). METHODS: Solubility in distilled water, radiopacity, and setting time were evaluated in accordance with ISO 6876:2012. The solubility was also measured after soaking the materials in phosphate-buffered saline buffer (PBS). All data were analyzed using 1-way analysis of variance and the Student-Newman-Keuls test. RESULTS: After immersion for 1 minute in distilled water, BioRoot RCS was significantly less soluble than AH Plus and MTA Fillapex (P < .05). At all other exposure times, AH Plus was significantly less soluble than BioRoot RCS, whereas BioRoot RCS was significantly more soluble than the other 2 sealers (P < .05). All sealers had the same solubility in PBS and distilled water, except for BioRoot RCS after 28 days. At this exposure time, BioRoot RCS was significantly less soluble in PBS than in distilled water and less soluble than MTA Fillapex (P < .05). All BioRoot RCS specimens immersed in PBS had a surface precipitate after 14 and 28 days. The radiopacity of all sealers was greater than 3 mm aluminum with no statistical significant difference between the sealers (P > .05). The final setting time was 324 (±1) minutes for BioRoot RCS and 612 (±4) minutes for AH Plus. The difference was statistically significant (P < .05). MTA Fillapex did not set completely even after 1 week. CONCLUSIONS: The solubility and radiopacity of the sealers were in accordance with ISO 6876:2012. PBS decreased the solubility of BioRoot RCS.

[Ab] Resumo:OBJECTIVE: To compare the effects of three root canal sealers with respect to time on biocompatibility of human periodontal ligament cells (hPDLCs).The sealers included zinc oxide and eugenol based sealers (ZOE), epoxy resin sealers (ERS) and silicone based sealers (SBS). METHODS: hPDLCs were primarily cultured,with the method combining of tissue explant and enzymatic digestion. The cells were then exposed to different extract fluids: (1) ZOE extracted for 24 h group ;(2) ZOE extracted for 1 week group;(3)ZOE extracted for 2 weeks group;(4) ERS extracted after 24 h group; (5) ERS extracted after 1 week group; (6) ERS extracted after 2 weeks group; (7) SBS extracted after 24 h group; (8) SBS extracted after 1 week group; (9) SBS extracted after 2 weeks group; (10) Dulbecco modified Eagle's medium/F12 (DMEM/F12) as negative control group. Cell morphology was observed under an inverted microscope.Cell proliferation was measured by methyl-thiazol-diphenyltetrazolium (MTT) assay. ALP assay kit was used for measuring alkaline phosphatase (ALP) activity. Sealers of 2 weeks' setting time were then immersed in an osteogenic medium for examination of mineral nodules and calcium deposits. RESULTS: Considering the relative growth rate(RGR),ZOE was severely to moderately cytotoxic(RGR:13.6%-39.9%), while ERS was slightly or not cytotoxic (RGR: 87.6%-95.3%).Only SBS did not show any cytotoxicity after setting (RGR: 91.8%-106.7%). The setting time influenced the cytotoxicity of ERS which decreased after 1 week. Considering the ALP activity,there was no difference between SBS group and control group (F=3.397,P=0.053). According to the results of calcium deposits, ZOE:D562 nm=0.180±0.050,ERS: D562 nm=2.968±0.201,SBS:D562 nm=3.623±0.039,Control:D562 nm=3.477±0.102,the ranking of ALP activity and calcium deposits was as follows: ZOE

[Ab] Resumo:INTRODUCTION: The present study aimed to rank the bond strength to root dentin of a new injectable pozzolan-based root canal sealer, EndoSeal MTA, as compared with MTA Fillapex and AH Plus. METHODS: Eighteen dentinal slices (1 ± 0.1 mm) were obtained from the middle third of 6 maxillary incisors previously selected. Three canal-like holes with 0.8 mm diameter were drilled perpendicularly on the axial surface of each slice. Then, a standardized irrigation was applied for all holes that were subsequently filled with 1 of 3 test root canal sealers. After that, slices were stored in contact with phosphate-buffered saline solution (pH 7.2) for 7 days at 37°C before the push-out assay. Friedman test and Wilcoxon signed rank test with a Bonferroni correction were used to rank the results. Significance boundary was set at α = 5%. RESULTS: Friedman test confirmed a significant dissimilarity in push-out ranks among the tested cements (P < .01). Wilcoxon signed rank test demonstrated AH Plus had significant superior resistance to dislodgment compared with Endo Seal (P < .01) or MTA Fillapex (P < .01), whereas MTA Fillapex presented the lowest push-out values as compared with Endo Seal (P < .01) or AH Plus (P < .01). CONCLUSIONS: EndoSeal presents satisfactory bond strength performance for application in endodontic therapy compared with MTA Fillapex, and although it displays a new alternative of injectable bio-tight root canal sealer, it is not able to improve adhesion compared with AH Plus.
