[Ab] Resumo:In South Dakota, the status of equid well being is relatively unknown. This study sought to (a) gain understanding about the current perceptions of nonhuman animal well being in South Dakota, with an emphasis on horses and other equids; (b) determine the level of care equids are reportedly receiving and the perceived challenges to equine well being in South Dakota, and (c) determine if people from diverse geographical locations (east or west of the Missouri River) have similar views on the well being of equids in South Dakota. Respondents indicated the current level of equid well being in South Dakota is sufficient, but there is room for improvement. Current challenges for the equid population of South Dakota were the high annual cost of horse care, poor horsemanship, dental problems, and whether caregivers understand basic equine care. Several significant associations arose between where a respondent lives (Western or Eastern South Dakota) and their level of agreement with various statements. The results provide a benchmark to gauge well being and help give direction for future educational needs that can continue to improve equid care.

[Ab] Resumo:Patients with inoperable, high-risk, and intermediate-risk aortic stenosis can now be treated with transcatheter aortic valve replacement. Centers for Medicare and Medicaid Services and the Food and Drug Administration selectively choose centers based on experience and require a collaborative, multidisciplinary team approach in the treatment and decision making for these patients. The work-up has been streamlined. Gated multislice computed tomography angiogram has emerged as the gold standard for assessment of valve anatomy and sizing of the transcatheter heart valve. Assessment of risk has evolved to include a more comprehensive functional and frailty evaluation. Long term-results are needed before the expansion of transcatheter aortic valve replacement into the low-risk category.

[Ab] Resumo:OBJECTIVE: The objective of this study is to optimize MRI logistics through evaluation of MRI workflow and analysis of performance, efficiency, and patient throughput in a tertiary care academic center. SUBJECTS AND METHODS: For 2 weeks, workflow data from two outpatient MRI scanners were prospectively collected and stratified by value added to the process (i.e., value-added time, business value-added time, or non-value-added time). Two separate time cycles were measured: the actual MRI process cycle as well as the complete length of patient stay in the department. In addition, the impact and frequency of delays across all observations were measured. RESULTS: A total of 305 MRI examinations were evaluated, including body (34.1%), neurologic (28.9%), musculoskeletal (21.0%), and breast examinations (16.1%). The MRI process cycle lasted a mean of 50.97 ± 24.4 (SD) minutes per examination; the mean non-value-added time was 13.21 ± 18.77 minutes (25.87% of the total process cycle time). The mean length-of-stay cycle was 83.51 ± 33.63 minutes; the mean non-value-added time was 24.33 ± 24.84 minutes (29.14% of the total patient stay). The delay with the highest frequency (5.57%) was IV or port placement, which had a mean delay of 22.82 minutes. The delay with the greatest impact on time was MRI arthrography for which joint injection of contrast medium was necessary but was not accounted for in the schedule (mean delay, 42.2 minutes; frequency, 1.64%). Of 305 patients, 34 (11.15%) did not arrive at or before their scheduled time. CONCLUSION: Non-value-added time represents approximately one-third of the total MRI process cycle and patient length of stay. Identifying specific delays may expedite the application of targeted improvement strategies, potentially increasing revenue, efficiency, and overall patient satisfaction.

[Ab] Resumo:Under the Congressional Review Act, Congress has passed, and the President has signed, Public Law 115-21, a resolution of disapproval of OSHA's final rule titled, "Clarification of Employer's Continuing Obligation to Make and Maintain an Accurate Record of each Recordable Injury and Illness." OSHA published the rule, which contained various amendments to OSHA's recordkeeping regulations, on December 19, 2016. The amendments became effective on January 18, 2017. Because Public Law 115-21 invalidates the amendments to OSHA's recordkeeping regulations contained in the rule promulgated on December 19, 2016, OSHA is hereby removing those amendments from the Code of Federal Regulations.

[Ab] Resumo:OBJECTIVE: To estimate the prevalence of non-nutritive sweeteners (NNS) consumption in a sample of patients with diabetes. MATERIAL AND METHODS: We applied two questionnaires, one of food frequency adapted to products containing NNS and the other of beliefs related to NNS. The prevalence of NNS consumption was determined and correlated with the body mass index, energy and sugar consumption, waist circumference, glycated hemoglobin, triglycerides, diabetes type, education and socioeconomic status. RESULTS: The prevalence of NNS consumption was 96%; the consumption was greater in men and in patients with type 1 diabetes. A negative correlation was found between the consumption and age and a positive correlation with glycated hemoglobin and education. CONCLUSIONS: The prevalence of NNS consumption is high due to the great availability of products in the market.

He included charts relating to diseases observed in different species such as horses, cows, goats, sheep, pigs and dogs. He described the evolution of the foot and mouth epidemic in the region during the years 1855 and 1856. Abraham Buehler stated that with this work he aimed to encourage his colleagues to also collect data.

[Ab] Resumo:Peer review serves as an important adjunct to other hospital quality and safety programs. Despite its importance, the available literature contains virtually no guidance regarding the structure and function of effective peer review committees. This Clinical Perspective provides a summary of the purposes, structure, and functioning of effective peer review committees. We also discuss important legal considerations that are a necessary component of such processes. This discussion includes useful templates for case selection and review. Proper committee structure, membership, work flow, and leadership as well as close cooperation with the hospital medical executive committee and legal representatives are essential to any effective peer review process. A thoughtful, fair, systematic, and organized approach to creating a peer review process will lead to confidence in the committee by providers, hospital leadership, and patients. If properly constructed, such committees may also assist in monitoring and enforcing compliance with departmental protocols, thus reducing harm and promoting high-quality practice.

[Ab] Resumo:Sedentary behaviour (SB) has emerged as a modifiable risk factor, but little is known about measurement errors of SB. The purpose of this study was to determine the validity of 24-h Physical Activity Recall (24PAR) relative to SenseWear Armband (SWA) for assessing SB. Each participant (n = 1485) undertook a series of data collection procedures on two randomly selected days: wearing a SWA for full 24-h, and then completing the telephone-administered 24PAR the following day to recall the past 24-h activities. Estimates of total sedentary time (TST) were computed without the inclusion of reported or recorded sleep time. Equivalence testing was used to compare estimates of TST. Analyses from equivalence testing showed no significant equivalence of 24PAR for TST (90% CI: 443.0 and 457.6 min · day ) relative to SWA (equivalence zone: 580.7 and 709.8 min · day ). Bland-Altman plots indicated individuals that were extremely or minimally sedentary provided relatively comparable sedentary time between 24PAR and SWA. Overweight/obese and/or older individuals were more likely to under-estimate sedentary time than normal weight and/or younger individuals. Measurement errors of 24PAR varied by the level of sedentary time and demographic indicators. This evidence informs future work to develop measurement error models to correct for errors of self-reports.
