[Ab] Resumo:Optical coherence tomography (OCT) has generally been used as a nondestructive technique to evaluate integrities of composite restorations. We investigated marginal and internal adaptations of ceramic inlay restorations with OCT and compared them to results with the silicone replica technique. Round-shaped class I cavities were prepared on 16 human maxillary first premolar teeth. Ceramic inlays were fabricated. Silicone replicas from inlays were obtained and sectioned to measure marginal and internal adaptations with a stereomicroscope (Leica Dfc 295, Bensheim, Germany). Inlays were cemented on respective teeth. Marginal and internal adaptations were then measured with the OCT system (Thorlabs, New Jersey, USA) in 200- µm intervals. Replica and OCT measurements were compared with independent samples t-tests. A paired t-test was used to evaluate the marginal and internal adaptations of each group (p < 0.05). Marginal and internal adaptations were 100.97 ± 31.36 and 113.94 ± 39.75 µm, respectively, using the replica technique and 28.97 ± 17.86 and 97.87 ± 21.83 µm, respectively, using OCT. The differences between the techniques were significant (p = 0.00 and p = 0.01, respectively). On evaluation within the groups, internal adaptation values were found to be significantly higher than the marginal adaptation values for the replica technique (p = 0.00) and OCT (p = 0.00). Therefore, the replica and OCT techniques showed different results, with higher values of marginal and internal adaptation found with the replica technique. Marginal and internal adaptation values of ceramic inlays, whether measured by replica or OCT techniques, were within clinically acceptable limits.

Hand searching of reference lists only.Study selectionRandomised controlled trials with a minimum of three years follow-up that compared direct to indirect inlays or onlays in posterior teeth. Primary outcome was failure (the need to replace or repair).Data extraction and synthesisTwo reviewers independently and in duplicate performed the study selection and two extracted data independently using a customised data extraction form. The unit of analysis was the restored tooth. Risk of bias was assessed using the Cochrane Risk of Bias tool. Meta-analysis was conducted on two studies using the random-effects model.ResultsThree studies were included. Across these studies there were 239 participants in whom 424 restorations were placed. Two studies compared direct and indirect inlays and had follow-up of five and 11 years respectively. One study compared direct and indirect onlays with a follow-up of five years. The studies were at unclear or high risk of bias. For direct and indirect inlays, Relative Risk (RR) of failure after five years was 1.54 (95% Cl: 0.42, 5.58; p = 0.52) in one study and, in another was 0.95 (95% Cl: 0.34, 2.63; p = 0.92) over 11 years. For onlays there was also no statistically-significant difference in survival, though overall five-year survival was 87% (95% CI: 81-93%).ConclusionsThere is insufficient evidence to favour the direct or indirect technique for the restoration of posterior teeth with inlays and onlays.

[Ab] Resumo:The objective of this follow-up study was to examine the performance of Cerec 1 inlays and onlays in terms of clinical quality over a mean functional period of 25 years. Out of 200 Cerec 1 inlays and onlays placed consecutively in 108 patients in a private practice between 1989 and early 1991, 141 restorations could be reevaluated in 65 patients after up to 26 years and 10 months. All ceramic inlays and onlays had been generated chairside using the Cerec 1 method, and had been seated adhesively using bonding composite. At follow-up examinations, the restorations were classified based on modified United States Public Health Service (USPHS) criteria. According to the Kaplan-Meier analysis, the success rate of Cerec 1 inlays and onlays dropped to 87.5% after up to 27 years. In 19 patients, a total of 23 failures were found. Of these failures, 78% were caused by either ceramic fractures (65%) or tooth fractures (13%). The reasons for the remaining failures were caries (18%), and endodontic problems (4%). Three-surface restorations had a significantly higher failure risk (P < 0.05) than one-, two-, and four-surface restorations, and restorations in premolars presented a lower failure risk than those in molars. The survival probability of 87.5% after up to 27 years of clinical service of Cerec 1 computer-assisted design/computer-assisted manufacturing (CAD/CAM) restorations made of Vita Mark I feldspathic ceramic proved to be highly acceptable in private practice.

[Ab] Resumo:PURPOSE: Intrasinus new bone formation (BF) has been observed after no-grafting osteotome sinus augmentation, and it is hypothesized to be influenced by the dimensions of the maxillary sinus. The aim of this clinical trial is to evaluate the influence of lateral-medial sinus width (SW) on no-grafting osteotome sinus augmentation outcomes using cone-beam computed tomography. PATIENTS AND METHODS: All patients recruited for this prospective study were treated with no-grafting osteotome sinus augmentation with simultaneous implant placement. Cone-beam computed tomography was obtained before, immediately after, and 6 months after the surgical procedure to use for measurements. Descriptive statistics were calculated and univariate, bivariate, and multivariate analysis were conducted to evaluate the influence of average SW and other relevant factors on procedure outcomes, including new BF, residual bone resorption (BR), and change of peri-implant bone height (CPBH). RESULTS: A total of 48 implants placed in 32 elevated sinuses of 29 patients were included. The average SW was 11.3 ± 1.8 mm. Intrasinus BF measured 1.7 ± 0.9 mm at 6 months after surgery. The amount of BR was 0.3 ± 0.9 mm, and CPBH was calculated as 1.3 ± 1.3 mm. Multivariate analysis showed a negative correlation between SW and BF (r = -0.469, P = .001), as well as between SW and CPBH (r = -0.562, P = .001). A positive correlation was discovered between SW and BR (r = 0.311, P = .027) in general. CONCLUSIONS: The lateral-medial SW was observed to have a negative correlation with new BF and CPBH after no-grafting osteotome sinus augmentation.

[Ab] Resumo:To examine the bonding state of metal-free CAD/CAM onlay restorations made from two popular resin composite blocks and a typical glass-ceramic block after cyclic loading, with and without immediate dentin sealing (IDS). Standardized mesial-distal-occlusalbuccal (MODB) cavities in 24 extracted human molars were prepared. The intra-cavity dentin surfaces of half of the cavities were immediately sealed with all-in-one adhesive and flowable composite, while those of the other half were not. All cavities were scanned, from which CAD/CAM onlays were fabricated from three types of block and cemented with an adhesive resin cement system. The restored specimens were subjected to cyclic loading and the intra-cavity microtensile bond strength was measured. IDS improves not only the internal bond strength, but also the bond reliability of metal-free CAD/CAM onlay restorations. The resin composite block seems to be more effective than a typical glass-ceramic block for achieving both high bond strength and excellent bond reliability.

[Ab] Resumo:The aims of this study were to evaluate the feasibility of 70% reduced inlay and 4-unit bridge models of International Standard (ISO 12836) assessing the accuracy of laboratory scanners to measure the accuracy of intraoral scanner. Four intraoral scanners (CS3500, Trios, Omnicam, and Bluecam) and one laboratory scanner (Ceramill MAP400) were used in this study. The height, depth, length, and angle of the models were measured from thirty scanned stereolithography (STL) images. There were no statistically significant mean deviations in distance accuracy and precision values of scanned images, except the angulation values of the inlay and 4-unit bridge models. The relative errors of inlay model and 4-unit bridge models quantifying the accuracy and precision of obtained mean deviations were less than 0.023 and 0.021, respectively. Thus, inlay and 4-unit bridge models suggested by this study is expected to be feasible tools for testing intraoral scanners.

The aim of this study was to compare to fracture resistance test of inlay restorations prepared using direct inlay technique (Gradia® Direct Composite) and Indirect Restoration System® (Gradia Indirect Composite) and CAD/CAD system (Vita Enamic® Block). . 48 noncarious extracted maxillary second primary molars were randomly divided into 4 groups with 12 in each group. All the teeth were prepared based on inlay class II preparations except for the control group. Other groups were restored with Gradia Direct Composite, Gradia Indirect Composite, and Vita Enamic Block, respectively. All restorations were cemented self-adhesive dual cure resin (3M Espe, RelyX™ Unicem Aplicap). A fracture test was performed using a compressive load. Results were analyzed using one-way analysis of variance and Duncan's post hoc multiple comparison tests ( = 0.05). . Vita Enamic Block and Gradia Indirect Composite showed significantly higher fracture resistance than Gradia Direct Composite ( < 0.05). There was no significant difference fracture resistance between Vita Enamic Block and Gradia Indirect Composite ( > 0.05). All restorations tested led to a significant reduction in fracture resistance ( < 0.05). In inlay restorations, Indirect Restoration Systems and CAD/CAM systems were applied successfully together with the self-adhesive dual cure resin cements in primary molars.

[Ab] Resumo:AIM: This in vitro study compares a new system for manual chair side fabrication of indirect composite restorations, which uses silicone models after alginate impressions, to CAD/CAM-technology and laboratory manual production techniques. MATRIALS AND METHODS: and study design Each 10 composite inlays were fabricated using different types of production techniques: CAD/CAM- technology (A), the new inlay system (B), plaster model after alginate impression (C) or silicone impression (D). The inlays were adapted into a metal tooth and silicone replicas of the cement gaps were made and measured. Statistical analysis was performed using ANOVA and Tukey's test. Results and Statistics In group A the biggest marginal gaps (174.9µm ± 106.2µm) were found. In group B the gaps were significantly smaller (119.5 µm ± 90.6 µm) than in group A (p=0.035). Between groups C (64.6 µm ± 68.0µm) and D (58.2 µm ± 61.7 µm) no significant differences could be found (p=0.998), but the gaps were significantly smaller compared with group B. Conclusion Chairside manufacturing of composite inlays resulted in better marginal precision than CAD/CAM technology. In comparison to build restorations in a laboratory, the new system is a timesaving and inexpensive alternative. Nevertheless, production of indirect composite restorations in the dental laboratory showed the highest precision.
