[Ab] Resumo:PURPOSE: To show the influence of retentive cavity, cavity wall preparation and different luting techniques on the fracture resistance of severely damaged teeth restored with adhesive core build-ups and monolithic zirconium crowns. METHODS: Extracted molars were prepared with 2 mm ferrule height and divided into eleven groups (n = 8/group). In nine groups a retentive occlusal cavity with a width and depth of 1 or 2 mm was prepared. Two control groups without a retentive cavity were made. Zirconium crowns were manufactured. 48 copings were cemented with glass-ionomer cement (Ketac Cem), the others (n = 40) with adhesive resin cement (Panavia F 2.0). Artificial ageing was carried out in the following way: n = 88, thermocycling (10,000 cycles, 6° C/60° C), n = 80 chewing simulation (1,200,000 cycles, 64 N). The samples were tested for load at first damage and fracture load with non-axial force. For statistical analysis ANCOVA with post hoc, Bonferroni-adjusted t-test were used ( p ≤ 0.05). RESULTS: No differences between the tested cements were detected. Influence of the cavity wall thickness was significant ( p = 0.001). Mostly, the samples with wall thickness of 2 mm showed better results. Both control groups (no cavity) showed results comparable to study groups with cavity. CONCLUSIONS: Retentive cavity is most likely not mandatory. However, if prepared, the cavity wall thickness is of higher importance than cavity depth. Glass-ionomer and adhesive resin cement are comparable for use with zirconia crowns.

[Ab] Resumo:The aim of the study was to compare accuracy of digital impression's finishing line and the zone under it taken by different intraoral scanning systems. Parameters of comparison were: different level of the finishing line to the gingiva and width of sulcus after retraction. For this purpose two synthetic jaw models with prepared teeth were scanned using intraoral scanning systems: 3D Progress (MHT S.P.A., IT - MHT Optic Research AG, CH); True Definition (3M ESPE, USA); Trios (3Shape A/S, DNK); CEREC AC Bluecam, CEREC Omnicam (Sirona Dental System GmbH, DE); Planscan (Planmeca, FIN) (each n=10). Reference-scanning was done by ATOS Core (GOM mbH, DE). The resulting digital impressions were superimposed with the master-scan. The lowest measured deviations (trueness) for intraoral scanners, where the finishing line was 0.5 mm above gingiva were with scanner True Definition - 18.8±6.63 (on the finishing line) and 51.0±14.33 µm (0.3 mm under the finishing line). In conditions where finishing line was on the same level with gingiva, scanner Trios showed the best results: 17.0±3.96 and 52.7±6.52 µm. When the finishing line was 0.5 mm under gingiva, none of the testing scanners could visualize the zone 0.3 mm lower the finishing line. The best results for accuracy o the finishing line in that circumstances showed Trios: 15.1±5.05 µm. The optimum visualization of the finishing line and the zone under it was reached when the sulcus was 0.3 mm after retraction. Thus, the best accuracy was obtained with Trios: 10.3±2.69 (on the finishing line) and 57.2±13.58 µm (0.3 mm under finishing line). The results show that intraoral scanners also provide enough accuracy for indicating finishing line and the zone under it in different conditions of preparation and gingiva retraction. However, not all of the testing scanners can properly indicate finishing line and the zone under it when shoulder is below gingiva and the width of sulcus is less than 0.2 mm.

[Ab] Resumo:PURPOSE: To evaluate the 3-D surface topography of ultrasonically prepared tooth. METHODS: Ten extracted human maxillary premolars were prepared for crowns. A split-tooth model was used to refine the margins: half of the margin was finished with traditional rotary instruments, the other with ultrasonic instruments. The 3-D profiles of the prepared surface were observed under scanning electron microscopy and Talysurf CCI 3000 non-contact 3D profiler, and a quantitative comparison of 3-D surface roughness average(SA) and the surface of skewness(SSK) was conducted with Talymap Gold software. The data were analyzed by independent sample t test using SPSS13.0 software package. RESULTS: The surface finished with ultrasonic instruments exhibited a smoother surface. 3-D surface roughness analysis showed that the SA and SSK of surfaces made by ultrasonic instruments were significantly lower, compared with surfaces produced using conventional rotary instruments. CONCLUSIONS: The ultrasonic instruments produce surface in better condition than the conventional rotary instruments.

In the current article, the author describes common issues that he has encountered before, during, and after All-on-4-style dental implant treatment of more than 500 arches and discusses methods for overcoming these obstacles.

[Ab] Resumo:Although traditional tooth preparation techniques (e.g., depth-groove-guided and index-guided techniques) are designed to improve preparation precision, the results are unsatisfactory because of the lack of proper estimating tools. This study proposed a novel technique, in which relevant details for preparation of drilling holes are provided and corresponding depth is estimated using a quantitive bur under a microscope. This technique offers a viable option for precise tooth preparation.

[Ab] Resumo:Target restoration space (TRS) is the most precise space required for designing optimal prosthesis. TRS consists of an internal or external tooth space to confirm the esthetics and function of the final restoration. Therefore, assisted with quantitive analysis transfer, TRS quantitative analysis is a significant improvement for minimum tooth preparation. This article presents TRS quantity-related measurement, analysis, transfer, and internal relevance of three TR. classifications. Results reveal the close bond between precision and minimally invasive treatment. This study can be used to improve the comprehension and execution of precise esthetic prosthodontics.

[Ab] Resumo:OBJECTIVE: To determine if full crown preparation causes an increase in pulpal blood flow (PBF), indicating inflammation, in human subjects. DESIGN: The experiments were carried out on 35 intact, mandibular posterior teeth in 13 subjects: 32 were abutments for 16 fixed bridges that replaced first molars; the other 3 were first premolars adjacent to abutment teeth that served as un-operated controls. Crown preparations were made using an air-rotor with water-spray under regional block anaesthesia (4% articaine with epinephrine 1:100,000). PBF was recorded with a laser Doppler flow meter (LDF) before and after administering the anaesthetic, with the LDF probe on the buccal enamel. PBF was then recorded from the abutment teeth with the probe on buccal dentine after preparing the buccal surfaces of both teeth, after completing the crown preparations, and after 1 and 7days. PBF was also recorded from the buccal enamel of the control teeth on each occasion. RESULTS: The mean±S.D. PBF values before and after anaesthesia were 2.63±2.13 and 2.42±2.38P.U. respectively, which were not significantly different (Paired t-test). The mean values for the abutment teeth after buccal preparation, after complete crown preparation, and after 1 and 7days were 5.20±2.49, 4.53±2.52, 4.92±2.98 and 5.48±2.65P.U. respectively. The 4 values for each tooth were not significantly different (two-way RM ANOVA). In the control group, the values under all six conditions were not significantly different. CONCLUSIONS: Regional block anaesthesia produced no change in PBF, nor did full-crown preparation, neither immediately after the procedure nor 1 and 7days later.

[Ab] Resumo:Cuspal-coverage restorations are important to preserve the integrity of a weakened tooth against the forces of occlusion. This article discusses the clinical indications for both direct and indirect cuspal-coverage restorations and the evidence supporting their use. Factors that modify a tooth's ability to dissipate normal occlusal forces and the effect cuspal-coverage restorations have on force distribution are examined. Clinical criteria, choice of restorative material and methods for tooth preparation are also discussed. CPD/Clinical Relevance: Re-enforcement of weakened teeth with cuspal-coverage restorations provides a minimally invasive alternative to conventional crowns.
