Título:Patients' demographics of a convenient clinic located in a large railway station in metropolitan Tokyo area.

Resumo:Hidden barriers to visit a medical facility especially for young busy workers have been neglected in the aging society. The aim of this cross-sectional study is to analyze demographics of patients who had visited the first known convenient clinic located inside a railway station, which is adjusted to the lifestyle of working generations.We analyzed de-identified data of patients who had visited the department of internal medicine of a clinic, which is located inside a railway station building and offers primary care with after-hours accessibility in Tokyo, between August 2013 and June 2016. Data were collected on patients' sex, age, time of visit, waiting time, presence or absence of an appointment, diagnosis, and patients' addresses using the electronic health and billing records.Overall, 28,001 patients visited 87,126 times. Number of visits increased in winter season compared with the other seasons. Sixty-one percent were women and the median age of all patients was 38 years (range, 0-102). The number of visits on Mondays was the highest in a week and the most frequent visiting time was between 6 and 7 p.m. The number of visits of working generations (from 15 to 65 years old) and men increased after 6 p.m. and on weekends. The 3 most common diagnoses were upper respiratory tract infection (22,457), allergic rhinitis (20,916), and hypertension (4869). The number of individuals who were referred to other medical institutions was 1022 (1.2%). The median waiting time was 748 seconds (range, 2-5344). The number of visits from within 2-, 5-, and 10-mile radius from our clinic was 41,696 (50.6%), 63,190 (76.7%), and 75,015 (91.1%), respectively, and patients' addresses were mainly located along the railway network.The locational and temporal convenience of our clinic has attracted the unmet medical demands especially for young workers who have difficulty in visiting conventional medical institutions.

Resumo:BACKGROUND/AIM: Programmed cell death ligand 1 (PD-L1) expression is a predictive marker for immunotherapy effects in advanced non-small cell lung cancer (NSCLC), but its association with patient characteristics or specimens is controversial. We aimed to retrospectively analyze the association of PD-L1 expression with clinicopathological features of NSCLC patients. MATERIALS AND METHODS: The PD-L1 expression and clinicopathological features of NSCLC patients were assessed from January 2017 to June 2017 in the Tokyo Metropolitan Cancer and Infectious Diseases Centre, Komagome Hospital were reviewed (n=108). RESULTS: For PD-L1 expressions of 0% and >1%, multivariate analysis showed that lymph node sample results were associated with positive PD-L1 expression. Archival samples and high serum carcinoembryonic antigen (CEA) levels were associated with negative PD-L1 expression. Sample preservation time and CEA levels correlated with PD-L1 expression. CONCLUSION: Nodal metastasis, sample preservation time and CEA levels were associated with PD-L1 expression in NSCLC.

Título:[Serial Food Poisoning Outbreaks Caused by Norovirus-Contaminated Shredded Dried Laver Seaweed Provided at School Lunch, Tokyo, 2017].

Resumo:In February 2017, four food poisoning outbreaks occurred in Tokyo, involving ten schools. Shredded dried laver seaweed processed by a single food manufacturer in December 2016 was provided in common for the school meals that caused all four outbreaks. Of 4,209 persons exposed, 1,193 (28.3%) had symptoms of gastroenteritis. Norovirus (NoV) GII was detected in 207 (78.1%) of 265 cases by real-time RT-PCR. Thirty-one shredded dried laver seaweed samples were examined and seven (22.6%) of them were positive for NoV GII. PCR fragments of NoV ORF1/2 junction region (302 bp) from seven shredded dried laver seaweed samples and 20 clinical samples derived from the four outbreaks were sequenced. All of them displayed complete homology, and the genotype was classified as GII.17. A nearly full-length sequence (7,420 bp) of NoV RNA derived from a case was obtained by next-generation sequencer analysis and phylogenetic analysis indicated that this strain belongs to the same cluster as Hu/GII/JP/2015/GII.P17_GII.17/Kawasaki308. Thus, our investigation elucidated that the causative agent of these four serial food poisoning outbreaks was NoV GII.17 and the infectious source was a single batch of shredded dried laver seaweed. The water activity of the shredded dried laver seaweed was found to be 0.119 to 0.129. It was epidemiologically clarified that NoV does not lose infectivity for about two months even in the dry state. We conclude that a large diffuse outbreak of food poisoning caused by NoV GII.17 contamination of shredded dried laver seaweed had occurred in Tokyo. Our elucidation of the causative agent indicated that the food poisoning outbreaks in multiple areas of Japan, including Tokyo, during January to February 2017 were caused by the same contaminated food.

Resumo:Chlorinated polycyclic aromatic hydrocarbons (ClPAHs) receive increasing attention as hazardous pollutants in terms of the high environmental persistence and toxicities. Ambient concentrations of 24 ClPAHs and 24 PAHs were investigated at 14 sites in the Tokyo Bay area of Japan. Twelve of 18 ClPAH species were detected in air samples, in spite of small sampling volumes. Mean concentrations of total PAHs in gas and particle phases were 5400 and 1400 pg/m , and mean concentrations of total ClPAHs in gas and particle phases were 40 and 14 pg/m , respectively. The spatial distributions of both total ClPAH and PAH concentrations indicated heavy pollution at sites in industrial activity areas. Principal component analysis suggested that the dominant sources of gaseous and particulate ClPAHs differed substantially from each other. In particular, gaseous ClPAHs could be produced by specific sources different from those of particulate ClPAHs. However, the dominant sources of particulate ClPAHs could be the same as those of particulate PAHs, including industrial activities such as steel and gas-production plants and natural gas-fired power plants. The influences of spatial relationships among sampling sites were represented using a network analysis. The constructed network showed that ambient ClPAHs and PAHs were dominated by local rather than regional pollution, because there were weaker relationships among nearby sites. Finally, exposure risks for ClPAHs were dominated by 7-chlorobenz[a]anthracene, followed by 9-chlorophenanthrene and 6-chlorobenzo[a]pyrene, and total risk was ∼1/200 that of PAHs.

Resumo:The first domestic outbreak of dengue fever in Japan since 1945 was reported in Tokyo in 2014. Meanwhile, daily mean summer temperatures are expected to continue to rise world-wide. Such conditions are expected to increase the risk of an arbovirus outbreak at the 2020 Tokyo Olympic Games. To address this possibility, the present study compared estimates of the risk of infection by dengue, chikungunya, and Zika viruses in urban areas. To compare the risk of infection by arboviruses transmitted by Ae. albopictus mosquitoes, the reproduction number for each of three arboviruses was estimated under the environmental conditions associated with the 2014 dengue outbreak in Tokyo, and additionally under conditions assuming a daily mean temperature elevation of 2° C. For dengue, chikungunya, and Zika, the estimated distributions of R were independently fitted to gamma distributions yielding median R values of 1.00, 0.46, and 0.36, respectively. If the daily mean temperature were to rise from 28° C to 30° C, our model predicts increases of the median R of 18% for dengue, 4.3% for chikungunya, and 11.1% for Zika. Strengthening of the public health responsivity for these emerging arboviral diseases will be needed in preparation for the 2020 Olympic Games in Tokyo.

Título:Emergence of encounter networks due to human mobility.

Resumo:There is a burst of work on human mobility and encounter networks. However, the connection between these two important fields just begun recently. It is clear that both are closely related: Mobility generates encounters, and these encounters might give rise to contagion phenomena or even friendship. We model a set of random walkers that visit locations in space following a strategy akin to Lévy flights. We measure the encounters in space and time and establish a link between walkers after they coincide several times. This generates a temporal network that is characterized by global quantities. We compare this dynamics with real data for two cities: New York City and Tokyo. We use data from the location-based social network Foursquare and obtain the emergent temporal encounter network, for these two cities, that we compare with our model. We found long-range (Lévy-like) distributions for traveled distances and time intervals that characterize the emergent social network due to human mobility. Studying this connection is important for several fields like epidemics, social influence, voting, contagion models, behavioral adoption and diffusion of ideas.

Resumo:BACKGROUND/AIM: Few studies have evaluated the risk of postoperative colorectal neoplasms stratified by the nature of primary colorectal cancer (CRC). In this study, we revealed it on the basis of the microsatellite (MS) status of primary CRC. MATERIALS AND METHODS: We retrospectively reviewed 338 patients with CRC and calculated the risk of neoplasms during postoperative surveillance colonoscopy in association with the MS status of primary CRC. A propensity score method was applied. RESULTS: We identified a higher incidence of metachronous rectal neoplasms after the resection of MS stable CRC than MS instable CRC (adjusted HR 5.74, p=0.04). We also observed a higher incidence of colorectal tubular adenoma in patients with MSS CRC (adjusted hazard ratio 7.09, p<0.01) and a higher incidence of postoperative tubulovillous/villous adenoma in patients with MS instable CRC (adjusted HR=8.50, p=0.03). CONCLUSION: The MS status of primary colorectal cancer influenced the risk of postoperative colorectal neoplasms.

Resumo:Long-term climate changes related with urbanization in Tokyo, Japan, and recent temperature and heavy rainfall distribution in the Tokyo metropolitan area are reviewed. A relatively high temperature increase in annual mean temperature at the rate of 3.0°C/century was detected in Tokyo for the period 1901-2015. Some observational evidence showed the existence of both thermal and mechanical effects of urbanization on recent heavy rainfall occurrences, and modeling studies also support precipitation enhancement. Urban influences were recognized in other climatological elements, such as number of fog days, relative humidity, and wind circulation.

Resumo:Although recent findings of cancer biology research indicate that prognostic power arises from genes expressed by stromal cells rather than epithelial cells, desmoplastic reaction (DR) has not been completely examined as a prognostic marker for colorectal cancer. A pathologic review of 821 stage II and III patients who underwent R0 resection for colorectal cancer at 4 independent institutions was conducted. DR was classified as mature, intermediate, or immature based on the existence of hyalinized keloid-like collagen and myxoid stroma at the extramural desmoplastic front. Totally, 325, 282, and 214 patients were classified as having mature, intermediate, and immature DR, respectively. DR significantly influenced the recurrence rate in the liver, lung, and peritoneum (P≤0.0001 to 0.01). Five-year relapse-free survival (RFS) rate was the highest in the mature group (85.7%), followed by the intermediate (77.3%) and immature (50.4%) groups. A significant adverse impact of immature stroma on RFS was observed in subset analyses of the 4 institutions. Multivariate analysis revealed that DR, along with T and N stages, is an independent prognostic factor. On the basis of Harrell's concordance index, the prognostic power of DR categorization (0.67) in stratifying RFS was greater than any other conventional prognostic factors, including TNM (0.64), N (0.62) and T stages (0.59), venous invasion (0.59), and tumor grade (0.54). Characterizing DR based on the histologic products of activated fibroblasts is valuable for evaluating prognostic outcomes. To our knowledge, this is the first study reporting a greater prognostic power of histology of the fibrotic stroma than that of tumor factors.

Resumo:The landscape architect Maggie Jencks felt that she wanted a place to "get herself back" when her breast cancer recurred and was told that she only had a few months remaining. Mr. Charles Jencks, her husband who fulfilled her wishes, opened the first "Maggie's Cancer Caring Centre" in Edinburgh in 1996. Masako Akiyama, the chief of Maggie's Tokyo found out about Maggie's Centre when she listened to a presentation from Andrew Anderson, the director of Maggie's Edinburgh, at an International Nursing Seminar in 2008. Masako Akiyama realized that support to "get back ones own power" is one of the important things. In addition, she felt that providing a "place" and "support" to patients and their families were predominant elements. When someone is diagnosed with cancer, the person gets a great shock and feels loneliness. Suddenly the person feels tomorrow will not come, every day becomes uncertain. These feelings affect their whole life such as enrollment, employment, loss of role, and change. They also affect people such as family, friends, colleagues, besides the person. Due to changes in treatment, the existential troubles that have been prolonged after the completion of the initial treatment appear. Cancer survivorship is called "Cancer journey"in the Maggie's flow. The cancer journey suddenly starts, when a person is diagnosed with cancer", without direction or a plan. Additionally, the "heart"is often left behind. To undertake the cancer journey, there are a lot of forks in the road that cannot turn back and they are urged to choose their path. "Where now ?"is a phrase in a pamphlet from Maggie's Centre, and the phrase shows that staff of Maggie's Centre can help cancer patient to recognize where they are in their Cancer journey. The Maggie's Centre has 2 important pillars: "building/environment"and "human support". As a "homely place" between hospital and my house, the building is "a healing space". Human support is provided free of charge and patients and families can stop by without reservation. Experienced cancer specialists and medical staff are always available. Unlike previous supportive methods for patients such as giving information and helping to solve problems, Maggie's emphasizes the 3 points of "snuggling up to each individual", "equal footing" and "to cherish their likeness empowerment". By talking to Maggie's medical professionals, patients and families can find their "heart" and regain their original strength.
