Resumo: High blood pressure (hypertension) takes a toll on the heart and blood vessels. So does diabetes mellitus. When both are present in the same individual, it stands to reason that there must be an increase in the risk of cardiovascular problems. The management of a chronic condition such as hypertension in the presence of another chronic condition, requires the development of a close inter-relationship between the patient and the healthcare team. The education of the patient about conditions and their potential sequelae, and the empowerment of the patient - ensuring not only his/her acquisition of knowledge but access to 'tools' for monitoring - are important requirements in the overall management of this 'double whammy'! This education and awareness process must not be restricted to patients, healthcare givers, family and support groups. It must also embrace the wider public and decision makers at the political level, in order to ensure that the best quality of life is afforded these patients within their community and country (AU)

Resumo: Severe or resistant hypertension is fortunately not very common in the general population. When discovered, the practitioner must be alert to the fact that alcohol excess and commonly available over-the-counter drugs could be contribution factors. An adequate history would eliminate the need for unnecessary investigations and treatment. Physical examination should be focused upon target organ damage, which if identified should lead to prompt refferal to a specialist centre for treatment. Treatment should be aggressive yet controlled (AU)

Resumo: In clinical practice, the idea of treating hypertension should not be confined to lowering blood pressure alone. Hypertensive patients are often obese, diabetic, and sedentary. A 'global' perspective is therefore important. Such an approach will recognise the need to promote lifestyle modifications, including advice on a healthy diet, exercise, cessation of cigarette smoking, and reduction of alcohol intake. This article is not meant to be a treatise on hypertensive therapy - the aim is to highlight issues relevant to hypertension treatment in the Caribbean (AU)

Resumo: Objective: To apply a recently proposed model and assessment tool created by the authors for critically evaluating the data available on the prevalence of hypertension in LAC and assessing their usefulness for surveillance. Methods: A bibliographic search to identify all publications that estimated the prevalence of hypertension was performed. Each of the papers located was assessed using a critical appraisal tool. Results: Of the 58 studies published between 1966 and 2000, only 28 of them (48percent) met the critical threshold to be considered useful for surveillance purposes. The distribution of the 28 studies in terms of their usefulness for surveillance was as follows: minimally useful, 16 studies; useful, 8 studies; and very useful, 4 studies. Several methodological shortcomings were identified, from inadequate sampling procedures and sample size to the poor quality of the primary data for planning purposes. Discussion: Published studies on the prevalence of hypertension in Latin America and the Caribbean have, as a whole, limited usefulness for surveillance activities (AU)

Resumo: This study describes the burden of stroke on hospital services in a Caribbean community. The settings are the two main acute general hospitals in Trinidad observed over a 12-month period. All subjects were admitted with a clinical diagnosis of acute stroke. The measures were hospital admission rates, length of hospital stay, case-fatality rates, disability at discharge, and risk factors for stroke. There were 1 105 hospital admissions with a diagnosis of stroke. The median length of stay was 4 days, with an interquartile range of 2 to 9, and stroke accounted for approximately 9 478 bed days per annum. The hospital admission fatality rate was 29 percent. Among surviving patients, 437 (56 percent) were severely disabled at discharge. Age-standardized admission rates for first strokes in persons aged 35-64 years were 114 (95 percent CI: 83 to 145) per 100 000 in Afro-Trinidadian men and 144 (109 to 179) in Indo-Trinidadian men. The equivalent rates for women were 115 (84 to 146) and 152 (118 10186). Among patients with first strokes, 348/531 (66 percent) reported physician-diagnosed hypertension, but only 226 (65 percent) of these reported being on antihypertensives at admission. Stroke in Trinidad and Tobago is associated with a high case-fatality rate and severe disability in survivors. Modifiable risk factors were reported in a majority of stroke cases, and there is a need to develop effective preventive strategies (AU)
