Título:Association of parasites and diarrhoea among children less than five years of age in a rural area in Egypt.

Resumo:A case control study was conducted in Kafr El-Sheikh Governorate, Egypt, a rural community, to assess the relation of parasites and diarrhoea among children <5 years. A sample of 196 cases with diarrhoea and 83 non-diarrhoeic controls were included in the study. A questionnaire was completed for every child and weight and height were recorded. A stool sample was obtained and examined for parasites present. The results revealed that 70.3% of cases had parasites compared to 59% of controls. The most common parasites found were Entamoeba histolytica, cryptosporidium, and Giardia lamblia consecutively. In both groups the percent positive parasitic infection increased with age. The different socioeconomic and sanitary indicators studied were similar among both groups and could not explain the diarrhoea present except for the significantly higher presence of piped water inside houses of controls. Study of malnutrition revealed that a significantly higher percentage were stunted (15.6%) among cases than among controls (6.5%) and 13.8% were wasted among cases compared to only 2% among controls. Undernourished cases were 14.8% compared to 4.3% of controls.

Resumo:This study was conducted to improve asthmatic school children's knowledge, belief and behavior related to asthma in order to improve their control of the disease. A total of 54 school children suffering from moderate or severe asthma who are attending the allergy center affiliated to school Students' Hospital of Alexandria were enrolled in the present study. An educational intervention was provided to these asthmatic children, each one was accompanied by one of his/her parents. It consisted of two components: classes conducted in group settings and individualized monthly visits. The intervention continued for a 6-month period. An interview questionnaire was used to assess both children's and parent's knowledge and beliefs related to asthma before and after the intervention. The children's inhaler use technique was also assessed twice by an observational check list. The study revealed significant improvement in both children's and parent's knowledge and beliefs concerning asthma inhaler use technique which significantly improved and positive behavioral changes were reported. The frequency of emergency room visits (ERV) due to asthma over a 6-month period after the initial assessment was substantially less than in the prior 6-month period (p<0.05). It could be concluded that patient education can significantly affect asthmatic's knowledge and behavior and consequently morbidity as revealed by reduction of frequency of ERV. It should be considered one of the major components of asthma care.

Resumo:Recent advances in medical technology have led to a marked improvement in the chances of survival of sick or preterm infants, thereby stimulating renewed ethical debate on the status of the newborn. Two contradictory attitudes to the medical care of preterm or congenitally malformed newborn infants can be discerned in our pluralistic society. The two attitudes have their historical roots in the classical Graeco-Roman and Judaeo-Christian ethical traditions respectively. The former views newborn infants as of potential value only whereas the latter emphasises the intrinsic worth and dignity of the individual made in God's image. Recent secular philosophical reflection has provided a rationale for infanticide of the sick or abnormal newborn. A Christian approach to the care of the newborn prohibits intentional killing yet may encompass the withdrawal of treatment that is inappropriate or unduly burdensome. Medical care should be based upon respect for the value of the individual, protection of the defenceless from abuse or exploitation, and wise stewardship of limited health-care resources.

Resumo:The admission of a child to the intensive care unit (ICU) represents a traumatic experience for parents. They may feel bombarded with information, loss of control and subject to enormous stress. The psychological stress of their child being admitted to ICU may render parents incapable of rational decision-making. Parents may receive frequent requests for consent at a time when their mental competence to do so may be questioned. Parental competence is affected by: information giving, effective communication and the perception of the person evaluating. The purpose of assessment of mental competence is to ensure that informed consent takes place and the best interests of the child are protected.

Resumo:To date, most school-based research has used passive parental consent. However, the Family Privacy Protection Act of 1995 aims to change these requirements. The proposed legislation requires written parental consent if minors are to be asked [quot ]sensitive[quot ] questions as part of any program or activity funded in whole or in part by the federal government. This act is representative of a growing trend toward restricting research involving minors. Whether or not this act is passed by Congress, two lines of concern are highlighted by this legislation. The first deals with ethical issues surrounding consent procedures. For instance, are parental rights compromised when active consent is not mandated? A second line of inquiry pertains to the effect of active consent procedures on response rates and sample bias. In this article, the authors discuss ethical issues surrounding passive and active consent procedures and then report response rates from two projects in which active consent procedures were implemented.
